Abstract

Systems and methods for preventing the seeding of cancerous cells during morcellation of a tissue specimen inside a patient's body and removal of the tissue specimen from inside the patient through a minimally-invasive body opening to outside the patient are provided. One system includes a cut-resistant tissue guard removably insertable into a containment bag. The tissue specimen is isolated and contained within the containment bag and the guard is configured to protect the containment bag and surrounding tissue from incidental contact with sharp instrumentation used during morcellation and extraction of the tissue specimen. The guard is adjustable for easy insertion and removal and configured to securely anchor to the body opening. Protection-focused and containment-based systems for tissue removal are provided that enable minimally invasive procedures to be performed safely and efficiently.

Claims (25)

1 . A system for safely removing a tissue specimen through a body opening, the body opening defining a tissue margin, the system comprising:

a retractor having a first ring and a second ring interconnected by a sidewall defining a central lumen having a top opening and a bottom opening; the second ring being compressible into a low-profile configuration for insertion through the body opening and the first ring having a first ring diameter; the retractor being configured such that the sidewall can be rolled up around the first ring to reduce a length of the retractor and to retract the tissue margin;

a shield made of cut-resistant material including a proximal end and a distal end interconnected by a sidewall; the shield having an inner surface and an outer surface and an elongate tubular-shaped working channel extending from the proximal end to the distal end along a longitudinal axis; the shield having a proximal flange at the proximal end that extends radially outwardly from the longitudinal axis to define an outer diameter at the perimeter of the proximal end;

wherein the shield is sized and configured to be removably connected to the retractor when the retractor is placed across the body opening and the shield is inserted into the central lumen of the retractor;

wherein the shield is split from the proximal end to the distal end to define a first end and a second end; and

wherein each of the first end and the second end of the shield includes an S-shaped curvature configured to overlap onto an outer surface of an adjacent shield portion when in a relaxed configuration.

2 . The system of claim 1 wherein the proximal end of the shield is configured to connect to the first ring of the retractor.

3 . The system of claim 1 wherein the proximal end of the shield is configured to connect to the first ring of the retractor by snapping the proximal flange under the first ring.

The system of claim 1 wherein the proximal flange forms a funnel-like entryway into the working channel of the shield; the proximal flange serving as a protective surface to guard the tissue margin, the retractor and/or a containment bag while providing a cutting-board surface to cut and reduce the tissue specimen.

5 . The system of claim 1 wherein in cross-sections taken perpendicular to the longitudinal axis, a size of the working channel of the shield increases with distance toward the distal end of the shield.

6 . The system of claim 1 wherein cross-sections of the shield taken perpendicular to the longitudinal axis have a circular, elliptical, or elongate oval/oblong shape.

7 . The system of claim 1 wherein the shield is radially adjustable to increase or decrease a size of the working channel.

8 . The system of claim 1 wherein the outer surface of the shield is concave along a length of the shield and flares progressively radially outwardly from an inflection point toward the distal end and proximal end of the shield.

9 . The system of claim 8 wherein the inflection point is located near the proximal flange above a mid-plane taken perpendicular to the longitudinal axis.

The system of claim 1 wherein the relaxed configuration includes a relaxed lateral dimension that is perpendicular to the longitudinal axis; wherein the shield further comprises a reduced configuration and an enlarged configuration; the shield is movable between the relaxed configuration and the reduced or enlarged configuration; the reduced configuration having a reduced lateral dimension that is smaller than the relaxed lateral dimension and the enlarged configuration having an enlarged lateral dimension that is larger than the relaxed lateral dimension.

11 . The system of claim 10 wherein the shield is molded with a bias towards the relaxed configuration such that when a size of the working channel is increased or decreased, the shield tends to spring back towards the relaxed configuration.

12 . The system of claim 1 wherein the shield is formed from a band of material having the first end and the second end interconnected between the proximal end and the distal end.

13 . The system of claim 12 wherein the band of material comprises a hard, rigid, or semi-rigid, or other cut-resistant material.

14 . The system of claim 1 wherein the S-shaped curvatures of the first end and the second end are each configured to transition into a respective notch near the proximal end and distal end of the shield, respectively.

15 . The system of claim 14 wherein the notches form finger-like extensions configured to mate with each other, thereby serving as a locking mechanism to fix a size of the working channel in a diametrical dimension.

The system of claim 1 wherein the shield includes a distal flange having a funnel-like shape with a progressively increasing radial dimension at the distal end; the distal flange extending radially outwardly from the longitudinal axis to define an inner diameter at the perimeter of the distal end.

The system of claim 1 further including a containment bag having an interior and a mouth for accessing the interior of the containment bag; the containment bag having a proximal end and distal end and is sized and configured to receive the retractor and the shield inside the containment bag such that the retractor is located between the shield and the containment bag.

The system of claim 1 further including a containment bag having an interior and a mouth for accessing the interior of the containment bag; the containment bag having a proximal end and distal end and is sized and configured to receive the shield inside the containment bag such that the containment bag is located between the shield and the retractor.

19 . The system of claim 1 wherein at least a portion of the shield overlaps onto itself to define an overlapping portion when in the relaxed configuration.

20 . The system of claim 19 wherein the shield is diametrically adjustable by increasing or decreasing a length of the overlapping portion to increase or decrease a size of the working channel, respectively.
