Abstract

Methods are provided for detecting the amount of one or more CAH panel analytes (i.e., pregnenolone, 17-OH pregnenolone, progesterone, 17-OH progesterone, dehydroepiandrosterone (DHEA), androstenedione, testosterone, deoxycorticosterone, 11-deoxycortisol, and cortisol) in a sample by mass spectrometry. The methods generally involve ionizing one or more CAH panel analytes in a sample and quantifying the generated ions to determine the amount of one or more CAH panel analytes in the sample. In methods where amounts of multiple CAH panel analytes are detected, the amounts of multiple analytes are detected in the same sample injection.

Claims (55)

1. A method for determining by mass spectrometry the amount in a sample of two or more analytes selected from the group consisting of 17-hydroxyprogesterone (17-OH progesterone), androstenedione, testosterone, and cortisol,

the method comprising:

a. subjecting said sample to turbulent flow liquid chromatography to obtain a sample enriched in said two or more analytes subject to determination;

b. subjecting said enriched sample to an ionization source under conditions suitable to produce one or more ions detectable by mass spectrometry from each of said two or more analytes;

c. determining the amounts of the one or more ions from each of the two or more analytes by tandem mass spectrometry; and

d. using the amounts of the one or more ions from each of the two or more analytes to determine the amounts of each of the two or more analytes in the sample;

wherein said two or more analytes are not derivatized prior to ionization.

2. The method of claim 1 , wherein said two or more analytes comprise 17-OH progesterone and androstenedione.

3. The method of claim 1 , wherein said two or more analytes comprise 17-OH progesterone and testosterone.

4. The method of claim 1 , wherein said two or more analytes comprise 17-OH progesterone and cortisol.

5. The method of claim 1 , wherein said two or more analytes comprise androstenedione and testosterone.

6. The method of claim 1 , wherein said two or more analytes comprise androstenedione and cortisol.

7. The method of claim 1 , wherein said two or more analytes comprise testosterone and cortisol.

The method of claim 1 , wherein said two or more analytes comprise 17-OH progesterone, and said one or more ions detectable by mass spectrometry from 17-OH progesterone comprise one or more ions selected from the group consisting of ions with mass to charge ratios (m/z) of 331.0±0.5, 109.0±0.5, and 96.9±0.5.

The method of claim 1 , wherein said two or more analytes comprise androstenedione, and said one or more ions detectable by mass spectrometry from androstenedione comprise one or more ions selected from the group consisting of ions with mass to charge ratios (m/z) of 287.1±0.5, 109.1±0.5, and 91.1±0.5.

The method of claim 1 , wherein said two or more analytes comprise testosterone, and said one or more ions detectable by mass spectrometry from testosterone comprise one or more ions selected from the group consisting of ions with mass to charge ratios (m/z) of 289.1±0.5, 109.0±0.5, and 97.0±0.5.

11. The method of claim 1 , wherein said two or more analytes comprise cortisol, and said one or more ions detectable by mass spectrometry from cortisol comprise one or more ions selected from the group consisting of ions with mass to charge ratios (m/z) of 363.1±0.5, 121.0±0.5, and 90.9±0.5.

12. The method of claim 1 , further comprising determining by mass spectrometry the amount of one or more additional analyte selected from the group of dehydroepiandrosterone (DHEA), pregnenolone, 17-hydroxypregnenolone (17-OH pregnenolone), progesterone, deoxycorticosterone, and 11-deoxycortisol.

13. The method of claim 1 , wherein said sample comprises a biological sample.

14. The method of claim 1 , wherein the sample comprises plasma or serum.

15. A method of diagnosing congenital adrenal hyperplasia (CAH) comprising:

a. obtaining a sample of a body fluid an individual to be diagnosed for CAH;

b. processing said body fluid sample to generate a processed sample;

c. subjecting a single injection of said processed sample to an ionization source under conditions suitable to ionize CAH panel analytes comprising 17-hydroxyprogesterone (17-OH progesterone), and one or more of androstenedione and cortisol from said processed sample to generate one or more ions detectable by mass spectrometry from each of said CAH panel analytes;

d. determining the amounts of one or more ions from each of said CAH panel analytes by tandem mass spectrometry; and

e. using the amounts of one or more ions from each of said CAH panel analytes to determine the amounts of said CAH panel analytes in said body fluid sample;

wherein said CAH panel analytes are not derivatized prior to ionization.

16. The method of claim 15 , wherein said one or more ions from each of said CAH panel analytes comprise one or more 17-OH progesterone ions selected from the group consisting of ions with mass to charge ratios (m/z) of 331.0±0.5, 109.0±0.5, and 96.9±0.5.

17. The method of claim 15 , wherein said CAH panel analytes comprise androstenedione, and one or more ions from androstenedione are selected from the group consisting of ions with mass to charge ratios (m/z) of 287.1±0.5, 109.1±0.5, and 91.1±0.5.

18. The method of claim 15 , wherein said CAH panel analytes comprise cortisol, and one or more ions from cortisol are selected from the group consisting of ions with mass to charge ratios (m/z) of 363.1±0.5, 121.0±0.5, and 90.9±0.5.

19. The method of claim 15 , wherein said CAH panel analytes comprise 17-OH progesterone, androstenedione, and cortisol.

20. The method of claim 15 , wherein said CAH panel analytes further comprise one or more analytes selected from the group consisting of dehydroepiandrosterone (DHEA), pregnenolone, 17-hydroxypregnenolone (17-OH pregnenolone), progesterone, deoxycorticosterone, 11-deoxycortisol, and testosterone.

21. The method of claim 15 , wherein said CAH panel analytes further comprise testosterone.

22. A method for determining the amount of pregnenolone in a sample by mass spectrometry, the method comprising:

a. subjecting a sample to an ionization source under conditions suitable to ionize pregnenolone from said sample to generate one or more pregnenolone ions detectable by mass spectrometry;

b. determining the amounts of one or more pregnenolone ions by tandem mass spectrometry; and

c. using the amounts of one or more pregnenolone ions to determine the amount of pregnenolone in the sample;

wherein said two or more analytes are not derivatized prior to ionization.

The method of claim 22 , wherein determining the amounts of one or more pregnenolone ions by tandem mass spectrometry comprises fragmenting a pregnenolone precursor ion with mass to charge ratio of 299.2±0.5 to produce one or more pregnenolone fragment ions selected from the group consisting of pregnenolone fragment ions with mass to charge ratios of 105.6±0.5 and 91.1±0.5.

24. The method of claim 22 , wherein the sample comprises a biological sample.

25. The method of claim 22 , wherein the sample comprises plasma or serum.

26. The method of claim 22 , wherein the sample has been purified by liquid chromatography prior to being subjected to an ionization source.

27. The method of claim 22 , wherein said liquid chromatography is selected from the group consisting of high performance liquid chromatography, ultra high performance liquid chromatography, and turbulent flow liquid chromatography.

28. The method of claim 22 , wherein the sample has been purified by turbulent flow liquid chromatography and either high performance liquid chromatography or ultra high performance liquid chromatography prior to being subjected to an ionization source.

29. A method for determining the amount of 17-hydroxypregnenolone (17-OH pregnenolone) in a sample by mass spectrometry, the method comprising:

a. subjecting a sample to an ionization source under conditions suitable to produce one or more 17-OH pregnenolone ions detectable by mass spectrometry;

b. determining the amounts of one or more 17-OH pregnenolone ions by tandem mass spectrometry; and

c. using the amounts of one or more 17-OH pregnenolone ions to determine the amount of 17-OH pregnenolone in the sample;

wherein said two or more analytes are not derivatized prior to ionization.

The method of claim 29 , wherein determining the amounts of one or more 17-OH pregnenolone ions by tandem mass spectrometry comprises fragmenting a 17-OH pregnenolone precursor ion with mass to charge ratio of 297.2±0.5 to produce one or more 17-OH pregnenolone fragment ions selected from the group consisting of 17-OH pregnenolone fragment ions with mass to charge ratios of 105.6±0.5 and 91.1±0.5.

31. The method of claim 29 , wherein the sample comprises a biological sample.

32. The method of claim 29 , wherein the sample comprises plasma or serum.

33. The method of claim 29 , wherein the sample has been purified by liquid chromatography prior to being subjected to an ionization source.

34. The method of claim 33 , wherein said liquid chromatography is selected from the group consisting of high performance liquid chromatography, ultra high performance liquid chromatography, and turbulent flow liquid chromatography.

35. The method of claim 29 , wherein the sample has been purified by turbulent flow liquid chromatography and either high performance liquid chromatography or ultra high performance liquid chromatography prior to being subjected to an ionization source.
