Abstract

Methods and apparatus employed in surgery involving making precise incisions in vessels of the body, particularly cardiac blood vessels in coronary revascularization procedures conducted on the stopped or beating heart are disclosed. Such incisions are created by applying an elongated electrosurgical cutting electrode to the outer surface of the vessel wall in substantially parallel alignment with the body vessel axis, the elongated electrosurgical cutting electrode having a predetermined cutting electrode length exceeding the cutting electrode width. RF energy is applied between the electrosurgical cutting electrode and the ground electrode at an energy level and for a duration sufficient to cut an elongated slit through the vessel wall where the elongated electrosurgical cutting electrode is applied to the surface of the vessel wall.

Claims (46)

1. An electrosurgical vessel wall cutting device adapted to be placed in contact with the body of a patient and coupled to a source of RF energy for making an elongated slit through a vessel wall and into a lumen of a body vessel, having a vessel axis, of a patient that is surgically accessed in a surgical operating field comprising:

a ground electrode adapted to be coupled with the source of RF energy;

an elongated tool body extending between a tool body proximal end and a tool body distal end;

a connector element supported by the tool body adapted to be coupled with the source of RF energy;

an elongated electrosurgical cutting electrode supported at the tool body distal end, the elongated electrosurgical cutting electrode having a predetermined cutting electrode length exceeding the cutting electrode width; and

an electrical conductor within the tool body extending between and coupled with the connector element and the elongated electrosurgical cutting electrode, whereby the elongated tool body is adapted to be manipulated to align and apply the cutting electrode length of the elongated electrosurgical cutting electrode against a surface of the vessel wall in substantially parallel alignment with the vessel axis so as to apply RF energy between the electrosurgical cutting electrode and the ground electrode at an energy level and for a duration sufficient to cut an elongated slit through the vessel wall where the elongated electrosurgical cutting electrode is applied to the surface of the vessel wall, wherein the ground electrode further comprises an exposed conductive ground electrode of an electrically insulated ground wire extending between a ground wire proximal end and a ground wire distal end adapted to be introduced into the lumen of the body vessel to dispose the ground electrode in proximity with the elongated electrosurgical cutting electrode applied to an outer surface of the vessel wall.

The electrosurgical vessel wall cutting tool of claim 1 , wherein at least a section of the elongated tool body is malleable to enable formation of a bend in the elongated axis of the tool body to allow the elongated electrosurgical cutting electrode to be placed in alignment with the axis of the body vessel and in contact with the surface of the vessel wall.

3. The electrosurgical vessel wall cutting tool of claim 1 , wherein the elongated electrosurgical cutting electrode is supported by a ball and socket mechanism of the tool body.

4. The electrosurgical vessel wall cutting tool of claim 1 , further comprising a mechanism for controlling the depth of the elongated slit cut into the vessel wall.

5. The electrosurgical vessel wall cutting tool of claim 4 , wherein the mechanism for controlling the depth of the elongated slit cut into the vessel wall shuts off the application of RF energy when a selected cut depth is acheived.

6. The electrosurgical vessel wall cutting tool of claim 4 , wherein the mechanism for controlling the depth of the elongated slit cut into the vessel wall comprises one or more sensing electrodes.

7. The electrosurgical vessel wall cutting tool of claim 4 , wherein the mechansim for controlling the depth of the elongated slit cut into the vessel wall is adjustable.

An electrosurgical vessel wall cutting tool adapted to be employed with a ground electrode in contact with the body of a patient and coupled to a source of RF energy for making an elongated slit through a vessel wall and into a lumen of a body vessel, having a vessel axis, of a patient that is surgically accessed in a surgical operating field comprising:

an elongated tool body extending between a tool body proximal end and a tool body distal end;

a connector element supported by the tool body adapted to be coupled with the source of RF energy;

an elongated electrosurgical cutting electrode supported at the tool body distal end, the elongated electrosurgical cutting electrode having a predetermined cutting electrode length exceeding the cutting electrode width;

an electrical conductor within the tool body extending between and coupled with the connector element and the elongated electrosurgical cutting electrode, whereby the elongated tool body is adapted the manipulated to align and apply the cutting electrode length of the elongated electrosurgical cutting electrode against a surface of the vessel wall in substantially parallel alignment with the vessel axis so as to a apply RF energy between the electrosurgical cutting electrode and the ground electrode at an energy level and for a duration sufficient to cut an elongated slit through the vessel wall where the elongated electrosurgical cutting electrode is applied to the surface of the vessel wall, wherein the body vessel is supported by body tissue and further comprising a stop member formed to extend away from the tool body distal end that limits the advancement of the elongated electrosurgical cutting electrode into the vessel lumen when the stop bears against the body tissue supporting the body vessel, wherein the elongated electrosurgical cutting electrode is supported by the tool body to nominally extend at a predetermined angle to an elongated axis of the tool body; and

at least a section of the elongated tool body is malleable to enable formation of a bend in the elongated axis of the tool body to adjust the predetermined angle to allow the elongated electrosurgical cutting electrode to be placed in alignment with the axis of the body vessel and in contact with the surface of the vessel wall.

The electrosurgical vessel wall cutting tool of claim 8 , wherein the ground electrode further comprises an exposed conductive ground electrode of an electrically insulated ground wire extending between a ground wire proximal end and a ground wire distal end adapted to be introduced into the lumen of the body vessel to dispose the ground electrode in proximity with the elongated electrosurgical cutting electrode applied to an outer surface of the vessel wall.

10. The electrosurgical vessel wall cutting tool of claim 8 , wherein the elongated electrosurgical cutting electrode is supported by a ball and socket mechanism of the tool body.

11. The electrosurgical vessel wall cutting tool of claim 8 , further comprising a mechanism for controlling the depth of the elongated slit cut into the vessel wall.

12. The electrosurgical vessel wall cutting tool of claim 11 , wherein the mechanism for controlling the depth of the elongated slit cut into the vessel wall shuts off the application of RF energy when a selected cut depth is acheived.

13. The electrosurgical vessel wall cutting tool of claim 11 , wherein the mechanism for controlling the depth of the elongated slit cut into the vessel wall comprises one or more sensing electrodes.

14. The electrosurgical vessel wall cutting tool of claim 11 , wherein the mechansim for controlling the depth of the elongated slit cut into the vessel wall is adjustable.

15. An electrosurgical vessel wall cutting tool adapted to be employed with a ground electrode in contact with the body of a patient and coupled to a source of RF energy for making an elongated slit through a vessel wall and into a lumen of a body vessel, having a vessel axis, of a patient that is surgically accessed in a surgical operating field comprising:

an elongated tool body extending between a tool body proximal end and a tool body distal end;

a connector element supported by the tool body adapted to be coupled with the source of RF energy;

an elongated electrosurgical cutting electrode supported at the tool body distal end, the elongated electrosurgical cutting electrode having a predetermined cutting electrode length exceeding the cutting electrode width; and

an electrical conductor within the tool body extending between and coupled with the connector element and the elongated electrosurgical cutting electrode, whereby the elongated tool body is adapted to be manipulated to align and apply the cutting electrode length of the elongated electrosurgical cutting electrode against a surface of the vessel wall in substantially parallel alignment with the vessel axis so as to apply RF energy between the electrosurgical cutting electrode and the ground electrode at an energy level and for a duration sufficient to cut an elongated slit through the vessel wall where the elongated electrosurgical cutting electrode is applied to the surface of the vessel wall, wherein the elongated electrosurgical cutting electrode is supported by a ball and socket mechanism of the tool body that enables the disposition of the elongated electrosurgical cutting electrode within a range of angles to an elongated axis of the tool body to allow the elongated electrosurgical cutting electrode to be placed in alignment with the axis of the body vessel and in contact with the surface of the vessel wall.

The electrosurgical vessel wall cutting tool of claim 15 , wherein at least a section of the elongated tool body is malleable to enable formation of a bend in the elongated axis of the tool body to allow the elongated electrosurgical cutting electrode to be placed in alignment with the axis of the body vessel and in contact with the surface of the vessel wall.

17. The electrosurgical vessel wall cutting tool of claim 15 , further comprising a mechanism for controlling the depth of the elongated slit cut into the vessel wall.

18. The electrosurgical vessel wall cutting tool of claim 17 , wherein the mechanism for controlling the depth of the elongated slit cut into the vessel wall shuts off the application of RF energy when a selected cut depth is acheived.

19. The electrosurgical vessel wall cutting tool of claim 18 , wherein the mechanism for controlling the depth of the elongated slit cut into the vessel wall comprises one or more sensing electrodes.

20. The electrosurgical vessel wall cutting tool of claim 18 , wherein the mechansim for controlling the depth of the elongated slit cut into the vessel wall is adjustable.

The electrosurgical vessel wall cutting tool of claim 15 , wherein the ground electrode further comprises an exposed conductive ground electrode of an electrically insulated ground wire extending between a ground wire proximal end and a ground wire distal end adapted to be introduced into the lumen of the body vessel to dispose the ground electrode in proximity with the elongated electrosurgical cutting electrode applied to an outer surface of the vessel wall.

An electrosurgical vessel wall cutting tool adapted to be employed with a ground electrode in contact with the body of a patient and coupled to a source of RF energy for making an elongated slit through a vessel wall and into a lumen of a body vessel, having a vessel axis, of a patient that is surgically accessed in a surgical operating field comprising:

an elongated tool body extending between a tool body proximal end and a tool body distal end;

a connector element supported by the tool body adapted to be coupled with the source of RF energy;

an elongated electrosurgical cutting electrode supported at the tool body distal end, the elongated electrosurgical cutting electrode having a predetermined cutting electrode length exceeding the cutting electrode width;

an electrical conductor within the tool body extending between and coupled with the connector element and the elongated electrosurgical cutting electrode, whereby the elongated tool body is adapted to be manipulated to align and apply the cutting electrode length of the elongated electrosurgical cutting electrode against a surface of the vessel wall in substantially parallel alignment with the vessel axis so as to apply RF energy between the electrosurgical cutting electrode and the ground electrode at an energy level and for a duration sufficient to cut an elongated slit through the vessel wall where the elongated electrosurgical cutting electrode is applied to the surface of the vessel wall; and

means for controlling the depth of the elongated slit cut into the vessel wall, wherein the means for controlling the depth of the elongated slit cut into the vessel wall shuts off the application of RF energy when a selected cut depth is acheived.

23. The electrosurgical vessel wall cutting tool of claim 22 , wherein the means for controlling the depth of the elongated slit cut into the vessel wall comprises one or more sensing electrodes.

24. The electrosurgical vessel wall cutting tool of claim 22 , wherein the means for controlling the depth of the elongated slit cut into the vessel wall is adjustable.

25. The electrosurgical vessel wall cutting tool of claim 22 , wherein at least a section of the elongated tool body is malleable.

26. The electrosurgical vessel wall cutting tool of claim 22 , wherein the elongated electrosurgical cutting electrode is supported by a ball and socket mechanism of the tool body.

The electrosurgical vessel wall cutting tool of claim 22 , wherein the ground electrode further comprises an exposed conductive ground electrode of an electrically insulated ground wire extending between a ground wire proximal end and a ground wire distal end adapted to be introduced into the lumen of the body vessel to dispose the ground electrode in proximity with the elongated electrosurgical cutting electrode applied to an outer surface of the vessel wall.
