Abstract

This document discusses, among other things, systems and methods for providing pain relief to a patient. Recording circuitry may receive electrical signals corresponding to evoked compound action potentials in the patient that may be produced in response to external stimulation of a location where the patient is experiencing pain. The received electrical signals may be stored in a memory. Internal stimulation may then be applied to the patient and control circuitry may receive electrical signals corresponding to evoked compound action potentials in the patient that may be produced in response to the internal stimulation. The control circuitry may then adjust electrical parameters of the internal stimulation, such as to reduce a difference between the electrical signals corresponding to evoked compound action potentials produced in response to the internal stimulation and electrical signals corresponding to evoked compound action potentials produced in response to the external stimulation.

Claims (47)

1. A method, comprising:

stimulating an external location on a patient's body to evoke a first instance of nerve activity;

sensing a first evoked compound action potential (ECAP) signal indicative of the first instance of nerve activity at a sensing location within the patient's body;

determining first data corresponding to the first instance of nerve activity, wherein the first data includes a first instance of at least one ECAP feature from the first ECAP signal, wherein the at least one ECAP feature includes for a waveform portion one or more of: an amplitude of the waveform portion, a delay of the waveform portion, a width of the waveform portion, a length of a curve for the waveform portion, an area under the curve for the waveform portion, or a ratio of amplitudes within the waveform portion;

stimulating an internal location within the patient's body using at least one stimulation parameter, wherein stimulation of the internal location evokes a second instance of nerve activity;

sensing a second ECAP signal indicative of the second instance of nerve activity at the sensing location within the patient's body;

determining second data corresponding to the second instance of nerve activity, wherein the second data includes a second instance of the at least one ECAP feature from the second ECAP signal;

evaluating a relationship between the first data and the second data, including evaluating a signal-to-signal relationship between the first instance of the at least one ECAP feature from the first ECAP signal and the second instance of the at least one ECAP feature from the second ECAP signal;

adjusting the at least one stimulation parameter based on the relationship between the first data and the second data, including based on the signal-to-signal relationship between the first instance of the at least one ECAP feature and the second instance of the at least one ECAP feature; and

stimulating the internal location using the adjusted at least one stimulation parameter.

2. The method of claim 1 , wherein stimulating the external location includes thermally stimulating, electrically stimulating, chemically stimulating, optically stimulating or mechanically stimulating the external location.

3. The method of claim 2 , wherein stimulating the external location includes electrically stimulating the external location.

4. The method of claim 1 , wherein the signal-to-signal relationship is evaluated by comparing the first instance of the at least one ECAP feature to the second instance of the at least one ECAP feature.

5. The method of claim 1 , wherein the signal-to-signal relationship is evaluated by evaluating a similarity between first data points in the first ECAP signal and second data points in the second ECAP signal.

6. The method of claim 1 , wherein the signal-to-signal relationship is evaluated by evaluating a correlation between first data points in the first ECAP signal and second data points in the second ECAP signal.

7. The method of claim 1 , wherein the signal-to-signal relationship is evaluated by evaluating a difference between the first instance of the at least one feature and the second instance of the at least one feature.

8. The method of claim 1 , wherein the location within the patient's body where the first and second instances of nerve activity is sensed includes a dorsal horn.

9. The method of claim 1 , wherein the location within the patient's body where the first and second instances of nerve activity is sensed includes a dorsal column.

10. The method of claim 1 , wherein the location within the patient's body where the first and second instances of nerve activity is sensed includes a dorsal root.

11. A non-transitory machine-readable medium including instructions, which when executed by a machine, cause the machine to perform operations, comprising:

stimulating an external location on a patient's body to evoke a first instance of nerve activity;

sensing a first evoked compound action potential (ECAP) signal indicative of the first instance of nerve activity at a location within the patient's body;

determining first data corresponding to the first instance of nerve activity, wherein the first data includes a first instance of at least one ECAP feature from the first ECAP signal, wherein the at least one ECAP feature includes for a waveform portion one or more of:

an amplitude of the waveform portion, a delay of the waveform portion, a width of the waveform portion, a length of a curve for the waveform portion, an area under the curve for the waveform portion, or a ratio of amplitudes within the waveform portion;

stimulating an internal location within the patient's body using at least one stimulation parameter, wherein stimulation of the internal location evokes a second instance of nerve activity;

sensing a second ECAP signal indicative of the second instance of nerve activity at the location within the patient's body;

determining second data corresponding to the second instance of nerve activity, wherein the second data includes a second instance of the at least one ECAP feature from the second ECAP signal;

evaluating a relationship between the first data and the second data, including evaluating a signal-to-signal relationship between the first instance of the at least one ECAP feature from the first ECAP signal and the second instance of the at least one ECAP feature from the second ECAP signal;

adjusting the at least one stimulation parameter based on the relationship between the first data and the second data, including based on the signal-to-signal relationship between the first instance of the at least one ECAP feature and the second instance of the at least one ECAP feature; and

stimulating the internal location using the adjusted at least one stimulation parameter.

12. The non-transitory machine-readable medium of claim 11 , wherein the location within the patient's body where the first and second instances of nerve activity is sensed includes a dorsal horn, a dorsal column, or a dorsal root.

13. The non-transitory machine-readable medium of claim 11 , wherein the signal-to-signal relationship is evaluated by comparing the first instance of the at least one ECAP feature to the second instance of the at least one ECAP feature.

14. The non-transitory machine-readable medium of claim 11 , wherein the signal-to-signal relationship is evaluated by evaluating a similarity between first data points in the first ECAP signal and second data points in the second ECAP signal.

15. The non-transitory machine-readable medium of claim 11 , wherein the signal-to-signal relationship is evaluated by evaluating a correlation between first data points in the first ECAP signal and second data points in the second ECAP signal.

16. The non-transitory machine-readable medium of claim 11 , wherein the signal-to-signal relationship is evaluated by evaluating a difference between the first instance of the at least one feature and the second instance of the at least one feature.

17. A system, comprising:

external stimulation circuitry configured for stimulating an external location on a patient's body to evoke a first instance of nerve activity within the patient's body;

internal stimulation circuitry configured for stimulating an internal location within the patient's body using at least one stimulation parameter, wherein stimulation of the internal location evokes a second instance of nerve activity within the patient's body;

sensing circuitry configured for sensing a first evoked compound action potential (ECAP) signal indicative of the first instance of nerve activity at a sensing location within the patient's body and a second electrical signal indicative of the second instance of nerve activity at the sensing location within the patient's body; and

control circuitry configured for determining first data corresponding to the first instance of nerve activity, wherein the first data includes a first instance of at least one ECAP feature from the first ECAP signal and determining second data corresponding to the second instance of nerve activity, wherein the second data includes a second instance of the at least one ECAP feature from the second ECAP signal, evaluating a relationship between the first data and the second data, including evaluating a signal-to-signal relationship between the first instance of the at least one ECAP feature from the first ECAP signal and the second instance of the at least one ECAP feature from the second ECAP signal, adjusting the at least one stimulation parameter based on the relationship between the first data and the second data, including based on the signal-to-signal relationship between the first instance of the at least one ECAP feature and the second instance of the at least one ECAP feature, and controlling the internal stimulation circuitry to stimulate the internal location using the adjusted at least one stimulation parameter.

18. The system of claim 17 , wherein the location within the patient's body where the first and second instances of nerve activity is sensed includes a dorsal horn, a dorsal column, or a dorsal root.

19. The system of claim 17 , wherein the external stimulation circuitry is configured to electrically stimulate the external location.

20. The system of claim 17 , wherein the signal-to-signal relationship is evaluated by:

comparing the first instance of the at least one ECAP feature to the second instance of the at least one ECAP feature;

evaluating a similarity between first data points in the first ECAP signal and second data points in the second ECAP signal;

evaluating a correlation between first data points in the first ECAP signal and second data points in the second ECAP signal; or

evaluating a difference between the first instance of the at least one feature and the second instance of the at least one feature.
