Abstract

Disclosed are embodiments of a device for occluding a left atrial appendage (LAA) and other cavities or openings within a body. Some embodiments of the device can include an implant configured to be deployed within the LAA or other cavity, configured to be expanded or moved against a wall portion of the LAA or other cavity, and configured to twist at least a portion of the LAA or other cavity when the implant is rotated. Thereafter, one or more securing elements, staples, sutures, or other fasteners can be implanted in the gathered tissue to hold the tissue in the gathered state, thereby occluding the opening of the LAA or other cavity. In some embodiments, the opening of the LAA or other cavity can be occluded by elongating or otherwise reshaping the opening using an implant device, and securing the opening in the occluded state.

Claims (38)

1 . A device for treating a left atrial appendage, comprising:

(a) an implant comprising:

(1) a contact member comprising a plurality of struts that are expandable from a contracted state to an expanded state such that a size of the contact member in a widthwise direction is greater when the contact member is in the expanded state than when the contact member is in the contracted state, wherein the plurality of struts extend from a proximal portion of the contact member to a distal portion of the contact member;

(2) a securing element having a plurality of arms that extend toward the contact member when the securing element is in a deployed state; and

(3) a retention element comprising a threaded shaft, wherein the threaded shaft is configured to be threadably coupled with both the securing element and the contact member at least when the implant is in a deployed state.

2 . The device of claim 1 , wherein the plurality of struts of the contact member each has a plurality of tissue anchors thereon extending from each of the struts in a circumferential direction.

The device of claim 1 , wherein the implant is configured such that a rotation of the retention element in a first direction will cause the securing element to move axially toward the contact member and a rotation of the retention element in a second direction will cause the securing element to move axially away from the contact member.

The device of claim 1 , wherein the securing element is rotationally coupled with the contact member when the securing element is in the deployed state such that the securing element is at least inhibited from rotating relative to the contact member when the securing element is in the deployed state.

5 . The device of claim 1 , wherein the implant comprises at least one projection that extends into a channel of the contact member to at least inhibit the securing element from rotating relative to the contact member at least when the securing element is in the deployed state.

6 . The device of claim 1 , wherein the contact member is self-expandable such that at least a portion of the contact member is configured to automatically expand from the contracted state to the expanded state when a restraint is removed from the contact member.

7 . The device of claim 1 , wherein the securing element has a plurality of tissue anchors at a distal end of the plurality of arms configured to engage with an internal wall of a heart, the internal wall being adjacent to the left atrial appendage.

The device of claim 1 , wherein the contact member is configured to be rotatable at least in a first direction from a first position to a second position that is at least 45 degrees from the first position to twist at least a portion of a left atrial appendage in the first direction from a first position to a second position that is at least 45 degrees from the first position of the left atrial appendage to close the left atrial appendage when the contact member is engaged with an inner wall of the left atrial appendage, and the securing element is configured to engage a tissue of a left atrium and/or the left atrial appendage to prevent the left atrial appendage that has been closed from opening back or untwisting to the first position of the left atrial appendage.

The device of claim 1 , wherein the contact member is configured to be rotatable at least in a first direction from a first position to a second position that is at least 45 degrees from the first position to twist at least a portion of a left atrial appendage in the first direction from a first position to a second position that is at least 45 degrees from the first position of the left atrial appendage when the contact member is engaged with an inner wall of the left atrial appendage, and the securing element is configured to engage a tissue of a left atrium and/or the left atrial appendage to prevent the left atrial appendage that has been twisted from untwisting back to the first position of the left atrial appendage.

10 . The device of claim 1 , wherein the securing element is switchable between at least a first state in which the contact member is rotatable relative to the securing element and a second state in which the contact member is prevented from rotating relative to the securing element.

The device of claim 1 , comprising a catheter having an core member selectively coupled with the contact member and configured to rotate the contact member from a first position to a second position when the core member is rotated from a first position to a second position when the contact member is coupled with the core member.

12 . The device of claim 1 , wherein the plurality of struts are coupled with a hub at a distal end of the contact member, the plurality of struts being configured to rotate at a distal end of the plurality of struts relative to the hub.

13 . An implant for reducing a size of an ostium of a left atrial appendage, comprising:

(a) a contact member comprising a plurality of struts that are expandable from a contracted state to an expanded state, wherein the plurality of struts extend from a proximal portion of the contact member to a distal portion of the contact member; and

(b) a securing element comprising a body portion and a plurality of anchors coupled with the body portion and extending away from the body portion and toward the contact member at least when the securing element is in a deployed state, wherein each anchor of the plurality of anchors is separate and discrete from each other anchor of the plurality of anchors;

wherein:

the contact member has an opening at a proximal end thereof sized and configured to receive at least a distal portion of the body portion of the securing element therein; and

the securing element is selectively movable toward and away from the contact member so as to cause the plurality of anchors of the securing element to move toward and away from the contact member; and

the implant is configured such that the body portion of the securing element passes through the opening of the contact member toward a distal end of the contact member as the securing element is moved toward the contact member and such that the body portion of the securing element passes through the opening of the contact member away from the distal end of the contact member as the securing element is moved away from the contact member; and

the implant is configured to cause a tissue of a left atrium and/or the left atrial appendage to constrict around an outer surface of the body portion of the securing element when the contact member is rotated from a first position to a second position, and the securing element is configured to engage with the tissue that has constricted around the outer surface of the body portion of the implant to prevent untwisting of at least a portion of the left atrial appendage back to the first position of the left atrial appendage.

14 . The implant of claim 13 , wherein the contact member is self-expandable such that at least a portion of the contact member is configured to automatically expand from the contracted state to the expanded state when a restraint is removed from the contact member.

15 . The implant of claim 13 , wherein the securing element is selectively securable at any of a range of distances relative to the contact member.

16 . The implant of claim 13 , comprising a retention element comprising a threaded shaft that is coupled with the securing element and the contact member at least when the implant is in a deployed state.

17 . The implant of claim 13 , wherein the contact member has a plurality of anchors or other tissue engaging features on an outside surface thereof.

18 . The implant of claim 13 , wherein the contact member has a plurality of anchors on an outside surface of the contact member and extending in a circumferential direction.

19 . An implant for reducing a size of an ostium of a left atrial appendage, comprising:

(a) a contact member comprising a plurality of struts that are expandable from a contracted state to an expanded state, wherein the plurality of struts extend from a proximal portion of the contact member to a distal portion of the contact member;

(b) a securing element comprising a body portion and a plurality of anchors coupled with the body portion and extending away from the body portion and toward the contact member at least when the securing element is in a deployed state, and

(c) a retention element comprising a threaded shaft, wherein the threaded shaft is configured to be threadably coupled with both the securing element and the contact member at least when the implant is in a deployed state;

wherein:

the contact member has an opening at a proximal end thereof sized and configured to receive at least a distal portion of the body portion of the securing element therein;

the securing element is selectively movable toward and away from the contact member so as to cause the plurality of anchors of the securing element to move toward and away from the contact member; and

the implant is configured such that the body portion of the securing element passes through the opening of the contact member toward a distal end of the contact member as the securing element is moved toward the contact member and such that the body portion of the securing element passes through the opening of the contact member away from the distal end of the contact member as the securing element is moved away from the contact member; and

wherein the implant is configured to cause a tissue of a left atrium and/or the left atrial appendage to constrict around an outer surface of the body portion of the securing element when the contact member is rotated from a first position to a second position, and the securing element is configured to engage with the tissue that has constricted around the outer surface of the body portion of the implant to prevent untwisting of at least a portion of the left atrial appendage back to the first position of the left atrial appendage.
