Abstract

A facet joint replacement system includes an inferior implant with an inferior articular surface, a superior implant with a superior articular surface, and an optional crossbar. The inferior implant and the superior implant are each polyaxially adjustably connected to fixation elements which anchor the implants to adjacent vertebrae. The optional crossbar may be polyaxially adjustably connected to bilateral implants. The system components may be provided in kits which provide components of various sizes and shapes. A set of surgical instruments may facilitate implantation of the facet joint replacement system by providing tools for bone preparation, trialing, implant insertion, implant alignment, and lock-out of modular interconnections.

Claims (48)

1. A method of threading a screw into a bone, the method comprising:

coupling a driver to a first end of the screw, the screw having a proximal threaded attachment portion,

wherein the driver comprises:

an outer sleeve,

an inner shaft in a fixed rotational alignment to the outer sleeve,

a screw coupling, wherein the screw coupling is formed by a blunt end of the inner shaft and an internally threaded distal end of the outer sleeve having threads complimentary to the threaded attachment portion of the screw, wherein the screw coupling has a locked configuration in which the blunt end of the inner shaft is in contact with the screw to retain the screw in the outer sleeve, and an unlocked configuration in which the blunt end of the inner shaft is not in contact with the screw and the screw is free to separate from the driver,

a handle, and

a lever attached to the handle, wherein when the lever is lifted away from the handle, the inner shaft is pulled away from the distal end of the outer sleeve, and when the lever is moved toward the handle, the inner shaft is moved toward the internally threaded distal end of the outer sleeve, thereby moving the screw coupling into the locked configuration;

threading a second end of the screw into the bone until a predetermined length of the first end of the screw protrudes from the bone;

decoupling the driver from the screw; and

attaching a broach to the proximal threaded attachment portion of the screw after decoupling the driver.

2. The method of claim 1 , comprising:

transmitting torque from the driver to a drive feature on the screw, wherein the drive feature is contained within a boundary defined by a shaft diameter at the first end of the screw.

3. The method of claim 1 , wherein:

in the unlocked configuration,

the blunt end of the inner shaft is distant from the distal end of the outer sleeve; and

in the locked configuration,

the blunt end of the inner shaft is close to the distal end of the outer sleeve.

4. The method of claim 1 , wherein the predetermined length is established when a first end of the driver contacts the bone around the screw to prevent the screw from advancing further into the bone.

5. The method of claim 4 , comprising: cutting at least a portion of the bone around the screw with the first end of the driver, wherein the first end of the driver comprises a replaceable reamer having cutting flutes oriented for at least one of side cutting and end cutting.

6. The method of claim 1 , wherein the broach is advanced into the bone until the broach penetrates to a predetermined depth relative to an exposed proximal end of the screw.

7. The method of claim 1 , wherein the screw is slideably coupled to the driver.

8. The method of claim 1 , wherein the screw has a distal threaded bone engaging portion.

9. A method of threading a screw into a bone, the method comprising:

coupling a driver over a threaded first end of the screw, wherein the coupling driver comprises an outer sleeve, an inner shaft, a screw coupling, a handle, and a lever attached to the handle, wherein the screw coupling is formed by a blunt end of the inner shaft and an internally threaded distal end of the outer sleeve, wherein the screw coupling has a locked configuration in which the blunt end of the inner shaft is in contact with the screw to retain the screw in the outer sleeve, and an unlocked configuration in which the blunt end of the inner shaft is not in contact with the screw and the screw is free to separate from the driver, wherein when the lever is lifted away from the handle, the inner shaft is pulled away from the distal end of the outer sleeve, and when the lever is moved toward the handle, the inner shaft is moved toward the internally threaded distal end of the outer sleeve, thereby moving the screw coupling into the locked configuration;

threading a second end of the screw into the bone until a predetermined length of the first end of the screw protrudes from the bone;

decoupling the driver from the screw; and

attaching a broach to the threaded first end of the screw after decoupling the driver.

10. The method of claim 9 , comprising:

transmitting torque from the driver to a drive feature on the screw, wherein the drive feature is bounded by a shaft diameter at the first end of the screw.

11. The method of claim 10 , wherein:

the drive feature comprises the threaded first end of the screw, wherein a major diameter of the threaded first end is bounded by the shaft diameter.

12. The method of claim 9 , wherein:

in the unlocked configuration, the blunt end of the inner shaft is displaced within the distal end of the outer sleeve by a distance greater than the predetermined length; and

in the locked configuration, the blunt end of the inner shaft is displaced within the distal end of the outer sleeve by the predetermined length.

13. The method of claim 9 , wherein the predetermined length is established when a first end of the driver contacts the bone around the screw to prevent the screw from advancing further into the bone.

14. The method of claim 13 , comprising: cutting at least a portion of the bone around the screw with the first end of the driver, wherein the first end of the driver comprises a replaceable reamer having cutting flutes oriented for at least one of side cutting and end cutting.

15. A method of threading a screw into a bone, the method comprising:

coupling a driver over a threaded first end of the screw, wherein the driver comprises:

an outer sleeve,

an inner shaft in a fixed rotational alignment to the outer sleeve,

a screw coupling, wherein the screw coupling is formed by a blunt end of the inner shaft and an internally threaded distal end of the outer sleeve, wherein the screw coupling has a locked configuration in which the blunt end of the inner shaft is in contact with the screw to retain the screw in the outer sleeve, and an unlocked configuration in which the blunt end of the inner shaft is not in contact with the screw and the screw is free to separate from the driver;

a handle, and

a lever attached to the handle, wherein when the lever is lifted away from the handle, the inner shaft is pulled away from the distal end of the outer sleeve, and when the lever is moved toward the handle, the inner shaft is moved toward the internally threaded distal end of the outer sleeve, thereby moving the screw coupling into the locked configuration;

threading a second end of the screw into the bone until a predetermined length of the first end of the screw protrudes from the bone; and

decoupling the driver from the screw;

attaching a broach to the threaded first end the screw after decoupling the driver from the screw,

wherein coupling and decoupling the driver and the screw are governed by relative translation of the outer sleeve and the inner shaft.
