Abstract

Systems and methods for crossing stenosis, partial occlusions, or complete occlusions within a body lumen. The systems generally include an elongate member such as a hollow guidewire that houses a rotatable and translatable drive shaft. The drive shaft typically has a distal portion that is advanced to create a path in the occlusive material that is large enough to allow the hollow guidewire to cross the occlusive material.

Claims (15)

1. A rotatable medical device for crossing through an anatomical structure, the medical device comprising:

a housing;

a motor positioned in the housing;

a hollow guidewire removably coupled to the housing and extending distally therefrom, the hollow guidewire advanceable intravascularly to a location proximate an anatomical structure; and

a drive shaft rotatably disposed within the hollow guidewire, the drive shaft extending distally from the motor positioned in the housing to a distal tip rotatably disposed at a distal end of the hollow guidewire;

the drive shaft being rotatable within the hollow guidewire with the motor to thereby rotate the distal tip;

wherein the housing and the motor are decouplable from the hollow guidewire and the drive shaft, respectively, while the drive shaft remains positioned within the hollow guidewire such that the hollow guidewire is configured to permit the advancement of an access device over the hollow guidewire to an anatomical location after decoupling the housing from the hollow guidewire.

2. The rotatable medical device of claim 1 , wherein the hollow guidewire has an outer diameter of between approximately 0.009 inches and approximately 0.040 inches.

3. The rotatable medical device of claim 1 , further comprising a luer assembly releasably coupling the hollow guidewire and the housing.

4. The rotatable medical device of claim 1 , wherein the motor and drive shaft are coupled together with a detachable linkage assembly.

5. The rotatable medical device of claim 1 , wherein the distal tip is configured to create an opening having a radius at least as large as a radius of the distal end of the hollow guidewire.

6. The rotatable medical device of claim 1 , wherein the hollow guidewire includes a coil proximate the distal end of the hollow guidewire.

7. The rotatable medical device of claim 1 , wherein the drive shaft is rotatable relative to the hollow guidewire.

8. The rotatable medical device of claim 1 , wherein the distal tip extends distal of the distal end of the hollow guidewire.

9. The rotatable medical device of claim 1 , wherein the hollow guidewire is flexible to navigate through a tortuous vasculature.
