Robotically controlled clot manipulation and removal
Certain aspects relate to systems, devices, and techniques for clot manipulation and removal. At least some of the devices for clot manipulation and removal can be robotically controlled. These devices can include one or more elongate members that can be robotically driven through a patient's vasculature. One such device can include a first elongate member that can serve as an access sheath, a second elongate member that can serve as a clot removal catheter, a third elongate member that can serve as a clot disruptor, and a fourth elongate member that can serve as a guidewire.
1 . A method of clot removal comprising:
driving a robotically controlled instrument system toward a clot within vasculature of a patient, wherein the instrument system comprises a first elongate member and a second elongate member positioned coaxially with the first elongate member, the first elongate member having a greater number of articulation cables than the second elongate member;
piercing the clot using a blunt distal tip of a third elongate member;
reducing the clot to smaller particles using a clot cutter of the third elongate member;
deploying a distal guard along a length of the third elongate member that is downstream from a location of the clot to prevent the smaller particles from being distributed downstream in the patient;
applying a pump of the second elongate member to aspirate the clot; and
capturing the clot using a retriever of the instrument system.
2 . The method of claim 1 , further comprising driving the third elongate member within the vasculature of the patient.
3 . The method of claim 1 , wherein the first elongate member and the second elongate member are driven in tandem.
4 . The method of claim 1 , further comprising displaying, on a display screen, a representation of the vasculature and a representation of the instrument system.
5 . The method of claim 4 , further comprising displaying, on the display screen, an indicator of a concentricity of the instrument system within the vasculature of the patient.
6 . The method of claim 1 , further comprising obtaining, by a processor system, information related to one or more of a location of an occlusion or a severity of blockage by the clot.
7 . The method of claim 1 , the blunt distal tip comprising a blade.
8 . The method of claim 1 , further comprising driving a fourth elongate member within the vasculature of the patient.
9 . The method of claim 8 , the fourth elongate member comprising a guidewire.
10 . The method of claim 1 , further comprising occluding a vessel using an inflatable member of the first elongate member.
