Abstract

Various aspects of the present subject matter relate to an implantable device. Various device embodiments comprise at least one port to connect to at least one lead with at least electrode, stimulation circuitry connected to the at least one port and adapted to provide at least one neural stimulation therapy to at least one neural stimulation target using the at least one electrode, sensing circuitry connected to the at least one port and adapted to provide a sensed signal, and a controller connected to the stimulation circuitry to provide the at least one neural stimulation therapy and to the sensing circuitry to receive the sensed signal. In response to a triggering event, the controller is adapted to switch between at least two modes. Other aspects and embodiments are provided herein.

Claims (40)

1. An implantable system for implantation in a patient, comprising:

at least one port to connect to at least one lead with at least electrode;

stimulation circuitry connected to the at least one port and adapted to provide at least one neural stimulation therapy to at least one neural stimulation target using the at least one electrode; and

a controller connected to the stimulation circuitry to provide the at least one neural stimulation therapy in at least two neural stimulation site modes of therapy when implanted in the patient and to automatically switch neural stimulation site modes of therapy in response to a triggering event, and the at least two neural stimulation site modes of therapy are provided with different neural stimulation targets for the at least one neural stimulation therapy, wherein the triggering event includes an automatic trigger selected from a group of automatic triggers consisting of:

an automatically detected presence of an unacceptably high level of electrical interference;

an automatically detected electrode failure;

an automatically detected lead failure;

an automatic End Of Life determination for a battery to power the stimulation circuitry; and

an automatically determined application of another therapy.

2. The system of claim 1 , further comprising sensing circuitry configured to provide a sensed signal to the controller, wherein the controller is adapted to switch between at least two sensing modes.

3. The system of claim 1 , further comprising sensing circuitry configured to provide a sensed signal to the controller, wherein the controller is adapted to switch among at least three sensing modes.

The system of claim 1 , wherein the stimulation circuitry includes at least one stimulation circuit connected to the at least one port and adapted to provide the at least one neural stimulation therapy to at least one neural stimulation target and to provide a cardiac rhythm management (CRM) therapy using the at least one electrode;

the controller is connected to the at least one stimulation circuitry to provide the at least one neural stimulation therapy and the CRM therapy, wherein in response to another triggering event the controller is adapted to switch between at least two modes selected from the group consisting of:

a neural stimulation therapy mode in which the neural stimulation therapy is provided;

a CRM therapy mode in which the CRM therapy is provided; and

a neural stimulation and CRM therapy mode in which the neural stimulation therapy is provided and the CRM therapy is provided.

The system of claim 4 , wherein the controller is adapted to switch between at least two modes selected from the group consisting of the neural stimulation therapy mode, the CRM therapy mode, the neural stimulation and CRM therapy mode, and a drug therapy mode in which the controller control a drug delivery.

6. The system of claim 4 , wherein the other triggering event includes an automatic trigger.

7. The system of claim 4 , wherein the other triggering event includes a patient-actuated trigger.

8. The system of claim 4 , further comprising a single device housing to contain the at least one stimulation circuit and at least one sensing circuit.

9. The system of claim 4 , further comprising a first device housing to contain at least one stimulation circuit and a second device housing to contain at least one other stimulation circuit.

The system of claim 1 , wherein the triggering event includes the detected presence of the unacceptably high level of electrical interference, and wherein the controller is adapted to switch to a stimulate only mode in response to the detected presence of the unacceptably high level of electrical interference, wherein neural sensing is disabled.

The system of claim 1 , wherein the triggering event includes the detected presence of the unacceptably high level of electrical interference, and wherein the controller is adapted to switch to a sense only mode in response to the detected presence of the unacceptably high level of electrical interference, wherein the neural stimulation is disabled in the sense only mode.

The system of claim 1 , wherein the controller is further adapted to switch between at least two modes selected from the group consisting of a neural stimulation therapy mode in which neural stimulation is delivered, a CRM therapy mode in which a CRM therapy is delivered, a neural stimulation and CRM therapy mode in which both a neural stimulation therapy and a CRM therapy is delivered, and a drug therapy mode in which a drug therapy is delivered.

13. An implantable system, comprising:

stimulation circuitry configured to use at least one electrode to provide at least one neural stimulation therapy to at least one neural stimulation target using the at least one electrode; and

memory including sets of programmed instructions stored therein to operate the implantable system to provide at least two neural stimulation site modes of therapy and to automatically switch neural stimulation site modes of therapy in response to a triggering event, and the set of neural stimulation site modes have different neural stimulation targets for the at least one neural stimulation therapy; and

a controller connected to the memory and the stimulation circuitry to control the stimulation circuitry, the controller configured to control the providing of the at least one neural stimulation therapy, the controller configured to execute the programmed instructions in the memory to operate the implantable system to provide the at least two neural stimulation site modes of therapy when implanted in a patient, wherein the triggering event includes an automatic trigger selected from a group of automatic triggers consisting of:

an automatically detected presence of an unacceptably high level of electrical interference;

an automatic End Of Life determination for a battery to power the stimulation circuitry; and

an automatically determined application of another therapy.

14. The system of claim 13 , further comprising sensing circuitry configured to provide a sensed signal to the controller, wherein the controller is adapted to switch between at least two sensing modes.

15. The system of claim 13 , further comprising sensing circuitry configured to provide a sensed signal to the controller, wherein the controller is adapted to switch among at least three sensing modes.

The system of claim 13 , wherein the stimulation circuitry includes at least one stimulation circuit connected to the at least one port and adapted to provide the at least one neural stimulation therapy to at least one neural stimulation target and to provide a cardiac rhythm management (CRM) therapy using the at least one electrode;

the controller is connected to the at least one stimulation circuitry to provide the at least one neural stimulation therapy and the CRM therapy, wherein in response to another triggering event the controller is adapted to switch between at least two modes selected from the group consisting of:

a neural stimulation therapy mode in which the neural stimulation therapy is provided;

a CRM therapy mode in which the CRM therapy is provided; and

a neural stimulation and CRM therapy mode in which the neural stimulation therapy is provided and the CRM therapy is provided.

The system of claim 16 , wherein the controller is further adapted to switch between at least two modes selected from the group consisting of the neural stimulation therapy mode, the CRM therapy mode, the neural stimulation and CRM therapy mode, and a drug therapy mode in which the controller control a drug delivery.

The system of claim 13 , wherein the controller is further adapted to switch between at least two modes selected from the group consisting of a neural stimulation therapy mode in which neural stimulation is delivered, a CRM therapy mode in which a CRM therapy is delivered, a neural stimulation and CRM therapy mode in which both a neural stimulation therapy and a CRM therapy is delivered, and a drug therapy mode in which a drug therapy is delivered.
