Abstract

The invention provides a method and apparatus for cranial fixation following a craniotomy and treatment for increased intracranial pressure. The cranial fixation device comprises two heads connected by a telescopic extension with one head attached to the skull and the other to the bone flap. The telescopic extension provides constrained movement of the bone flap relative to the skull to accommodate an increase in the intracranial pressure.

Claims (35)

1. A method comprising the steps of: creating a skull defect and removing a bone flap from a skull; securing a first portion of a fixation device to the skull; and securing a second portion of the fixation device to the hone flap; the first and second portions of said fixation device connected by a telescopic portion residing in the skull defect and allowing for movement of the bone flap relative to the skull.

2. The method of claim 1 , wherein the bone flap moves outward to accommodate an increase in the intracranial pressure and moves inward up to the skull when the intracranial pressure normalizes.

The method of claim 2 , wherein the increase in the intracranial pressure is from one or more of the following: traumatic injury, subdural hemorrhage, epidural hemorrhage, subarachnoid hemorrhage, intra-ventricular hemorrhage, brain hemorrhage, ischemic stroke, hemorrhagic stroke, hypoxia, tumor, infection, brain swelling, and/or seizure.

4. The method of claim 1 , wherein the telescopic portion placed in the retracted state anatomically approximates the skull and the bone flap.

5. The method of claim 1 , wherein the telescopic portion is placed in an extended state by an increase in intracranial pressure.

6. The method of claim 1 , wherein the device prevents the outer surface of the bone flap from moving inward beyond the outer surface of the skull.

7. The method of claim 1 , wherein the telescopic portion comprising of an engaging mechanism which prevents the telescopic portions from pulling out beyond a completely extended position.

8. The method of claim 1 , wherein the first portion is positioned on the outer surface of the skull and the second portion is positioned on the outer surface of the bone flap.

9. The method of claim 1 , wherein the telescopic portion comprises of two extensions: one extension connected to the said head attached to the skull and the second extension connected to the said head attached to the bone flap.

10. The method of claim 1 , wherein the telescopic portion is selected from a group of telescopic segments of various size and configuration.

11. The method of claim 10 , wherein the telescopic portions configuration is one or more of the following: hollow, solid, tapered, semi-circular, rectangular, tubular, oval, square, flat, semi-oval, L-shape, C-shape, V-shape, X-shape.

12. The method of claim 1 , where in the first and second portions are selected from a group of head segments of various size and configuration.

13. The cranial fixation device of claim 12 , wherein the head configuration being one of the following: round, oval, rectangular, square, semi-circular, semi-oval, C-shape, L-shape, T-shape, X-shape, V-shape, Z-shape, fan shaped.

14. The cranial fixation device of claim 1 , wherein the device portions cover at least a portion of a burr hole or skull opening.

The method of claim 1 , wherein said fixation device being made of a physiological compatible substance comprising of one or more of the following: bio-absorbable material radiolucent material, metal titanium, titanium alloy, plastic, bone, carbon fiber, ultra high molecular weight polyethylene, polyaryletherketone, polyetheretherketone.

16. The method of claim 1 , wherein securing the device portions to the skull and/or bone flap comprises attaching through one or more bone fasteners.

17. The method of claim 16 , wherein the bone fastener comprises one or more of the following: screw, self-tapping screw, self-drilling screw, wires.

18. The method of claim 1 , wherein the telescopic portion comprises of one extension connected to the portion attached to the skull, a second extension connected to the portion attached to the bone flap, and one or more intermediate extensions.

19. The method of claim 1 , wherein the telescopic portion comprising of a locking mechanism.

20. The method of claim 19 , wherein said locking mechanism engages when the telescopic portion is in one or more of the following positions: a maximal retracted state, maximal extended state, any position between maximal retracted and extended state.

21. The method of claim 19 , wherein the locking mechanism comprises of ridges in said first telescopic extension with notches in said second telescopic extension.

22. A method comprising the steps of: creating a skull defect and removing a bone flap from a skull; securing a first portion of a fixation device to the skull; and securing a second portion of the fixation device to the bone flap; the first and second portions of said fixation device connected by a telescopic portion residing in the skull defect allowing for movement of the bone flap relative to the skull and comprising of a locking mechanism.

23. The method of claim 22 , wherein the locking mechanism comprises of one or more of the following: ratchet teeth, ridges, ball and socket mechanism, collapsible ratchet teeth, hook.

24. The method of claim 22 , wherein the bone flap moves outward to accommodate an increase in the intracranial pressure and moves inward up to the skull when the intracranial pressure normalizes.

The method of claim 24 , wherein the increase in the intracranial pressure is from one or more of the following: traumatic injury, subdural hemorrhage, epidural hemorrhage, subarachnoid hemorrhage, intra-ventricular hemorrhage, brain hemorrhage, ischemic stroke, hemorrhagic stroke, hypoxia, tumor, infection, brain swelling, and/or seizure.

26. The method of claim 22 , wherein the telescopic portion placed in the retracted state engages the locking mechanism and anatomically approximates the skull and the bone flap.

27. The method of claim 22 , wherein the telescopic portion is placed in an extended state from a retracted state in response to an increase in intracranial pressure; the locking mechanism prevents the telescopic portions from pulling out beyond a completely extended position.

28. The method of claim 22 , wherein the telescopic portion locking mechanism engages in a retracted state; the said locking mechanism being disengaged by an increase in intracranial pressure placing the telescopic extension in an extended state.

29. The method of claim 22 , wherein securing the portion to the skull and/or bone flap comprises attaching through one or more bone fasteners.

30. The method of claim 29 , wherein the bone fastener comprises one or more of the following: screw, self-tapping screw, self-drilling screw, wires.

A method comprising the steps of: creating a skull defect and creating a bone flap from the skull; securing a first portion of a fixation device to the skull and securing a second portion of the fixation device to the bone flap; the first and second portions of said fixation device connected by a telescopic portion comprising of two or more connectors residing in the skull defect and allowing for movement of the bone flap relative to the skull.

32. The method of claim 31 , wherein the first portion is positioned on the outer surface of the skull and the second portion is positioned on the outer surface of the bone flap.

33. The method of claim 31 , wherein the telescopic portion comprises of two or more inter-connected extensions with one extension connected to the portion attached to the skull and another extension connected to the portion attached to the bone flap.

34. The method of claim 31 , wherein securing the portion to the skull and/or bone flap comprises attaching through one or more bone fasteners.

35. The method of claim 34 , wherein the bone fastener comprises one or more of the following: screw, self-tapping screw, self-drilling screw, wires.
