Abstract

Techniques and systems for monitoring cardiac arrhythmias and delivering electrical stimulation therapy using a subcutaneous implantable cardioverter defibrillator (SICD) and a leadless pacing device (LPD) are described. For example, the SICD may detect a tachyarrhythmia within a first electrical signal from a heart and determine, based on the tachyarrhythmia, to deliver anti-tachyarrhythmia shock therapy to the patient to treat the detected arrhythmia. The LPD may receive communication from the SICD requesting the LPD deliver anti-tachycardia pacing to the heart and determine, based on a second electrical signal from the heart sensed by the LPD, whether to deliver anti-tachycardia pacing (ATP) to the heart. In this manner, the SICD and LPD may communicate to coordinate ATP and/or cardioversion/defibrillation therapy. In another example, the LPD may be configured to deliver post-shock pacing after detecting delivery of anti-tachyarrhythmia shock therapy.

Claims (40)

1. A leadless pacing device (LPD) comprising:

a housing;

at least two or more electrodes carried on the housing and configured to sense electrical signals of the heart;

an activity sensor configured to detect a mechanical motion of the heart;

a processor within the housing and configured to analyze the sensed electrical signals and the mechanical motion of the heart to detect tachyarrhythmia; and

a signal generator within the housing and configured to generate and deliver, in response to the detection of the tachyarrhythmia and without receiving a communication from another implanted device, anti-tachycardia pacing therapy (ATP) to the heart of the patient via at least a subset of the two or more electrodes.

2. The LPD of claim 1 , further comprising a shock detector within the housing and configured to detect delivery of anti-tachyarrhythmia shock therapy, wherein the processor activates the shock detector after the detection of the tachyarrhythmia.

The LPD of claim 1 , further comprising a shock detector within the housing and configured to detect delivery of anti-tachyarrhythmia shock therapy, wherein the signal generator is configured to generate and deliver post-shock pacing in response to the delivery of the anti-tachyarrhythmia shock therapy.

4. The LPD of claim 3 , wherein the signal generator ceases the generation and delivery of the ATP in response to the detection of the delivery of anti-tachyarrhythmia shock therapy.

The LPD of claim 1 , further comprising a shock detector within the housing and configured to detect delivery of anti-tachyarrhythmia shock therapy, wherein the processor is configured to analyze the sensed electrical signal from the heart, detects one of bradycardia and asystole subsequent to the detection of the delivery of anti-tachyarrhythmia shock therapy, control the signal generator to generate and deliver post-shock pacing therapy to the heart of the patient in response to the detection of the delivery of anti-tachyarrhythmia shock therapy and the detection of bradycardia or asystole subsequent to the anti-tachyarrhythmia shock therapy.

6. The LPD of claim 1 , further comprising a communication module within the housing and configured to transmit a communication to a subcutaneous implantable cardioverter-defibrillator in response to the detection of the tachyarrhythmia.

7. The LPD of claim 6 , wherein the communication transmitted to the subcutaneous implantable cardioverter-defibrillator is a request for delivery of a shock to the heart.

8. The LPD of claim 1 , wherein the activity sensor comprises an accelerometer.

9. The LPD of claim 8 , wherein the accelerometer is configured to monitor the accelerations corresponding with movement of a chamber of the heart.

10. A method comprising:

sensing electrical signals of a heart using two or more electrodes carried on a housing of a pacing device;

detecting a mechanical motion of the heart using an activity sensor;

analyzing the sensed electrical signals and the mechanical motion of the heart to detect a tachyarrhythmia;

generating, with a signal generator of the pacing device, anti-tachycardia pacing (ATP) therapy in response to the detection of the tachyarrhythmia and without receiving a communication from another implanted device; and

delivering the ATP therapy to the heart of the patient via at least a subset of the two or more electrodes.

11. The method of claim 10 , further comprising:

activating a shock detector after the detection of the tachyarrhythmia; and

detecting delivery of anti-tachyarrhythmia shock therapy using the shock detector.

12. The method of claim 10 , further comprising:

detecting delivery of anti-tachyarrhythmia shock therapy; and

delivering post-shock pacing in response to the delivery of the anti-tachyarrhythmia shock therapy.

13. The method of claim 12 , further comprising ceasing delivery of the ATP therapy in response to the detection of the delivery of anti-tachyarrhythmia shock therapy.

14. The method of claim 10 , further comprising:

detecting delivery of anti-tachyarrhythmia shock therapy;

detecting one of bradycardia and asystole subsequent to the detection of the delivery of anti-tachyarrhythmia shock therapy; and

delivering post-shock pacing therapy to the heart of the patient in response to the detection of the delivery of anti-tachyarrhythmia shock therapy and the detection of bradycardia or asystole subsequent to the anti-tachyarrhythmia shock therapy.

15. The method of claim 10 , further comprising transmitting a communication to a subcutaneous implantable cardioverter-defibrillator in response to the detection of the tachyarrhythmia.

16. The method of claim 15 , wherein transmitting the communication comprises transmitting a request for delivery of a shock to the heart.

17. The method of claim 10 , wherein the activity sensor comprises an accelerometer configured to detect accelerations corresponding with movement of a chamber of the heart.

18. A leadless pacing device (LPD) comprising:

a housing;

a two or more electrodes carried on the housing and configured to sense electrical signals of the heart;

a processor within the housing and configured to detect tachyarrhythmia within the sensed electrical signals of the heart;

a signal generator within the housing and configured to generate and deliver, in response to the detection of the tachyarrhythmia and without receiving a communication from another implanted device, anti-tachycardia pacing therapy (ATP) to the heart of the patient via at least a subset of the two or more electrodes; and

a shock detector within the housing and configured to detect delivery of anti-tachyarrhythmia shock therapy, wherein the processor activates the shock detector in response to the detection of the tachyarrhythmia.
