Neuromodulation methods using balloon catheter
According to some embodiments, a method of treating a subject having diabetes or symptoms associated with diabetes is provided. The method includes delivering a neuromodulation catheter within a vessel (e.g., hepatic artery) having surrounding nerves that innervate the liver (e.g., sympathetic nerves of the hepatic plexus). The method may also include modulating (e.g., disrupting, ablating, stimulating) the nerves by mechanical compression, energy delivery, or fluid delivery.
1. A method of facilitating modulation of nerves surrounding a hepatic artery of a subject, comprising:
inserting a balloon catheter into a vasculature of a subject,
wherein the balloon catheter comprises a balloon having at least one electrode positioned on an external surface of the balloon;
advancing a distal section of the balloon catheter to a location within a portion of a hepatic artery of the vasculature;
at least partially occluding the portion of the hepatic artery using the balloon catheter,
causing the at least one electrode to contact a wall of the portion of the hepatic artery at a target site;
denervating the portion of the hepatic artery at the target site using the balloon catheter sufficient to reduce a blood glucose level of the subject; and
removing the balloon catheter from the vasculature of the subject.
2. The method of claim 1 , wherein at least partially occluding the portion of the hepatic artery comprises fully occluding the portion of the hepatic artery.
3. The method of claim 1 , wherein the denervating is effective to reduce at least one of a hepatic norepinephrine or lipid level in the subject.
4. The method of claim 1 , wherein denervating the portion of the hepatic artery at the target site comprises denervating one or more sympathetic nerves of the hepatic plexus.
5. The method of claim 1 , wherein a contact pressure between 0.1 g/mm 2 and 100 g/mm 2 is exerted on the wall by the balloon catheter.
6. The method of claim 1 , wherein the location within the portion of the hepatic artery is within at least one of a common hepatic artery, a proper hepatic artery, a right hepatic artery or a left hepatic artery.
7. The method of claim 1 , further comprising positioning the balloon catheter within a gastroduodenal artery and denervating one or more nerves surrounding the gastroduodenal artery.
8. The method of claim 4 ,
wherein the balloon catheter is configured to deliver RF energy at a frequency in the range of 50 kHz to 5 MHz; and
wherein the RF energy delivered is sufficient to cause permanent ablation of the one or more sympathetic nerves.
9. The method of claim 1 , further comprising providing cooling to a portion of the wall adjacent to the target site.
10. A method of facilitating modulation of nerves surrounding a hepatic artery of a subject comprising:
inserting a balloon catheter into a vasculature of a subject,
wherein the balloon catheter comprises a catheter body and a distal balloon having at least one electrode positioned on an external surface of the balloon;
advancing a distal section of the balloon catheter to a location within a portion of a hepatic artery of the vasculature;
at least partially occluding the portion of the hepatic artery using the balloon catheter,
causing the at least one electrode to contact a wall of the portion of the hepatic artery at a target treatment site;
modulating one or more nerves that innervate at least one of a liver or a pancreas of the subject at the target treatment site using the balloon catheter sufficient to reduce a blood glucose level of the subject; and
removing the balloon catheter from the vasculature of the subject.
11. The method of claim 10 , wherein the balloon catheter comprises a plurality of electrodes configured to contact the wall at multiple locations.
12. The method of claim 10 , wherein the catheter body comprises a lumen configured to continuously infuse a fluid into the balloon.
13. The method of claim 10 , wherein the balloon comprises one or more hydrogel portions spaced around a circumference of the balloon.
14. The method of claim 10 , wherein the balloon catheter comprises two balloons, each balloon comprising at least one electrode, and wherein the method further comprises positioning a first balloon in a first branch of the hepatic artery and a second balloon in a second branch of the hepatic artery.
15. The method of claim 10 , wherein modulating one or more nerves that innervate at least one of the liver or the pancreas comprises denervating one or more sympathetic nerves of a hepatic plexus.
16. The method of claim 10 ,
wherein the balloon catheter is configured to deliver RF energy at a frequency in the range of 50 kHz to 5 MHz; and
wherein the RF energy is delivered to heat one or more sympathetic nerves adjacent to the wall of the hepatic artery to a temperature of up to 90 degrees Celsius and sufficient to cause ablation of the one or more sympathetic nerves.
17. A method of facilitating modulation of nerves surrounding a hepatic artery of a subject comprising:
providing an energy delivery device comprising an expandable balloon having at least one electrode, the energy delivery device configured for:
intravascularly accessing a portion of a hepatic artery;
causing the at least one electrode of the expandable balloon to contact a wall of the hepatic artery; and
delivering energy through the wall of the hepatic artery sufficient to denervate one or more sympathetic nerves associated with a liver.
18. The method of claim 17 ,
wherein the energy comprises radiofrequency (RF) energy at a frequency in the range of 50 kHz to 5 MHz, and
wherein the RF energy delivered is sufficient to cause ablation of the one or more sympathetic nerves.
19. The method of claim 17 , further comprising occluding the portion of the hepatic artery.
20. The method of claim 17 , wherein the denervation is effective to reduce at least one of a hepatic norepinephrine level, lipid level or blood glucose level in the subject.
