Abstract

The present invention relates to an ophthalmic knife and methods of its use for treatment of various conditions including eye diseases, such as glaucoma, using minimally invasive surgical techniques. The invention relates to a multi-blade device for cutting the tissues within the eye, for example, a trabecular meshwork (TM).

Claims (39)

1. An ophthalmic knife, comprising:

a shaft;

a platform connected to the shaft at a first angle relative to a plane, the platform comprising:

a top surface; and

a bottom surface;

first and second cutting blades disposed in parallel that are separated by a gap and extend from the top surface to the shaft at a second angle relative to the plane, wherein the second angle is different than the first angle; and

a lumen extending through the shaft and terminating at an exit port extending through the bottom surface of the platform.

2. The ophthalmic knife of claim 1 , further comprising a handle, wherein the lumen extends longitudinally within the handle, the shaft, and the platform.

3. The ophthalmic knife of claim 1 , wherein the exit port is opposite the first and second platform blades.

4. The ophthalmic knife of claim 1 , wherein the bottom surface is planar.

5. The ophthalmic knife of claim 1 , further comprising a handle, wherein the lumen further terminates at an entry port in the handle.

6. The ophthalmic knife of claim 1 , wherein the lumen is configured to receive a viscoelastic fluid or an aspiration fluid.

7. The ophthalmic knife of claim 1 , wherein the top surface of the platform provides a ramp that is devoid of a cutting edge.

8. The ophthalmic knife of claim 1 , wherein the bottom surface of the platform extends in a direction transverse to an axis of the shaft.

9. The ophthalmic knife of claim 1 , wherein the platform further comprises a piercing tip, wherein the top and bottom surfaces extend from the piercing tip on opposing sides of the platform.

The ophthalmic knife of claim 1 , wherein the first and second cutting blades each having a cutting edge configured to pierce tissue, so that the first and second cutting blades are configured to cut a strip of tissue having a width corresponding to the gap as the first and second cutting blades are advanced through the tissue.

11. An ophthalmic knife, comprising:

a shaft defining a shaft axis;

a platform connected to the shaft at a first angle from the shaft axis, the platform comprising:

a piercing tip;

a top surface extending from the piercing tip; and

a bottom surface on an opposite side of the platform from the top surface, the bottom surface extending from the piercing tip in a direction transverse to the shaft axis;

first and second cutting blades disposed in parallel that are separated by a gap and extend from the top surface to the shaft at a second angle from the shaft axis, each of the first and second cutting blades having a cutting edge that is configured to pierce tissue, wherein the second angle is different than the first angle; and

a lumen extending through the shaft and terminating at an exit port extending through the bottom surface of the platform.

12. The ophthalmic knife of claim 11 , wherein the top surface of the platform provides a ramp that is devoid of a cutting edge.

13. The ophthalmic knife of claim 11 , further comprising a handle, wherein the lumen extends longitudinally within the handle, the shaft, and the platform.

14. The ophthalmic knife of claim 11 , wherein the lumen extends in a direction along the shaft axis.

15. The ophthalmic knife of claim 11 , wherein the bottom surface is flat.

16. An ophthalmic knife, comprising:

a shaft;

a platform connected to the shaft at a first angle relative to a vertical plane, the platform comprising:

a top surface; and

a bottom surface;

first and second cutting blades disposed in parallel that are separated by a gap and extend from the top surface to the shaft at a second angle relative to the vertical plane, wherein the second angle is different than the first angle, the first and second cutting blades being configured to cut a strip of trabecular meshwork within an eye as the bottom surface of the platform is advanced along a Schlemm's canal of the eye; and

a lumen extending through the shaft and terminating at an exit port extending through the bottom surface of the platform.

17. The ophthalmic knife of claim 16 , wherein the top surface of the platform provides a ramp that is devoid of a cutting edge and configured to elevate the trabecular meshwork towards the first and second cutting blades.

18. The ophthalmic knife of claim 16 , wherein the bottom surface of the platform extends in a direction transverse to an axis of the shaft.

19. The ophthalmic knife of claim 16 , wherein the bottom surface is flat so that a bulk of the bottom surface does not touch an outer wall of the Schlemm's canal as the platform is advanced.

20. The ophthalmic knife of claim 16 , wherein the platform further comprises a piercing tip configured to pierce the trabecular meshwork, wherein the top and bottom surfaces extend from the piercing tip on opposing sides of the platform.
