Methods for core body temperature management
Relatively non-invasive devices and methods for heating or cooling a patient's body are disclosed. Devices and methods for treating ischemic conditions by inducing therapeutic hypothermia are disclosed. Devices and methods for inducing therapeutic hypothermia through esophageal cooling are disclosed. Devices and methods for operative temperature management are disclosed.
1. A method for maintaining normothermia or preventing hypothermia in a subject during a surgical procedure in an operating room environment, wherein the surgical procedure and/or the operating room environment create a risk of inadvertent hypothermia, the method comprising:
nasally or orally inserting a distal end of a heat transfer device into the subject;
positioning a heat transfer region of the heat transfer device in the subject's esophagus, wherein the heat transfer region comprises at least one lumen providing a fluid path for flow of a heat transfer medium and wherein the heat transfer region has a diameter of about 1.0 to about 2.0 centimeters;
initiating flow of the heat transfer medium through the fluid path; and
circulating the heat transfer medium along the fluid path for a time sufficient to maintain normothermia or prevent hypothermia in the subject during the surgical procedure in the operating room environment.
2. The method of claim 1 , whereby core body temperature of the subject is maintained within about ±0.5° C. of a goal temperature during the surgical procedure.
3. The method of claim 2 , wherein the goal temperature is about 37° C.
4. The method of claim 1 , whereby core body temperature of the subject is maintained above about 36° C. during the surgical procedure.
5. The method of claim 1 , wherein the heat transfer region has a diameter of about 1.2 centimeters.
6. The method of claim 1 , wherein the operating room environment includes (i) exposure of the subject to ambient temperature, (ii) administration of intravenous fluid hydration to the subject, and/or (iii) administration of anesthesia to the subject.
7. The method of claim 1 , comprising raising core body temperature of the subject by at least about 0.5° C. during the surgical procedure.
8. The method of claim 1 , wherein flow of the heat transfer medium through the fluid path is initiated prior to positioning the heat transfer region of the heat transfer device in the subject's esophagus.
9. The method of claim 1 , wherein core body temperature of the subject is controlled for at least two hours.
10. The method of claim 1 , wherein the method does not comprise using a stylet to aid in inserting or positioning the device.
11. The method of claim 1 , wherein at least one lumen is an inflow lumen defined by a semi-rigid supply tube and the semi-rigid supply tube is not inflatable.
12. The method of claim 11 , wherein the heat transfer device further comprises a heat transfer medium input port connected to the inflow lumen.
13. The method of claim 11 , wherein the semi-rigid supply tube comprises a plastic or elasotmer selected from the group consisting of ethylene tetrafluoroethylene (ETFE), polytetrafluoroethylene (PTFE), perfluoroalkoxy (PFA), fluorinated ethylene propylene (FEP), and silicone rubber.
14. The method of claim 11 , wherein the semi-rigid supply tube comprises silicon rubber.
15. The method of claim 11 , wherein the heat transfer region includes at least a portion of the semi-rigid supply tube.
16. The method of claim 1 , wherein the heat transfer region does not include a balloon or partially inflatable lumen.
17. The method of claim 1 , wherein the heat transfer device comprises a gastric tube that, when the heat transfer region is positioned in an esophagus of the subject, extends into a stomach of the subject.
18. The method of claim 1 , wherein the subject is a human adult.
19. A method for raising core body temperature of a subject during a surgical procedure in an operating room environment, wherein the surgical procedure and/or the operating room environment create a risk of inadvertent hypothermia, the method comprising:
nasally or orally inserting a distal end of a heat transfer device into the subject;
positioning a heat transfer region of the heat transfer device in the subject's esophagus, wherein the heat transfer region comprises at least one lumen providing a fluid path for flow of a heat transfer medium and wherein the heat transfer region has a diameter of about 1.0 to about 2.0 centimeters;
initiating flow of the heat transfer medium through the fluid path; and
circulating the heat transfer medium along the fluid path for a time sufficient to raise core body temperature in the subject during the surgical procedure in the operating room environment.
20. The method of claim 19 , wherein the subject is initially hypothermic.
21. The method of claim 19 , wherein the subject is initially in an induced hypothermic state.
22. The method of claim 19 , wherein the surgical procedure includes (i) exposing the subject to ambient temperature, (ii) administering intravenous fluid hydration to the subject, and/or (iii) administering anesthesia to the subject.
23. The method of claim 19 , wherein the heat transfer region has a diameter of about 1.2 centimeters.
24. The method of claim 19 , comprising raising the core body temperature of the subject by at least about 0.5° C. during the surgical procedure.
25. The method of claim 19 , wherein flow of the heat transfer medium through the fluid path is initiated prior to positioning the heat transfer region of the heat transfer device in the subject's esophagus.
26. The method of claim 19 , wherein the method does not comprise using a stylet to aid in inserting or positioning the device.
27. The method of claim 19 , wherein at least one lumen is an inflow lumen defined by a semi-rigid supply tube and the semi-rigid supply tube is not inflatable.
28. The method of claim 27 , wherein the heat transfer device further comprises a heat transfer medium input port connected to the inflow lumen.
29. The method of claim 27 , wherein the semi-rigid supply tube comprises a plastic or elasotmer selected from the group consisting of ethylene tetrafluoroethylene (ETFE), polytetrafluoroethylene (PTFE), perfluoroalkoxy (PFA), fluorinated ethylene propylene (FEP), and silicone rubber.
30. The method of claim 27 , wherein the semi-rigid supply tube comprises silicon rubber.
31. The method of claim 27 , wherein the heat transfer region includes at least a portion of the semi-rigid supply tube.
32. The method of claim 19 , wherein the heat transfer region does not include a balloon or partially inflatable lumen.
33. The method of claim 19 , wherein the heat transfer device comprises a gastric tube that, when the heat transfer region is positioned in an esophagus of the subject, extends into a stomach of the subject.
34. The method of claim 19 , wherein the subject is a human adult.
