Abstract

Improved tissue fasteners are disclosed that can be inserted into (and optionally through) tissue structures underlying a tissue surface for affixing overlapping tissues and tissue planes together, and the like. In some embodiments, an elongate anvil body may protrude distally and/or laterally from a fastener support disposed along the base. The anvil body may have a sharpened end and be configured to penetrate into the tissue, with the elongate anvil body optionally having a bend from a more distal orientation adjacent the clip support to a more lateral orientation adjacent the sharpened end during at least a portion of the deployment. The first leg can be driven through a desired location on the surface of the first tissue and against a receptacle of the anvil body so as to deform the fastener and affix it to the first tissue.

Claims (23)

1. A device for deploying a surgical fastener in a tissue, the device comprising:

an anvil configured for advancing into the tissue;

a rotatable lobe coupled with the anvil, the rotatable lobe including a non-penetrating tissue engaging surface configured for non-penetrating placement distally against a surface of the tissue;

wherein the lobe is rotatable against the surface of the tissue to control a location of the anvil relative to the tissue during deployment of the fastener into the tissue; and

wherein the rotation of the lobe against the surface of the tissue controls an advancement depth of the anvil relative to the tissue.

2. The device of claim 1 , wherein the anvil and the lobe are incorporated together in one piece.

3. The device of claim 1 , further comprising a surgical fastener, the fastener having a base portion and at least one leg extending distally from the base portion, and wherein the anvil includes a channel defining a leg-receiving surface.

4. The device of claim 3 , wherein the at least one leg of the fastener is introduced into the channel such that the anvil and the at least one leg of the fastener concurrently advance into the tissue during fastener deployment.

5. The device of claim 4 , wherein the anvil includes a sharpened distal end for penetrating the tissue surface.

6. The device of claim 5 , wherein the at least one leg of the fastener has a diameter in a range from about 0.001 to 0.010 inches.

7. The device of claim 5 , wherein the anvil deforms the at least one leg of the fastener relative to the base portion of the fastener during lobe rotation against the surface of the tissue.

8. The device of claim 7 , wherein lobe rotation results in the base portion of the fastener being positioned directly against the tissue surface.

9. The device of claim 1 , wherein the tissue comprises an eye tissue, and wherein rotation of the lobe against the tissue surface controls the advancement depth of the anvil such that the anvil does not penetrate the full thickness of the tissue.

10. The device of claim 1 , wherein a fastener is deployed by pushing the lobe distally against the surface of the tissue and articulating a linkage coupled with the anvil and the rotatable lobe, the linkage rotating the lobe relative to the tissue surface.

11. The device of claim 10 , wherein articulation of the linkage rotates and laterally translates the anvil.

12. The device of claim 1 , further comprising:

a surgical fastener, the fastener having a base portion and at least two legs extending distally from opposite sides of the base portion;

a second anvil;

a second rotatable lobe coupled with the second anvil, the second rotatable lobe including a tissue engaging surface configured for placement distally against the surface of the tissue;

wherein the anvils are configured for advancing into the tissue;

wherein each anvil includes a channel defining a leg receiving surface and wherein the legs of the surgical fastener are introduced into the channels such that the anvils and the legs of the fastener concurrently advance into the tissue during fastener deployment.

The device of claim 12 , wherein the lobes are rotatable in opposite directions against the surface of the tissue to control the location of the anvils during fastener deployment, the anvils deforming the legs of the fastener relative to the base portion of the fastener during lobe rotation against the surface of the tissue; and

wherein lobe rotation results in the base portion of the fastener being positioned directly against the tissue surface.
