Abstract

A spinal implant comprises an implant body extending between an anterior surface and a posterior surface, and including a first vertebral engaging surface and a second vertebral engaging surface. The implant body further including an inner surface that defines at least a first cavity and a second cavity spaced from the first cavity. The first cavity is oriented to implant a fastener oblique relative to a bilateral axis of a subject body. The second cavity is oriented to implant a fastener in substantial alignment with the bilateral axis. Systems and methods of use are disclosed.

Claims (32)

1. A spinal implant system comprising:

an implant extending between an anterior surface and a posterior surface, and including a first vertebral engaging surface and a second vertebral engaging surface, the implant further including a first inner surface that defines at least a first cavity and a second inner surface that defines a second cavity spaced from the first cavity, the first cavity being oriented to implant a fastener oblique relative to a bilateral axis of a subject body and the second cavity being oriented to implant a fastener in substantial alignment with the bilateral axis, the implant including a bore that extends transverse to each of the cavities;

a member positioned in the bore such that a first through hole of the member is aligned with the first cavity and a second through hole of the member is aligned with the second cavity, the through holes each having a circular cross section;

a plate connected with the implant via the fastener; and

a guide configured to connect the plate and the fastener with the implant along a surgical pathway.

A spinal implant system as recited in claim 1 , further comprising an inserter that mates with the guide, the inserter comprising a removable shaft that extends through the inserter between opposite proximal and distal ends, the distal end comprising a threaded portion configured to engage the member.

3. A spinal implant system as recited in claim 2 , wherein the proximal end comprises an actuator configured to manipulate engagement of the threaded portion with the member.

4. A spinal implant system as recited in claim 3 , wherein:

the actuator is rotated in a first direction to engage the threaded portion with a threaded portion of the member such that the shaft engages the member causing the inserter to become rigid; and

the actuator is rotated in an opposite second direction such that the threaded portion partially disengages from the member to facilitate relative freedom of movement such that a practitioner can maneuver the implant.

5. A spinal implant system as recited in claim 2 , wherein the shaft is inserted through the inserter, the guide, the plate and into the member such that the threaded portion engages the member.

6. A spinal implant system as recited in claim 1 , wherein

the system further comprises a first fastener that extends through a first opening in the plate, the first cavity and the first through hole and a second fastener that extends through a second opening in the plate, the second cavity and the second through hole.

A spinal implant system as recited in claim 6 , wherein the first fastener comprises a threaded outer surface that engages threads of the first cavity and threads of the first through hole and the second fastener comprises a threaded outer surface that engages threads of the second cavity and threads of the second through hole.

A spinal implant system as recited in claim 6 , wherein the plate comprises a plurality of spaced apart third openings and a back out element movable between a first orientation in which the back out element overlaps the third openings and a second orientation in which the back out element is spaced apart from the third openings.

A spinal implant system as recited in claim 1 , further comprising a first fastener that extends through a first opening in the plate, the first cavity and one of the through holes and a second fastener that extends through a second opening in the plate, the second cavity and one of the through holes.

10. A spinal implant system as recited in claim 9 , wherein the fasteners each extend through opposite inner and outer surfaces of the plate.

11. A spinal implant system as recited in claim 1 , wherein:

the bore is defined by a third inner surface of the implant, the third inner surface defining opposing first and second limit surfaces; and

the member includes a first end having a stop configured to engage one of the limit surfaces.

A spinal implant system as recited in claim 11 , wherein the limit surfaces are engageable with the stop to limit a range of movement of the member between a first movable limit defined by engagement with the first limit surface, in a first direction, and a second movable limit defined by engagement with the second limit surface in an opposite second direction.

13. A spinal implant system as recited in claim 1 , wherein the first vertebral engaging surface and the second vertebral engaging surface are alternately disposable in a cephalad orientation of the subject body.

14. A spinal implant system as recited in claim 1 , wherein the first cavity is disposable in alignment with an oblique surgical pathway or the second cavity is disposable in alignment with an anterior surgical pathway.

15. A spinal implant system as recited in claim 1 , wherein the member is rotatable relative to the plate.

16. A spinal implant system as recited in claim 1 , wherein the member is rotatable relative to the plate in a range of rotation of approximately 10 degrees in a first direction and approximately 10 degrees in a second direction.

17. A spinal implant system as recited in claim 1 , wherein the implant comprises opposite first and second side surfaces that extend between the anterior and posterior surfaces and between the engaging surfaces, the first cavity extending through the anterior surface and one of the side surfaces.

18. A spinal implant system as recited in claim 1 , wherein the implant comprises opposite first and second side surfaces that extend between the anterior and posterior surfaces and between the engaging surfaces, the bore extending through each of the side surfaces.

19. A spinal implant system comprising:

an implant extending between an anterior surface and a posterior surface, and including a first vertebral engaging surface and a second vertebral engaging surface, the implant further including a first inner surface that defines at least a first cavity and a second inner surface that defines a second cavity spaced from the first cavity, the first cavity being oriented to implant a fastener oblique relative to a bilateral axis of a subject body and the second cavity being oriented to implant a fastener in substantial alignment with the bilateral axis, the implant including a bore that extends transverse to each of the cavities;

a member positioned in the bore such that through holes of the member are aligned with the cavities the through holes each having a circular cross section; and

a plate connected with the implant via the fastener,

wherein the through holes include a first threaded through hole and a second threaded through hole, the first threaded through hole being aligned with the first cavity and the second threaded through hole being aligned with the second cavity, and the system further comprises a first fastener having a threaded outer surface that extends through a first opening in the plate, the first cavity and the first through hole and a second fastener having a threaded outer surface that extends through a second opening in the plate, the second cavity and the second through hole.
