Abstract

Delivering ultrasonic energy to a target musculoskeletal tissue site includes connecting a delivery device to a vacuum source, a fluid source, and a power signal source. The delivery device has a housing portion maintaining an ultrasound transducer and a tip portion having a sleeve and a cannula. The cannula is coupled to the ultrasound transducer and received in the sleeve to define a covered portion and an exposed portion. Ultrasonic energy is generated by sending a power signal from the power signal source to the ultrasound transducer. The ultrasonic energy is transmitted from the ultrasound transducer to the cannula, such that the exposed portion of the cannula delivers ultrasonic energy at a frequency that is pre-selected to debride musculoskeletal tissue upon percutaneous insertion of the tip portion.

Claims (29)

1. A system comprising:

a delivery device configured to deliver ultrasonic energy to musculoskeletal tissue, the device including:

a housing portion defining a compartment and having an aspiration conduit and

an irrigation conduit;

an ultrasonic transducer disposed in the compartment of the housing portion, secured coaxially about the mounting post, and configured to translate a power signal to ultrasonic energy for musculoskeletal debridement;

a tip portion coupled to the housing portion, the tip portion configured to deliver fluid coming through the irrigation conduit to a musculoskeletal tissue site and to deliver detritus coming from the musculoskeletal tissue site through the aspiration channel, the tip portion including:

a cannula having a proximal portion and a distal portion, the cannula being coupled to the ultrasonic transducer to receive ultrasonic energy from the ultrasonic transducer and deliver the ultrasonic energy to the musculoskeletal tissue site; and

a sleeve adapted for percutaneous insertion and forming a lumen receiving the proximal portion of the cannula; and

a controller configured to deliver the power signal to the ultrasonic transducer,

wherein the system is optimized to deliver an ultrasonic energy frequency from 25 kHz to 29 kHz; and

wherein the device is configured to be ultrasound imaged.

2. The system of claim 1 , wherein the aspiration conduit passes through the ultrasonic transducer.

3. The system of claim 1 , wherein the lumen of the sleeve and the cannula define a fluid space that is in communication with the irrigation conduit of the housing.

4. The system of claim 1 , wherein the fluid space circumscribes the proximal portion of the cannula.

5. The system of claim 1 , wherein the tip portion is removably attached to the housing portion.

6. The system of claim 1 , wherein the tip portion includes echogenic material such that the tip portion is configured to be ultrasound imaged.

7. The system of claim 1 , wherein the sleeve has a connector portion and an insertion portion having an effective outer diameter that is less than the connector portion, the effective outer diameter of the insertion portion is in the range of 12 gauge to 25 gauge.

8. The system of claim 4 , wherein the cannula and the housing portion each include proximal threading for removably attaching the tip portion to the housing portion.

9. The system of claim 1 , wherein the cannula defines an effective diameter in the range of 12 gauge to 25 gauge or less.

10. The system of claim 1 , wherein the controller includes a user interface comprising a sequentially guide through an operation schedule.

11. The system of claim 1 , wherein the controller includes a vacuum pump and a vacuum port remotely coupled to the aspiration conduit of the delivery device and a fluid pump and a fluid port remotely coupled to the irrigation conduit of the delivery device.

The system of claim 1 , wherein the controller is remotely connected to the delivery device to deliver a fluid flow and a vacuum flow to the delivery device, the controller having a main housing and a disposable and detachable tubing cassette removably secured to the housing and configured to be remotely connected to the aspiration conduit and the irrigation conduit of the delivery device.

13. The system of claim 1 , wherein the controller is maintained by the housing portion of the delivery device.

14. The system of claim 1 , wherein the delivery device is configured to deliver ultrasonic energy via at least one of longitudinal and transverse vibration of the cannula.

15. The system of claim 1 ,

wherein the transducer generates both longitudinal and transverse vibrations.

16. The system of claim 15 , wherein the cannula comprises a tip that is cylindrical and does not have serrated edges.

17. The system of claim 1 ,

wherein the cannula comprises a tip that is cylindrical and does not have serrated edges.
