Abstract

Implantable medical systems enter an exposure mode of operation, either manually via a down linked programming instruction or by automatic detection by the implantable system of exposure to a magnetic disturbance. A controller then determines the appropriate exposure mode by considering various pieces of information including the device type including whether the device has defibrillation capability, pre-exposure mode of therapy including which chambers have been paced, and pre-exposure cardiac activity that is either intrinsic or paced rates. Additional considerations may include determining whether a sensed rate during the exposure mode is physiologic or artificially produced by the magnetic disturbance. When the sensed rate is physiologic, then the controller uses the sensed rate to trigger pacing and otherwise uses asynchronous pacing at a fixed rate.

Claims (23)

1. A method comprising:

after detecting a need to switch to operation of a device to an exposure mode of therapy, sensing an intrinsic rate of cardiac activity;

comparing the intrinsic rate to at least one reference value to determine if the intrinsic rate is physiologic or artificial;

when the intrinsic rate is physiologic then implementing the exposure mode of therapy that includes sensing and using triggered pacing; and

when the intrinsic rate is artificial, then implementing the exposure mode of therapy that includes asynchronous pacing.

2. The method of claim 1 , wherein a rate for asynchronous pacing is based on pre-exposure rates of cardiac activity.

3. The method of claim 2 , wherein the pre-exposure rates include intrinsic rates.

4. The method of claim 1 , wherein the rate for asynchronous pacing is based on a percentile range taken from a statistical distribution of pre-exposure historical rate data for cardiac activity.

5. The method of claim 4 , wherein the pre-exposure historical rate data includes intrinsic rates.

6. The method of claim 1 , wherein the at least one reference value comprises a range of values that represent physiologic intrinsic rates and are based on prior rates of cardiac activity plus a fixed adjustment.

The method of claim 6 , wherein when the intrinsic rate falls within the range of a physiologic rate, comparing the intrinsic rate to a prior intrinsic rate sensed within a set period of time preceding the determination of the intrinsic rate, and when the intrinsic rate differs from the prior intrinsic rate by a threshold that indicates an artificial change to the intrinsic rate, then implementing an exposure mode of therapy that includes asynchronous pacing.

The method of claim 1 , wherein the at least one reference value comprises a prior intrinsic rate sensed within a set period of time preceding the determination of the intrinsic rate, the method further comprising comparing the intrinsic rate to the prior intrinsic rate and when the intrinsic rate differs from the prior intrinsic rate by at least a threshold amount that indicates an artificial change to the intrinsic rate, then implementing an exposure mode of therapy that includes asynchronous pacing.

9. An implantable medical device comprising:

a sensing module configured to sense cardiac activity of a patient;

a controller configured to:

control operation of the device,

detect an intrinsic rate of the cardiac activity after detecting a need to switch operation of the device to an exposure mode of therapy,

compare the intrinsic rate to at least one reference value to determine if the intrinsic rate is physiologic or artificial,

when the intrinsic rate is physiologic then implement the exposure mode of therapy that includes sensing and uses triggered pacing, and

when the intrinsic rate is artificial then implement the exposure mode of therapy that includes asynchronous pacing.

10. The implantable medical device of claim 9 , wherein the at least one reference value comprises a range of values that represent physiologic intrinsic rates and are based on prior rates of cardiac activity plus a fixed adjustment.

The implantable medical device of claim 10 , wherein when the intrinsic rate falls within the range of a physiologic rate, the controller compares the intrinsic rate to a prior intrinsic rate sensed within a set period of time preceding the determination of the intrinsic rate, and when the intrinsic rate differs from the prior intrinsic rate by a threshold that indicates an artificial change to the intrinsic rate, the controller implements an exposure mode of therapy that includes asynchronous pacing.

The implantable medical device of claim 9 , wherein the at least one reference value comprises a prior intrinsic rate sensed within a set period of time preceding the determination of the intrinsic rate, and the controller compares the intrinsic rate to the prior intrinsic rate and when the intrinsic rate differs from the prior intrinsic rate by a threshold that indicates an artificial change to the intrinsic rate, the controller implements an exposure mode of therapy that includes asynchronous pacing.
