Abstract

Described herein are flexible implantable occluding devices that can, for example, navigate the tortuous vessels of the neurovasculature. The occluding devices can also conform to the shape of the tortuous vessels of the vasculature. In some embodiments, the occluding devices can direct blood flow within a vessel away from an aneurysm or limit blood flow to the aneurysm. Some embodiments describe methods and apparatus for adjusting, along a length of the device, the porosity of the occluding device. In some embodiments, the occluding devices allows adequate blood flow to be provided to adjacent structures such that those structures, whether they are branch vessels or oxygen-demanding tissues, are not deprived of the necessary blood flow.

Claims (24)

1. A system, for treating a patient's vessel, comprising:

a catheter;

an elongate body at least partially within the catheter and having a proximal portion with a proximal end, a distal portion with a distal end, and a lumen extending between the proximal end and the distal end;

a stent, residing entirely within the elongate body in a compressed stent configuration and configured to be advanced relative to the elongate body, the stent having a distal section and a proximal section, the stent being configured to change between the compressed stent configuration and an expanded stent configuration; and

at least one cutting member mounted directly to an outer surface of the catheter and being configured to increase a cross-sectional dimension of a lumen of the vessel in a stenotic region of the vessel, by dislodging plaque material in the stenotic region by moving rotationally relative to the stent, while the distal section remains engaged with, and rotationally fixed relative to, a wall of the vessel;

wherein the distal section of the stent is configured to engage the wall and to change from a compressed distal configuration to an expanded distal configuration when advanced distal to the distal end of the elongate body;

wherein the proximal section of the stent is configured to engage the wall and to change from a compressed proximal configuration to an expanded proximal configuration by advancing the stent relative to the elongate body;

wherein, while the expanded distal section of the stent is engaged at the vessel wall, while the proximal section of the stent is retained by the elongate body, and while the at least one cutting member dislodges the plaque material, the distal section captures debris dislodged from the stenotic region by the at least one cutting member.

2. The system of claim 1 , wherein the stent, when unrestrained in the expanded stent configuration, comprises a porous wall and has a porosity of the wall of the stent between about 60% and about 85%.

3. The system of claim 1 , wherein the stent, when unrestrained in the expanded stent configuration, comprises a porous wall and has a porosity of the wall of the stent between about 65% and about 75%.

4. The system of claim 2 , wherein the stent comprises a porous wall and is axially compressible when in the expanded stent configuration, and the porosity of the wall of the stent is reducible to between about 5% and about 30% without substantially changing an outer diameter of the stent.

5. The system of claim 1 , wherein the stent, in the expanded stent configuration, comprises a porous wall and is adjustable to decrease porosities in the wall of the stent in a respective plurality of locations between the proximal section and the distal section.

6. The system of claim 1 , wherein the stent, in the expanded stent configuration, comprises a porous wall and is changeable in a plurality of locations along the stent to have a porosity of the wall of the stent in at least one of the locations of from about 5% and about 30%.

7. A system, for treating a patient's vessel, comprising:

a catheter;

a elongate body within the catheter and having a lumen extending between a proximal end and a distal end;

a stent, having a distal section and a proximal section, residing entirely within the lumen of the elongate body in a compressed stent configuration and configured to be advanced relative to the elongate body; and

at least one cutting member mounted directly to an outer surface of the catheter and being configured to increase a cross-sectional dimension of a stenotic region of the vessel by dislodging plaque material in the stenotic region as the catheter moves rotationally relative to the stent, while the distal section remains engaged with, and rotationally fixed relative to, a wall of the vessel;

wherein a distal section of the stent is configured to engage the wall and to change from a compressed distal configuration to an expanded distal configuration when advanced distal to the distal end of the elongate body;

wherein a proximal section of the stent is configured to engage the wall and to change from a compressed proximal configuration to an expanded proximal configuration by advancing the stent relative to the elongate body;

wherein, when the expanded distal section of the stent engages the vessel wall, and while the proximal section of the stent is in the compressed proximal configuration, the distal section captures debris dislodged from the stenotic region by the at least one cutting member.

8. The system of claim 7 , wherein the stent, in an expanded stent configuration, comprises a porous wall and is adjustable to decrease porosities in the porous wall of the stent in a respective plurality of locations along or between the proximal section and the distal section.

The system of claim 7 , wherein the at least one cutting member is configured to rotate about the proximal section of the stent while the proximal section of the stent is in the compressed proximal configuration the distal section is in the expanded distal configuration and engaging the vessel wall.

10. The system of claim 7 , wherein the cutting member is configured to rotate about the elongate body.
