Abstract

Various embodiments include methods for diagnosing and treating medical conditions that involve an autoimmune response to connective tissue such as collagen found in organs such as the lung. In one method pulmonary disease and disorders such as Idiopathic Pulmonary Fibrosis (IPF) are diagnosed by analyzing fluid or tissue samples obtained from a patient for evidence of an autoimmune response to various types of collagen including, for example, Type V. One type of assay for evidence of an autoimmune response to Type V collagen comprises the steps of obtaining a fluid or tissue sample from a patient, contacting at least a portion of the sample with antigen to anti-Type V collagen antibody and monitoring the mixture of sample and antigen for changes indicative of the presence of anti-Type V collagen in the sample. Another embodiment includes treating pulmonary diseases such as IPF by administering a therapeutically effective dose of epitopes of various collagens including Type V collagen.

Claims (16)

1. A method for determining if a patient afflicted with pulmonary disease is a candidate for immunosuppressive therapy comprising;

contacting at least a portion of a sample from the patient with Type V collagen or an antigenic component thereof; and

measuring a Type V collagen-specific immune response;

wherein the presence of a Type V collagen-specific immune response indicates that the patient is a candidate for immunosuppressive therapy.

2. The method according to claim 1 wherein the pulmonary disease is selected from the group consisting of Idiopathic Pulmonary Fibrosis, Adult Respiratory Distress Syndrome, Acute Respiratory Distress Syndrome, secondary collagen vascular disease, any form of fibrotic lung disease.

3. The method according to claim 1 wherein the immunosuppressive therapy comprises immune tolerance therapy.

4. The method according to claim 3 wherein the immune tolerance therapy comprises oral feeding, intrapulmonary instillation, inhalation, or intravenous injection of Type V collagen or an antigenic component thereof.

5. A method for screening a patient awaiting a lung transplant for an elevated risk of lung transplant rejection, the method comprising:

contacting at least a portion of a sample from the patient with Type V collagen or an antigenic component thereof; and

measuring a Type V collagen-specific immune response;

wherein the presence of a Type V collagen-specific immune response indicates an elevated risk of lung transplant rejection.

6. The method of any one of claims 1 - 5 wherein the measuring a Type V collagen-specific immune response comprises measuring the level of Type V collagen-specific antibodies in the sample.

7. The method of claim 6 wherein the Type V collagen-specific antibody is measured using Type V collagen-coated beads.

8. The method of any one of claims 1 - 5 wherein the measuring a Type V collagen-specific immune response comprises measuring a Type V collagen-specific T cell response.

9. The method of any one of claims 1 - 5 wherein the measuring a Type V collagen-specific immune response comprises measuring a Type-V collagen-specific delayed-type hypersensitivity response.

10. The method of any one of claims 1 - 5 wherein the sample is selected from the group consisting of blood, sera, interstitial lung fluid, sputum, mucus and tissue.
