Abstract

Described herein are methods and systems of performing immunotherapy on a subject and/or determining if a subject will be responsive to ablation immunotherapy.

Claims (28)

1. A medical diagnostic system comprising:

a generator, a processor, and a memory coupled to the processor, the generator capable of applying one or more electrical pulses in an amount sufficient to ablate a treatment site of a patient;

a current output monitor to operably couple to the generator, the current output monitor capable of collecting data relating to electrical current resulting from the applying of one or more applied electrical pulses;

the memory comprising computer-executable instructions that when executed by the processor, cause the processor to determine an average of the electrical current or a change in the electrical current from the electrical current data collected by the current output monitor, and based on the average electrical current and/or the change in electrical current determine whether and/or how the patient is expected to respond to a subsequent immunotherapy treatment.

2. The system of claim 1 , wherein one or more of the electrical pulses are capable of being applied in an amount sufficient to cause non-thermal ablation of cells of tissue in the treatment site of the patient.

3. The system of claim 2 , wherein one or more of the electrical pulses are configured to be non-immunosuppressive.

The system of claim 1 , wherein the computer-executable instructions are capable of causing the processor to determine whether and/or when to deliver one or more subsequent treatments, wherein such subsequent treatments include chemotherapy, radiation, treating one or more cancers, tissue resection, thermal ablation, non-thermal ablation, irreversible electroporation, immunotherapy, adjuvant therapy, biologic therapy, genetic therapy, and/or combinations thereof.

The system of claim 1 , wherein one or more of the electrical pulses are capable of being applied in an amount sufficient to cause any of the following: electroporation; non-thermal ablation; irreversible electroporation; reversible electroporation; non-thermal electroporation; non thermal irreversible electroporation; non-thermal reversible electroporation; or ablation of cells of one or more tissue.

The system of claim 1 , wherein the system is capable of determining that an immunosuppressive downstream treatment and/or an immunosuppressive secondary treatment can be delayed when: (i) the absolute value of the electrical current increases to about 25 Amps to 100 Amps; (ii) the percentage increase in the electrical current increases to 100% to 400%; (iii) the bulk conductivity of a tissue of the patient increases to about 0.7 S/m to 1.5 S/m; and/or (iv) the change in electrical current or the change in average electrical current is a change of at least 25 Amps.

The system of claim 1 , wherein based on the average electrical current and/or the change in electrical current, the processor is capable of determining (i) whether and/or when to stop delivering one or more of the electrical pulses to the patient, or (ii) whether to delay a subsequent treatment, to allow sufficient time for an immune response to peak.

8. A system comprising:

a generator to operably couple to at least one electrode, a processor, and a memory coupled to the processor, wherein the generator is capable of applying one or more electrical pulses to the at least one electrode to deliver the electrical pulses to a patient;

a current output monitor to operably couple to the generator, the current output monitor capable of collecting data relating to electrical current resulting from one or more of the delivered electrical pulses; and

the memory comprising computer-executable instructions that when executed by the processor, cause the processor to determine a change in a current parameter of the electrical current data collected by the current output monitor over a time required to deliver the one or more electrical pulses, and based on the change in the current parameter determine if the patient is expected to respond to a subsequent immunotherapy procedure.

9. The system of claim 8 , wherein one or more of the electrical pulses results in irreversible electroporation of cells of one or more tissue.

10. The system of claim 8 , wherein the current parameter comprises an increase in the absolute value of the electrical current, a percentage increase in the electrical current, and/or a percentage increase or change in the bulk conductivity of a tissue of the patient.

11. The system of claim 8 , wherein one or more of the electrical pulses are configured to increase the likelihood of a positive immune response in the patient.

The system of claim 8 , wherein the system is capable of determining that an immunosuppressive downstream treatment and/or an immunosuppressive secondary treatment can be delayed when: (i) the absolute value in the electrical current increases to about 25 Amps to 100 Amps; (ii) the percentage increase in the electrical current increases to 100% to 400%; (iii) the bulk conductivity of a tissue of the patient increases to about 0.7 S/m to 1.5 S/m; and/or (iv) the change in the current parameter is a change in the electrical current or a change in average electrical current and is a change of at least 25 Amps.

13. A diagnostic system comprising:

a generator, a processor, and a memory coupled to the processor, the generator capable of applying electrical energy in an amount sufficient to ablate a treatment site of a patient;

a current output monitor to operably couple to the generator and which is capable of collecting data relating to electrical current resulting from one or more applied electrical pulses;

the memory comprising computer-executable instructions that when executed by the processor, cause the processor to determine a change in a current parameter of the electrical current data collected by the current output monitor over a time required to deliver the one or more electrical pulses, and determine:

(i) if the patient is expected to respond to an immunotherapy treatment based on the change in the current parameter; and

(ii) determine whether and/or when to stop delivering the one or more electrical pulses in response to the change in the current parameter.

14. The diagnostic system of claim 13 , which is capable of (i) determining whether and/or when to deliver a subsequent immunosuppressive treatment; and/or (ii) determining whether and/or when to deliver the immunotherapy treatment to the patient.

The diagnostic system of claim 13 , wherein the generator is capable of being operably coupled to at least one electrode and is capable of applying the one or more electrical pulses to the at least one electrode in an amount sufficient to non-thermally ablate tissue of the patient; and wherein the generator is capable of being operably coupled to a current output monitor to collect data relating to electrical current resulting from one or more of the electrical pulses delivered.

The diagnostic system of claim 13 , wherein the current parameter comprises any of the following: an increase in the absolute value of the electrical current, a percentage increase in the electrical current, and/or a percentage increase or change in the bulk conductivity of a tissue of the patient.

The diagnostic system of claim 13 , wherein the system is capable of further determining a recommended subsequent treatment step in response to the change in a current parameter; wherein the subsequent treatment step comprises: leaving the tumor in situ in the patient and waiting a period of time, such as days or weeks or months, and optionally 1 day, 2 days, 3 days, 4 days, 5 days, 6 days, 7 days, 8 days, 1 week, 2 weeks, 3 weeks, and/or a month or more, for an optimal immune response before resecting the tumor and administering immunotherapy to the patient.
