Abstract

A hemostasis analyzer, such as the Thrombelastograph® (TEG®) hemostasis analyzer is utilized to measure continuously in real time, the hemostasis process from the initial fibrin formation, through platelet-fibrin interaction and lysis to generate blood hemostasis parameters. The measured blood hemostasis parameters permit preparation of an individualized assessment of ischemic event risk and individualized treatment of a subject.

Claims (21)

1. A method of preparing an individualized ischemic event risk assessment and individualized treatment for a subject, the method comprising:

measuring by thrombelastography a first blood sample portion obtained from a subject to determine a first clot characteristic related quantitative indication of hemostasis of the first blood sample portion;

determining a parameter indicative of the risk of the subject having an ischemic event based upon the first clot characteristic related quantitative indication of hemostasis and determining a treatment for the subject based upon the parameter;

measuring by thrombelastography a second blood sample portion obtained from the subject post in vivo administration of a hemostasis therapy to determine a second clot characteristic related quantitative indication of hemostasis of the second blood sample portion; and

determining a post therapy parameter indicative of the risk of the subject having an ischemic event based upon the second clot characteristic related quantitative indication of hemostasis and determining an efficacy of the hemostasis therapy for the subject based upon the parameters.

2. The method of claim 1 , wherein each of the first blood sample portion and the second blood sample portion are prepared including in vitro administration of an activator.

3. The method of claim 1 , wherein the first and second quantitative indications comprise one of initial clot formation, clot strength, clot elasticity, rate of clot formation or rate of clot lysis.

4. The method of claim 1 , comprising sequentially measuring by thrombelastography the first blood sample portion and the second blood sample portion.

5. The method of claim 1 , comprising providing a first and second hemostasis analyzer, and sequentially measuring by thrombelastography the first and the second blood sample portions.

6. The method of claim 5 , wherein the first and second hemostasis analyzers comprise first and second testing cells of a single hemostasis analyzer.

7. The method of claim 1 , comprising comparing the first and the second clot characteristic quantitative indications to correlation data and determining a dosing parameter of the hemostasis therapy in view of the correlation data.

8. An apparatus for preparing an individualized ischemic event risk assessment and providing individualized hemostasis therapy for a subject comprising:

a hemostasis testing cell for testing a first blood sample portion obtained from a subject pre-treatment to determine by thrombelastographic measurement a first blood sample clot characteristic related haemostasis parameter for the first portion;

means for providing a first parameter indicative of the risk of the subject having an ischemic event based upon the first blood sample clot characteristic related hemostasis parameter;

means for providing an indication of an individualized treatment for the subject based upon the first parameter;

the hemostasis testing cell further for testing a second blood sample portion obtained from the subject—post in vivo administration of a hemostasis therapy to determine by thrombelastographic measurement a second blood sample clot characteristic related haemostasis parameter for the second portion, so that during testing of the second portion the second blood sample clot characteristic related hemostasis parameter is distinguishable from other blood sample hemostasis characteristics; and

means for providing a second parameter indicative of the risk of the subject having an ischemic event based upon the second blood sample clot characteristic related hemostasis parameter and the second parameter being indicative of the efficacy of the hemostasis therapy.

9. The apparatus of claim 8 , wherein the testing cell comprises first and second testing cells of a single testing apparatus.

10. The apparatus of claim 8 further comprising a processor for evaluating the first and second blood sample clot characteristic parameters.

11. The apparatus of claim 10 , wherein the processor is linked to a database comprising correlation data, the processor being configured to determine a dosing parameter of the hemostasis therapy in view of the correlation data.

12. The apparatus of claim 11 , wherein the database is remote from the apparatus, the apparatus being linked to the database via a communication link or network.
