Abstract

A surgical anchor device for being anchored on the mucous membrane of the inner wall of the intestine, the device having a temporary anchor element presenting anchoring that can be modified in a controlled manner and having a first substantially cylindrical multiply-perforated wall presenting properties of radial elasticity whereby the first wall presents an outer diameter that can be varied in controlled manner, wherein a portion of the inner surface of the first wall is lined with an independent leakproof inner sheath having only its longitudinal ends fastened to the anchor element to define a suction chamber between the inner sheath and the first wall, the temporary anchor element being coupled to a flexible or semi-rigid tube extending outside the anchor element, an open end of the injection-suction tube opening out into the suction chamber enabling air to be injected into or sucked out from the suction chamber.

Claims (41)

1 . An anchoring system for protecting an anastomosis in an intestine during healing, the anchoring system comprising:

a negative pressure anchor configured to be disposed in the intestine upstream of the anastomosis in the intestine, the negative pressure anchor configured to define a suction chamber;

a sheath coupled to the negative pressure anchor, the sheath defining a lumen, wherein the sheath extends downstream from the negative pressure anchor to cover and protect the anastomosis in the intestine from fecal matter flowing through the lumen; and

an external suction device configured to create a vacuum in the suction chamber to cause the negative pressure anchor to be anchored in place at an anchor site in the intestine upstream of the anastomosis,

wherein the negative pressure anchor is configured to remain anchored against an inner wall of the intestine at the anchor site during a period that is after post-operative restarting of a digestive process.

2 . The anchoring system of claim 1 , further comprising a vacuum indicator configured to indicate a degree of vacuum in the suction chamber.

The anchoring system of claim 2 , wherein the external suction device is configured to, responsive to the indication of the degree of vacuum from the vacuum indicator, permit monitoring and control or modification of anchor force of the negative pressure anchor against the inner wall of the intestine.

4 . The anchoring system of claim 1 , wherein the negative pressure anchor comprises pores or mini-perforations.

5 . The anchoring system of claim 1 , wherein the sheath extends through the negative pressure anchor and is coupled to an upstream end and a downstream end of the negative pressure anchor.

6 . The anchoring system of claim 1 , further comprising a tube in fluid connection with the suction chamber and the external suction device, wherein the external suction device is configured to create the vacuum in the suction chamber via the tube.

7 . The anchoring system of claim 6 , wherein the tube is coupled to a connection endpiece comprising a valve.

8 . The anchoring system of claim 6 , further comprising an additional tube for creating the vacuum in the suction chamber.

9 . The anchoring system of claim 1 , further comprising a first leaktight fastener disposed on an upstream end of the negative pressure anchor and a second leaktight fastener disposed on a downstream end of the negative pressure anchor.

10 . The anchoring system of claim 9 , wherein the first and second leaktight fasteners form a leaktight sealed configuration with the inner wall of the intestine at the anchor site positioned upstream from the anastomosis.

11 . The anchoring system of claim 10 , wherein the first and second leaktight fasteners comprise first and second annular gaskets, respectively.

The anchoring system of claim 1 , wherein the negative pressure anchor comprises an expandable downstream region and an expandable upstream region each configured to expand to a diameter greater than that of a cylindrical region therebetween to inhibit migration of the negative pressure anchor from the anchor site.

The anchoring system of claim 1 , wherein the negative pressure anchor comprises radial expansion properties such that the outer surface of the negative pressure anchor is configured to, during peristalsis, continue to contact the inner wall of the intestine in a leaktight sealed configuration.

14 . The anchoring system of claim 1 , wherein the negative pressure anchor has a diameter, in an expanded position, selected such that the outer surface of the negative pressure anchor contacts the inner wall of the intestine for anchoring.

15 . The anchoring system of claim 1 , wherein the sheath is comprised of biocompatible synthetic material.

16 . The anchoring system of claim 1 , wherein the negative pressure anchor comprises texturing on an outer surface of the negative pressure anchor configured to increase a coefficient of friction of the outer surface of the negative pressure anchor to resist migration.

17 . The anchoring system of claim 1 , wherein the negative pressure anchor has a diameter in a range of 18 mm to 45 mm and a length in a range of 40 mm to 150 mm.

18 . The anchoring system of claim 1 , wherein the sheath has a length to extend downstream from the negative pressure anchor to outside an anal orifice.

19 . The anchoring system of claim 1 , wherein the negative pressure anchor is configured to be disposed in the intestine for at least three days until removal.

20 . The anchoring system of claim 1 , wherein the negative pressure anchor comprises a shape memory material.

21 . The anchoring system of claim 1 , wherein the negative pressure anchor comprises a mesh of spiral-wound metal wires made of nitinol.

22 . The anchoring system of claim 1 , wherein the negative pressure anchor comprises a stent.

23 . The anchoring system of claim 1 , wherein the negative pressure anchor comprises two stents configured to be secured together.

24 . The anchoring system of claim 1 , wherein the negative pressure anchor comprises a lasso configured to facilitate release of the negative pressure anchor from the anchor site for removal of the negative pressure anchor from the intestine via the anal orifice after healing of the anastomosis.

25 . The anchoring system of claim 1 , wherein the negative pressure anchor is configured to be removed using a coloscope.

The anchoring system of claim 1 , wherein the suction chamber is configured to receive gas or liquid via a tube in fluid connection with the suction chamber to release the negative pressure anchor from the anchor site for removal from the intestine via the anal orifice after healing of the anastomosis.

27 . A delivery system comprising:

a tubular outer shell configured to hold the anchoring system of claim 1 in a retracted position; and

a pusher tube within the tubular outer shell,

wherein the anchoring system is configured to be introduced into the intestine via an anal orifice and pushed out the tubular outer shell via the pusher tube for anchoring the negative pressure anchor in the intestine upstream of the anastomosis.

28 . An anchoring device for protecting an anastomosis in an intestine during healing, the anchoring device comprising:

a negative pressure anchor configured to be disposed in the intestine upstream of the anastomosis in the intestine, wherein the negative pressure anchor is configured to define a suction chamber;

a texturing on an outer surface of the negative pressure anchor configured to increase a coefficient of friction of the outer surface of the negative pressure anchor to resist migration;

a tube in fluid connection with the suction chamber, wherein negative pressure applied to the suction chamber via the tube creates a vacuum in the suction chamber to anchor the negative pressure anchor in the intestine at an anchor site upstream of the anastomosis,

wherein the negative pressure anchor is configured to remain anchored against an inner wall of the intestine at the anchor site during a period that is after post-operative restarting of a digestive process.

29 . The anchoring device of claim 28 , wherein the negative pressure anchor comprises two stents configured to be secured together.

30 . The anchoring device of claim 28 , wherein the negative pressure anchor comprises pores or mini-perforations.
