Abstract

A multi-pole synchronous pulmonary artery radiofrequency ablation catheter may comprise a control handle, a catheter body and an annular ring. One end of the catheter body may be flexible, and the flexible end of the catheter body may be connected to the annular ring. The other end of the catheter body may be connected to the control handle. A shape memory wire may be arranged in the annular ring. One end of the shape memory wire may extend to an end of the annular ring and the other end of the shape memory wire may pass through a root of the annular ring and be fixed on the flexible end of the catheter body. The annular ring may be provided with an electrode group. The device possesses advantages of simple operation, short operation time and controllable precise ablation. The device can be used to treat pulmonary hypertension with pulmonary denervation.

Claims (35)

1. A method of performing pulmonary artery denervation comprising:

positioning an ablation device within or in proximity to a pulmonary artery trunk of a patient, the pulmonary artery trunk including a distal portion of a main pulmonary artery, a proximal portion of a left pulmonary artery, and a proximal portion of a right pulmonary artery; and

ablating target tissue in the pulmonary artery trunk under conditions effective to affect a reduction in mean pulmonary artery pressure of the patient,

wherein the target tissue is in a portion of the pulmonary artery trunk proximal to a left side lateral wall at the distal portion of the main pulmonary artery and proximal to a lower wall of the proximal portion of the left pulmonary artery, and

wherein the conditions effective to affect the reduction in mean pulmonary artery pressure also affect a reduction in systolic pulmonary artery pressure.

2. The method of claim 1 , wherein the reduction in mean pulmonary artery pressure is about 15%.

3. The method of claim 1 , wherein the reduction in mean pulmonary artery pressure is about 25%.

4. The method of claim 1 , wherein the reduction in systolic pulmonary artery pressure is about 15%.

5. The method of claim 1 , wherein the reduction in systolic pulmonary artery pressure is about 20%.

6. The method of claim 1 , wherein the conditions effective to affect the reduction in mean pulmonary artery pressure also affect an in increase in 6-minute walk distance (6MWD).

7. The method of claim 6 , wherein the increase reduction 6MWD is about 25%.

8. The method of claim 1 , wherein ablating the target tissue comprises ablating the target tissue at a temperature of about 50° degrees C. to about 60° degrees C.

9. The method of claim 1 , wherein ablating the target tissue comprises ablating the target tissue with an energy of between about 3 W to about 15 W.

10. The method of claim 1 , wherein ablating the target tissue comprises ablating the target tissue for between about 60 and about 120 seconds.

11. The method of claim 1 , wherein ablating the target tissue comprises applying at least one of radiofrequency (RF) energy, microwave energy, ultrasound energy, focused ultrasound energy, ionizing energy, electroporation, a drug, a chemical agent, or cryoablation to the target tissue.

12. The method of claim 1 , wherein ablating the target tissue causes disruption of sympathetic nerves at the pulmonary artery trunk.

13. The method of claim 1 , wherein ablating the target tissue comprises generating a continuous ablation pattern.

14. The method of claim 1 , wherein ablating the target tissue comprises generating a non-continuous ablation pattern.

15. The method of claim 1 , wherein positioning the ablation device comprises positioning a distal elliptical ring adjacent the target tissue.

16. The method of claim 15 , wherein positioning the distal elliptical ring comprises one or more of rotating or pushing a handle of the ablation device.

17. The method of claim 15 , wherein ablating the target tissue comprises ablating the target tissue with the distal elliptical ring positioned adjacent the target tissue, the distal elliptical ring comprising one or more ablation electrodes.

18. The method of claim 17 , wherein the distal elliptical ring comprises first, second, and third ablation electrodes.

The method of claim 18 , wherein ablating the target tissue comprises ablating one or more of a first ablation site at a left lateral apex of the distal portion of the main pulmonary artery with the first ablation electrode, a second ablation site at an anterior side of the left lateral apex of the distal portion of the main pulmonary artery, or third ablation site at a posterior side of the left lateral apex of the distal portion of the main pulmonary artery with the third electrode.

20. The method of claim 19 , wherein ablating the target tissue comprises ablating two or more of the first, second, or third ablation sites.

21. The method of claim 20 , wherein ablating the target tissue comprises ablating the first, second, and third ablation sites.

22. The method of claim 17 , wherein the one or more ablation electrodes are positioned to straddle an apex of the major axis of the distal elliptical ring.

The method of claim 1 , wherein positioning the ablation device within or in proximity to the pulmonary artery trunk of the patient comprises passing the ablation device into a femoral vein of the patient, upwardly into an inferior vena cava of the patient, then upwards into a right atrium of the patient, then down into a right ventricle of the patient, and then up through a pulmonary semilunar valve of the patient to the main pulmonary artery trunk.

24. The method of claim 1 , further comprising visualizing at least one of the main pulmonary artery trunk or the ablation device positioned in the main pulmonary artery trunk to confirm to positioning of the ablation device.

25. The method of claim 1 , further comprising visualizing the main pulmonary artery prior to positioning the ablation device in the pulmonary artery trunk.

26. The method of claim 1 , wherein the main pulmonary artery is visualized with one or more of magnetic resonance imaging (MRI) or computed tomography (CT) scanning.

27. The method of claim 1 , further comprising withdrawing the ablation device from the main pulmonary artery after the target tissue has been ablated.

28. The method of claim 27 , further comprising visualizing the main pulmonary artery after the ablation device has been withdrawn.

29. The method of claim 1 , wherein ablating the target tissue comprises ablating a first ablation site at a left lateral apex of the distal portion of the main pulmonary artery.

30. The method of claim 29 , wherein ablating the target tissue comprises ablating one or more of a second ablation site at an anterior side of the left lateral apex of the distal portion of the main pulmonary artery or a third ablation site at a posterior side of the left lateral apex of the distal portion of the main pulmonary artery.

31. The method of claim 30 , wherein ablating the target tissue comprises ablating each of the first, second, and third ablation sites.
