Abstract

According to some embodiments, a method of treating a subject having diabetes or symptoms associated with diabetes is provided. The method includes delivering a neuromodulation catheter within a vessel (e.g., hepatic artery) having surrounding nerves that innervate the liver (e.g., sympathetic nerves of the hepatic plexus). The method may also include modulating (e.g., disrupting, ablating, stimulating) the nerves by mechanical compression, energy delivery, or fluid delivery.

Claims (49)

1. A method of facilitating modulation of nerves surrounding a hepatic vessel, the method comprising:

inserting an energy delivery device within vasculature of a subject;

intravascularly advancing a distal portion of the energy delivery device to a location within a portion of a common hepatic artery; and

delivering energy sufficient to ablate nerves surrounding the common hepatic artery,

wherein the nerves innervate at least one organ of the subject; and

removing the energy delivery device from the subject.

2. The method of claim 1 , wherein the at least one organ comprises a liver, and wherein the nerves comprise sympathetic nerves that innervate the liver.

3. The method of claim 1 , wherein the delivered energy is radiofrequency energy having a frequency in a range of 50 kHz to 5 MHz.

4. The method of claim 1 , further comprising confirming ablation of the nerves.

5. The method of claim 3 , wherein:

the frequency is between 100 kHz and 2.5 MHz,

a distal portion of the energy delivery device comprises at least one electrode, and

wherein the distal portion of the energy delivery device is configured to cause maintained contact of the at least one electrode with an inner wall of the common hepatic artery.

6. The method of claim 1 , wherein the at least one organ comprises a liver and at least one of a pancreas, a small intestine and a stomach, and wherein the nerves comprise nerves that innervate the liver and nerves that innervate at least one of the pancreas, the small intestine and the stomach.

7. The method of claim 1 , wherein the at least one organ comprises a pancreas and wherein the nerves comprise sympathetic nerves that innervate the pancreas.

8. The method of claim 1 , further comprising positioning the energy delivery device within a portion of a proper hepatic artery and delivering energy sufficient to ablate nerves surrounding the proper hepatic artery.

9. A method of facilitating modulation of nerves surrounding a hepatic vessel, the method comprising:

inserting an energy delivery device within vasculature of a subject;

intravascularly advancing a distal portion of the energy delivery device to a location within a portion of a hepatic artery; and

delivering energy sufficient to modulate nerves surrounding the hepatic artery,

wherein the nerves innervate at least one organ of the subject; and

removing the energy delivery device from the subject.

10. The method of claim 9 ,

wherein the delivered energy is radiofrequency energy having a frequency in a range of 50 kHz to 5 MHz,

wherein the at least one organ comprises a liver,

wherein the nerves comprise sympathetic nerves that innervate the liver, and

wherein the delivered energy heats the nerves to a temperature of up to 90 degrees Celsius and sufficient to cause denervation of the nerves.

11. The method of claim 9 , wherein the delivered energy is ultrasound energy having a frequency in a range of 20 kHz to 20 MHz.

12. The method of claim 10 , wherein:

the frequency is between 100 kHz and 2.5 MHz,

a distal portion of the energy delivery device comprises at least one electrode, and

wherein the distal portion of the energy delivery device is configured to cause maintained contact of the at least one electrode with an inner wall of the hepatic artery.

13. The method of claim 12 , wherein the at least one electrode is configured to maintain contact with the inner wall at a contact pressure of between 0.1 g/mm 2 and 10 g/mm 2 .

The method of claim 10 , wherein the at least one organ comprises a liver and at least one of a pancreas, a small intestine and a stomach, and wherein the nerves comprise nerves that innervate the liver and nerves that innervate at least one of the pancreas, the small intestine and the stomach.

15. The method of claim 10 , further comprising positioning the energy delivery device within a portion of a gastroduodenal artery and delivering energy sufficient to denervate nerves surrounding the gastroduodenal artery.

16. The method of claim 15 , wherein the nerves surrounding the gastroduodenal artery comprise sympathetic nerves that innervate at least one of: a pancreas, a small intestine of the subject and a stomach of the subject.

17. A method of facilitating modulation of nerves surrounding a hepatic vessel, the method comprising:

causing energy to be delivered by at least one energy delivery element of an energy delivery device positioned to deliver energy to nerves innervating at least one organ surrounding a hepatic artery from a location within the hepatic artery,

wherein the at least one organ comprises a pancreas,

wherein the energy is delivered at a frequency in a range of 3 kHz to 300 GHz, and

wherein the delivered energy is sufficient to denervate the nerves.

18. The method of claim 17 ,

wherein the hepatic artery is a common hepatic artery,

wherein the energy delivery element comprises at least one radiofrequency electrode,

wherein the nerves comprise sympathetic nerves,

wherein the energy is delivered to the nerves at a frequency in a range of 100 kHz to 2.5 MHz, and

wherein the energy is delivered to heat the nerves to a temperature of up to 90 degrees Celsius and sufficient to cause ablation of the nerves.

The method of claim 18 , further comprising positioning the energy delivery device in a proper hepatic artery and causing energy to be delivered by at least one energy delivery element of the energy delivery device to nerves innervating the pancreas from a location within the proper hepatic artery.

20. The method of claim 18 , further comprising positioning the energy delivery device in a gastroduodenal artery and causing energy to be delivered by at least one energy delivery element of the energy delivery device to nerves innervating the pancreas from a location within the gastroduodenal artery.
