Abstract

Knee prostheses that more faithfully replicate the structure and function of the human knee joint in order to provide, among other benefits: greater flexion of the knee in a more natural way by promoting or accommodating internal tibial rotation, replication of the natural screw home mechanism, and controlled articulation of the tibia and femur respective to each other in a natural way. Such prostheses may include an insert component disposed between a femoral component and a tibial component, the insert component featuring, among other things, a reversely contoured posterolateral bearing surface that helps impart internal rotation to the tibia as the knee flexes. Other surfaces can be shaped using iterative automated techniques that allow testing and iterative design taking into account a manageable set of major forces acting on the knee during normal functioning, together with information that is known about natural knee joint kinetics and kinematics.

Claims (37)

1. A prosthesis for replacement of a least part of a knee joint in a leg, the leg including a femur and a tibia, the prosthesis comprising:

a femoral component adapted to fit on a distal end of the femur, the femoral component comprising:

a lateral condylar structure defining a lateral condylar bearing surface; and

a medial condylar structure defining a medial condylar bearing surface;

a tibial component adapted to fit on a proximal end of the tibia, the tibial component comprising:

a medial portion comprising a medial bearing surface configured to articulate with the medial condylar bearing surface when the prosthesis is installed and a lateral portion comprising a lateral bearing surface configured to articulate with the lateral condylar bearing surface when the prosthesis is installed, wherein:

the medial bearing surface comprises an anterior-most edge and a posterior-most edge;

the lateral bearing surface comprises an anterior-most edge and a posterior-most edge;

the medial bearing surface has a minimum thickness that is thicker in a superior-inferior direction at the medial bearing surface posterior-most edge than a minimum thickness of the lateral bearing surface at the lateral bearing surface posterior-most edge; and

the medial bearing surface has a minimum thickness that is thinner in a superior-inferior direction at the medial bearing surface anterior-most edge than a minimum thickness of the lateral bearing surface at the lateral bearing surface anterior-most edge.

2. The prosthesis of claim 1 , further comprising a central post positioned between the medial bearing surface and the lateral bearing surface.

3. The prosthesis of claim 1 , wherein the lateral bearing surface comprises a portion that is concave.

The prosthesis of claim 1 , wherein the posterior-most edge of the medial bearing surface is an outermost edge of the medial bearing surface in a posterior direction and the posterior-most edge of the lateral bearing surface is an outermost edge of the lateral bearing surface in the posterior direction.

5. The prosthesis of claim 1 , wherein the medial bearing surface is continuously concave in an anterior-posterior direction.

6. The prosthesis of claim 1 , wherein the thickness difference at the medial and lateral posterior-most edges is configured to increase internal rotation of the tibial component relative to the femoral component during flexion.

7. The prosthesis of claim 6 , wherein:

the femoral component further comprises a cam between the medial and lateral condylar structures; and

the tibial component further comprises a raised post between the medial and lateral bearing surfaces, the tibial component comprising a posterior surface configured to contact the cam of the femoral component at least at some flexion angles to assist the internal rotation of the tibial component relative to the femoral component during flexion.

8. The prosthesis of claim 1 , wherein the minimum thickness of the lateral bearing surface at the lateral bearing surface anterior-most edge is thicker than the minimum thickness of the lateral bearing surface at the lateral bearing surface posterior-most edge.

9. The prosthesis of claim 8 , wherein the medial bearing surface curves upwardly towards the posterior-most edge, and wherein the lateral bearing surface is relatively flat towards the posterior-most edge, relative to the medial bearing surface.

10. The prosthesis of claim 9 , wherein the tibial component further comprises an anterior chamfer between the medial and lateral portions of the tibial component, wherein the anterior chamfer is configured to provide clearance for a patellar tendon of the knee joint.

11. The prosthesis of claim 10 , wherein the tibial component further comprises a posterior chamfer, wherein the posterior chamfer is configured to provide clearance for the femur of the knee joint during flexion.

The prosthesis of claim 1 , wherein, during flexion moving from full extension, the femoral component and the tibial component move relative to one another such that the medial condylar bearing surface and the lateral condylar bearing surface move posteriorly relative to the tibial component, wherein the lateral condylar bearing surface move further in a posterior direction compared to the medial condylar bearing surface.

13. The prosthesis of claim 1 , wherein the femoral component and the tibial component are configured to articulate relative to one another through a range of flexion including angles of approximately 0 degrees and approximately 150 degrees.

14. The prosthesis of claim 1 , wherein the medial portion extends further in a posterior direction than the lateral portion.

15. A prosthesis for replacement of a least part of a knee joint in a leg, the leg including a femur and a tibia, the prosthesis comprising:

a femoral component adapted to fit on a distal end of the femur, the femoral component comprising:

a lateral condylar structure defining a lateral condylar bearing surface; and

a medial condylar structure defining a medial condylar bearing surface;

a tibial component adapted to fit on a proximal end of the tibia, the tibial component comprising:

a medial portion comprising a medial bearing surface and a lateral portion comprising a lateral bearing surface, the medial bearing surface comprising a posterior-most edge and the lateral bearing surface comprising a posterior-most edge, wherein:

the medial bearing surface is continuously concave in an anterior-posterior direction; and

the medial bearing surface has a greater distance between a most distal point of the medial bearing surface and the posterior-most edge of the medial bearing surface in a superior-inferior direction compared to a distance between a most distal point of the lateral bearing surface and the posterior-most edge of the lateral bearing surface in a superior-inferior direction.

16. The prosthesis of claim 15 , further comprising a central post positioned between the medial bearing surface and the lateral bearing surface.

17. The prosthesis of claim 15 , wherein the lateral bearing surface comprises a portion that is concave.

18. The prosthesis of claim 15 , wherein the posterior-most edge of the medial bearing surface is an outermost edge of the medial bearing surface in a posterior direction and the posterior-most edge of the lateral bearing surface is an outermost edge of the lateral bearing surface in the posterior direction.

19. The prosthesis of claim 15 , wherein the medial portion extends further in a posterior direction than the lateral portion.
