Abstract

Systems, methods, and devices are provided for the treatment of edema. In one aspect a method for implanting an indwelling catheter within a vein of a patient is provided. The catheter can extend from a position upstream of at least one outflow port of a duct of the lymph system to a terminal position downstream of the at least one outflow port. In use, a first restriction can be created within the vein proximal to a distal region of the catheter. The first restriction can define a localized low pressure zone distal of the restriction and within a portion of the vein housing the catheter. The low pressure zone can be adjacent to the at least one outflow port to enable fluid to pass from the at least one lymph duct outflow port into the vein.

Claims (23)

1. A method of treating edema in a subject, the method comprising: reducing pressure at a region of outflow from a thoracic duct and/or a lymphatic duct while maintaining intravascular pressure of a subject, thereby increasing flow through the thoracic duct and/or the lymphatic duct, and treating edema in the subject.

2. The method of claim 1 , wherein reducing pressure at the region of outflow from the thoracic duct and/or the lymphatic duct includes inserting a device into a body lumen of a patient and positioning the device in a vicinity of the region of outflow from the thoracic duct and/or the lymphatic duct.

3. The method of claim 2 , further comprising deploying from the device at least a first restrictor within the body lumen.

4. The method of claim 3 , further comprising operating an impeller proximate the restrictor to reduce the pressure.

5. The method of claim 3 , further comprising introducing by the device a second restrictor within the body lumen, wherein the pressure is reduced between the first restrictor and the second restrictor.

6. The method of claim 5 , further comprising positioning the first restrictor and the second restrictor to flank the region of outflow from the thoracic duct and/or the lymphatic duct.

7. The method of claim 5 , further comprising operating a pump to withdraw fluid from the subject through a suction lumen of the device.

The method of claim 7 , further comprising positioning the first restrictor and the second restrictor to flank the region of outflow from the thoracic duct and/or the lymphatic duct, wherein the method comprises operating a pump to withdraw fluid from the subject via an inlet on the suction lumen between the first restrictor and the second restrictor, the method further comprising returning the fluid to the subject.

9. The method of claim 2 , further comprising monitoring, via one or more pressure sensors on the device, fluid pressure within the body lumen and/or within the region associated with the thoracic duct and/or lymphatic duct.

10. The method of claim 9 , wherein the pressure at the region of the thoracic duct and/or lymphatic duct is reduced to at or below 6 mmHg.

11. A method of treating edema in a subject, the method comprising:

providing a device comprising at least one conduit, at least one restrictor and at least one pump operably associated with the at least one conduit;

positioning the device within a body lumen of a subject in a region near outflow from a thoracic duct and/or a lymphatic duct; and

deploying the at least one restrictor and activating the pump to create a region of reduced pressure at the thoracic duct and/or lymphatic duct while maintaining intravascular pressure, thereby increasing flow through the thoracic duct and/or lymphatic duct and treating edema in the subject.

12. The method of claim 11 , wherein the pump is proximate the restrictor.

13. The method of claim 12 , wherein the pump comprises an impeller.

14. The method of claim 13 , wherein the impeller is distal the restrictor and the conduit tapers from the restrictor toward the impeller.

15. The method of claim 11 , wherein the device comprises a plurality of restrictors.

16. The method of claim 15 , wherein the pump is located external to the subject and operably coupled to the device.

17. The method of claim 16 , wherein deploying the plurality of restrictors creates the region of reduced pressure at the thoracic duct and/or lymphatic duct between the plurality of restrictors.

18. The method of claim 17 , wherein operating the pump draws fluid from a region associated with the thoracic duct and/or lymphatic duct into the conduit through an opening in the conduit between the plurality of restrictors.

19. The method of claim 18 , wherein the fluid is returned to the subject upstream of the plurality of restrictors.

20. The method of claim 11 , further comprising monitoring, via one or more pressure sensors on the device, fluid pressure within the body lumen and/or within the region associated with the thoracic duct and/or lymphatic duct.
