Guidewire with ablation and coagulation functionality
Apparatus, including a guidewire, having a distal end dimensioned to penetrate into a nasal sinus and a balloon, which is fitted over the guidewire in proximity to the distal end. There is an inflation channel, which runs along the guidewire and is coupled to convey a pressurized fluid into the balloon so as to inflate the balloon. The apparatus also includes a first electrode, fixedly attached to a distal tip of the guidewire, and a second electrode, fixedly attached to the guidewire at a location proximal to the distal tip. There are conductive leads running along the guidewire and coupled to apply an electrical potential between the first and second electrodes.
1. A method, comprising:
(a) providing a guidewire, having a distal end dimensioned to penetrate into a nasal sinus;
(b) slidably fitting a balloon directly over the guidewire in proximity to the distal end such that the balloon is in contact with the guidewire;
(c) running an inflation channel, that is coupled to convey a pressurized fluid into the balloon so as to inflate the balloon, along the guidewire;
(d) fixedly attaching a first electrode to a distal tip of the guidewire;
(e) fixedly attaching a second electrode to the guidewire at a location proximal to the distal tip, wherein the first and second electrodes are distal and separate from the balloon;
(f) running conductive leads, that are coupled to apply an electrical potential between the first and second electrodes, along the guidewire;
(g) inserting the guidewire into the nasal cavity to a desired position and orientation;
(h) sliding the balloon distally along the guidewire after the act of inserting;
(i) inflating the balloon to perform a sinuplasty procedure after the act of sliding;
(j) generating the electrical potential; and
(k) applying the electrical potential to the first and second electrodes so as to treat material proximate to the nasal sinus.
2. The method according to claim 1 , wherein the act of generating includes generating the electrical potential as a low frequency AC (alternating current) potential, wherein the act of applying includes applying the electrical potential to the first and second electrodes so as to coagulate the material proximate to the nasal sinus.
3. The method according to claim 2 , wherein the low frequency AC potential has a power of up to 5 W and a frequency of up to 30 kHz.

The method according to claim 1 , wherein the act of generating includes generating the electrical potential as a high frequency AC (alternating current) potential, wherein the act of applying includes applying the electrical potential to the first and second electrodes so as to ablate the material proximate to the nasal sinus.
5. The method according to claim 4 , wherein the high frequency AC potential has a power of up to 20 W and a frequency greater than 30 kHz.
6. The method according to claim 4 , wherein the act of applying the electrical potential to the first and second electrodes so as to ablate the material proximate to the nasal sinus obtains access to a region of the nasal sinus.
7. The method according to claim 6 , wherein the act of inflating is performed within the accessed region of the nasal sinus after the act of applying.
8. The method according to claim 4 , wherein the act of applying includes generating a mixture of blood, mucus, and/or particulate matter, the method further comprising applying an additional electrical potential to the first and second electrodes so as to coagulate the mixture of blood, mucus, and/or particulate matter.
9. The method according to claim 1 , further comprising positioning an insulating section to separate the distal end into a distal section comprising the distal tip, and a proximal section comprising the location proximal to the distal tip.
10. A method, comprising:
(a) providing a guidewire, having a distal end and a proximal conductive section proximal to the distal end and insulated therefrom, the distal end and the conductive section being dimensioned to penetrate into a nasal sinus;
(b) slidably fitting a balloon directly over the guidewire in proximity to the distal end such that the balloon is in contact with the guidewire;
(c) running an inflation channel, coupled to convey a pressurized fluid into the balloon so as to inflate the balloon, along the guidewire;
(d) fixedly attaching an electrode to a distal tip of the guidewire, wherein the electrode and the proximal conductive section are distal and separate from the balloon;
(e) running conductive leads, coupled to apply an electrical potential between the electrode and the proximal conductive section of the guidewire, along the guidewire;
(f) inserting the guidewire into a nasal cavity to a desired position and orientation;
(g) sliding the balloon distally along the guidewire after the act of inserting;
(h) inflating the balloon to perform a sinuplasty procedure after the act of sliding;
(i) generating the electrical potential; and
(j) applying the electrical potential to the electrode and the proximal conductive section so as to treat material proximate to the nasal sinus.
11. The method according to claim 10 , wherein the act of generating includes generating the electrical potential as a low frequency AC (alternating current) potential, wherein the act of applying includes applying the electrical potential to the electrode and the proximal conductive section so as to coagulate the material proximate to the nasal sinus.
12. The method according to claim 11 , wherein the low frequency AC potential has a power of up to 5 W and a frequency of up to 30 kHz.
13. The method according to claim 10 , wherein the act of generating includes generating the electrical potential as a high frequency AC (alternating current) potential, wherein the act of applying includes applying the electrical potential to the electrode and the proximal conductive section so as to ablate the material proximate to the nasal sinus.
14. The method according to claim 13 , wherein the high frequency AC potential has a power of up to 20 W and a frequency greater than 30 kHz.
15. The method according to claim 13 , wherein the act of applying the electrical potential to the electrode and the proximal conductive section so as to ablate the material proximate to the nasal sinus obtains access to a region of the nasal sinus.
16. The method according to claim 15 , wherein the act of inflating is performed within the accessed region of the nasal sinus after the act of applying.
17. The method according to claim 13 , wherein the act of applying includes generating a mixture of blood, mucus, and/or particulate matter, the method further comprising applying an additional electrical potential to the electrode and the proximal conductive section so as to coagulate the mixture of blood, mucus, and/or particulate matter.
18. A method of using an apparatus comprising (i) a guidewire having a distal end dimensioned to penetrate into a nasal sinus, (ii) a balloon slidably fitted directly over the guidewire in proximity to the distal end such that the balloon is in contact with the guidewire, (iii) an inflation channel running along the guidewire and configured to convey a pressurized fluid into the balloon so as to inflate the balloon, (iv) a first electrode fixedly attached to a distal tip of the guidewire, (v) a second electrode fixedly attached to the guidewire at a location proximal to the distal tip, wherein the first and second electrodes are distal and separate from the balloon, and (vi) conductive leads running along the guidewire and configured to apply an electrical potential between the first and second electrodes, the method comprising:
(a) inserting the guidewire into a nasal cavity to a desired position and orientation;
(b) sliding the balloon distally along the guidewire after the act of inserting;
(c) inflating the balloon to perform a sinuplasty procedure after the act of sliding;
(d) generating the electrical potential; and
(e) applying the electrical potential to the first and second electrodes so as to treat material proximate to the nasal sinus.
19. The method according to claim 18 , wherein the act of generating includes generating the electrical potential as a high frequency AC (alternating current) potential, wherein the act of applying includes applying the electrical potential to the first and second electrodes so as to ablate the material proximate to the nasal sinus.
20. The method according to claim 19 , wherein the act of inflating is performed after the act of applying.
21. A method of using an apparatus comprising (i) a guidewire having a distal end and a proximal conductive section proximal to the distal end and insulated therefrom, the distal end and the conductive section being dimensioned to penetrate into a nasal sinus, (ii) a balloon slidably fitted directly over the guidewire in proximity to the distal end such that the balloon is in contact with the guidewire, (iii) an inflation channel running along the guidewire and configured to convey a pressurized fluid into the balloon so as to inflate the balloon, (iv) an electrode fixedly attached to a distal tip of the guidewire, wherein the electrode and the proximal conductive section are distal and separate from the balloon, (v) conductive leads running along the guidewire and configured to apply an electrical potential between the electrode and the proximal conductive section of the guidewire, the method comprising:
(a) inserting the guidewire into a nasal cavity to a desired position and orientation;
(b) sliding the balloon distally along the guidewire after the act of inserting the guidewire into the nasal cavity to a desired position and orientation;
(c) inflating the balloon to perform a sinuplasty procedure after the act of sliding the balloon distally along the guidewire;
(d) generating the electrical potential; and
(e) applying the electrical potential to the electrode and the proximal conductive section so as to treat material proximate to the nasal sinus.
22. The method according to claim 21 , wherein the act of generating includes generating the electrical potential as a high frequency AC (alternating current) potential, wherein the act of applying includes applying the electrical potential to the first and second electrodes so as to ablate the material proximate to the nasal sinus, wherein the act of inflating is performed after the act of applying.
