Abstract

A method and system for use with an implantable medical device for subcutaneous implant within a patient to determine a likelihood of the patient experiencing a cardiac event that includes sensing a cardiac signal along a plurality of different sensing vectors, determining state information of each vector of the plurality of sensing vectors, determining a cross correlation of the determined state information of each vector of the plurality of sensing vectors, comparing the cross correlation of the determined state information of each vector of the plurality of sensing vectors to a threshold, and detecting the cardiac event in response to the comparing.

Claims (70)

1. A medical device comprising:

a sense amplifier configured to sense, during each time period of a plurality of time periods, a respective cardiac signal segment with a sensing vector;

processing circuitry configured to:

for each of the cardiac signal segments, classify the cardiac signal segment as shockable or not shockable based on a morphology of the cardiac signal segment;

determine a number of the cardiac signal segments classified as shockable; and

determine to deliver an anti-tachyarrhythmia shock based at least on the number of the cardiac signal segments classified as shockable; and

therapy delivery circuitry configured to deliver the anti-tachyarrhythmia shock in response to the determination to deliver the anti-tachyarrhythmia shock.

2. The medical device of claim 1 ,

wherein to sense, during each time period of the plurality of time periods, the respective cardiac signal segment with the sensing vector, the sense amplifier is configured to sense, during each time period of the plurality of time periods, a first cardiac signal segment with a first sensing vector and a second cardiac signal segment with a second sensing vector,

wherein to classify the cardiac signal segment as shockable or not shockable based on the morphology of the cardiac signal segment, the processing circuitry is configured to:

for each first cardiac signal segment, classify the first cardiac signal segment as shockable or not shockable based on a morphology of the first cardiac signal segment;

for each second cardiac signal segment, classify the second cardiac signal segment as shockable or not shockable based on a morphology of the second cardiac signal segment,

wherein to determine the number of the cardiac signal segments classified as shockable, the processing circuitry is configured to determine a number of the first cardiac signal segments classified as shockable and a number of the second cardiac signal segments classified as shockable, and

wherein to determine to deliver the anti-tachyarrhythmia shock based at least on the number of the cardiac signal segments classified as shockable, the processing circuitry is configured to determine to deliver the anti-tachyarrhythmia shock based at least on the number of the first cardiac signal segments classified as shockable and the number of the second cardiac signal segments classified as shockable.

The medical device of claim 2 , wherein to classify the first cardiac signal segment as shockable or not shockable based on the morphology of the first cardiac signal segment and classify the second cardiac signal segment as shockable or not shockable based on the morphology of the second cardiac signal segment, the processing circuitry is configured to perform one of:

classifying both the first cardiac signal segment and the second cardiac signal segment as shockable based on the respective morphologies of the first cardiac signal segment and the second cardiac signal segment;

classifying both the first cardiac signal segment and the second cardiac signal segment as not shockable based on the respective morphologies of the first cardiac signal segment and the second cardiac signal segment; or

classifying only one of the first cardiac signal segment and the second cardiac signal segment as shockable and the other of the first cardiac signal segment and the second cardiac signal segment as not shockable based on the respective morphologies of the first cardiac signal segment and the second cardiac signal segment.

4. The medical device of claim 2 ,

wherein the processing circuitry is further configured to classify both the first cardiac signal segment and the second cardiac signal segment as corrupted by noise based on the respective morphologies of the first cardiac signal segment and the second cardiac signal segment, and

wherein to classify the first cardiac signal segment as shockable or not shockable based on the morphology of the first cardiac signal segment and classifying the second cardiac signal segment as shockable or not shockable based on the morphology of the second cardiac signal segment, the processing circuitry is further configured to classify both the first cardiac signal segment and the second cardiac signal segment as not shockable based on both the first cardiac signal segment and the second cardiac signal segment being corrupted by noise.

5. The medical device of claim 1 , wherein each time period of the plurality of time periods has a duration of about 3 seconds.

6. The medical device of claim 1 , wherein each time period of the plurality of time periods is a duration of an R-R interval of a patient.

7. The medical device of claim 1 , wherein the medical device further comprises one or more subcutaneous electrodes disposed on a subcutaneous lead, the lead operatively coupled to the sense amplifier.

8. The medical device of claim 1 , wherein the medical device further comprises one or more electrodes disposed on a lead positioned within a thoracic cavity of a patient, the lead operatively coupled to the sense amplifier.

9. A medical device system comprising:

a medical device comprising a sense amplifier configured to sense, during each time period of a plurality of time periods, a respective cardiac signal segment with a sensing vector; and

processing circuitry configured to:

for each of the cardiac signal segments, classify the cardiac signal segment as shockable or not shockable based on a morphology of the cardiac signal segment;

determine a number of the cardiac signal segments classified as shockable; and

determine to deliver an anti-tachyarrhythmia shock based at least on the number of the cardiac signal segments classified as shockable,

wherein the medical device further comprises therapy delivery circuitry configured to deliver the anti-tachyarrhythmia shock in response to the determination to deliver the anti-tachyarrhythmia shock.

10. The medical device system of claim 8 ,

wherein to sense, during each time period of the plurality of time periods, the respective cardiac signal segment with the sensing vector, the sense amplifier is configured to sense, during each time period of the plurality of time periods, a first cardiac signal segment with a first sensing vector and a second cardiac signal segment with a second sensing vector,

wherein to classify the cardiac signal segment as shockable or not shockable based on the morphology of the cardiac signal segment, the processing circuitry is configured to:

for each first cardiac signal segment, classify the first cardiac signal segment as shockable or not shockable based on a morphology of the first cardiac signal segment;

for each second cardiac signal segment, classify the second cardiac signal segment as shockable or not shockable based on a morphology of the second cardiac signal segment,

wherein to determine the number of the cardiac signal segments classified as shockable, the processing circuitry is configured to determine a number of the first cardiac signal segments classified as shockable and a number of the second cardiac signal segments classified as shockable, and

wherein to determine to deliver the anti-tachyarrhythmia shock based at least on the number of the cardiac signal segments classified as shockable, the processing circuitry is configured to determine to deliver the anti-tachyarrhythmia shock based at least on the number of the first cardiac signal segments classified as shockable and the number of the second cardiac signal segments classified as shockable.

11. A method comprising:

sensing, during each time period of a plurality of time periods, a respective cardiac signal segment with a sensing vector;

for each of the cardiac signal segments, classifying the cardiac signal segment as shockable or not shockable based on a morphology of the cardiac signal segment;

determining a number of the cardiac signal segments classified as shockable;

determining to deliver an anti-tachyarrhythmia shock based at least on the number of the cardiac signal segments classified as shockable; and

delivering the anti-tachyarrhythmia shock in response to the determination to deliver the anti-tachyarrhythmia shock.

12. The method of claim 11 ,

wherein sensing, during each time period of the plurality of time periods, the respective cardiac signal segment with the sensing vector comprises sensing, during each time period of the plurality of time periods, a first cardiac signal segment with a first sensing vector and a second cardiac signal segment with a second sensing vector,

wherein classifying the cardiac signal segment as shockable or not shockable based on the morphology of the cardiac signal segment comprises:

for each first cardiac signal segment, classifying the first cardiac signal segment as shockable or not shockable based on a morphology of the first cardiac signal segment;

for each second cardiac signal segment, classifying the second cardiac signal segment as shockable or not shockable based on a morphology of the second cardiac signal segment,

wherein determining the number of the cardiac signal segments classified as shockable comprises determining a number of the first cardiac signal segments classified as shockable and a number of the second cardiac signal segments classified as shockable, and

wherein determining to deliver the anti-tachyarrhythmia shock based at least on the number of the cardiac signal segments classified as shockable comprises determining to deliver the anti-tachyarrhythmia shock based at least on the number of the first cardiac signal segments classified as shockable and the number of the second cardiac signal segments classified as shockable.

The method of claim 12 , wherein classifying the first cardiac signal segment as shockable or not shockable based on the morphology of the first cardiac signal segment and classifying the second cardiac signal segment as shockable or not shockable based on the morphology of the second cardiac signal segment comprises one of:

classifying both the first cardiac signal segment and the second cardiac signal segment as shockable based on the respective morphologies of the first cardiac signal segment and the second cardiac signal segment;

classifying both the first cardiac signal segment and the second cardiac signal segment as not shockable based on the respective morphologies of the first cardiac signal segment and the second cardiac signal segment; or

classifying only one of the first cardiac signal segment and the second cardiac signal segment as shockable and the other of the first cardiac signal segment and the second cardiac signal segment as not shockable based on the respective morphologies of the first cardiac signal segment and the second cardiac signal segment.

14. The method of claim 12 ,

wherein the method further comprises classifying both the first cardiac signal segment and the second cardiac signal segment as corrupted by noise based on the respective morphologies of the first cardiac signal segment and the second cardiac signal segment, and

wherein classifying the first cardiac signal segment as shockable or not shockable based on the morphology of the first cardiac signal segment and classifying the second cardiac signal segment as shockable or not shockable based on the morphology of the second cardiac signal segment comprises classifying both the first cardiac signal segment and the second cardiac signal segment as not shockable based on both the first cardiac signal segment and the second cardiac signal segment being corrupted by noise.

15. The method of claim 11 , wherein each time period of the plurality of time periods has a duration of about 3 seconds.

16. The method of claim 11 , wherein each time period of the plurality of time periods is a duration of an R-R interval of a patient.

17. The method of claim 11 , wherein sensing the respective cardiac signal segment with a sensing vector comprises sensing, by a sense amplifier and one or more subcutaneous electrodes disposed on a subcutaneous lead, the respective cardiac signal segment with the sensing vector.

The method of claim 11 , wherein sensing the respective cardiac signal segment with a sensing vector comprises sensing, by a sense amplifier and one or more electrodes disposed on a lead positioned within a thoracic cavity of a patient, the respective cardiac signal segment with the sensing vector.

19. The method of claim 11 ,

wherein sensing the respective cardiac signal segment with a sensing vector comprises sensing, by a sense amplifier of an implantable medical device and one or more electrodes disposed on a lead of the implantable medical device, the respective cardiac signal segment with the sensing vector,

wherein classifying the cardiac signal segment as shockable or not shockable based on the morphology of the cardiac signal segment comprises classifying, by processing circuitry of the implantable medical device, the cardiac signal segment as shockable or not shockable based on the morphology of the cardiac signal segment,

wherein determining the number of the cardiac signal segments classified as shockable comprises determining, by the processing circuitry of the implantable medical device, the number of the cardiac signal segments classified as shockable,

wherein determining to deliver the anti-tachyarrhythmia shock based at least on the number of the cardiac signal segments classified as shockable comprises determining, by the processing circuitry of the implantable medical device, to deliver the anti-tachyarrhythmia shock based at least on the number of the cardiac signal segments classified as shockable, and

wherein delivering the anti-tachyarrhythmia shock in response to the determination to deliver the anti-tachyarrhythmia shock comprises delivering, by therapy delivery circuitry of the implantable medical device, the anti-tachyarrhythmia shock in response to the determination to deliver the anti-tachyarrhythmia shock.

20. The method of claim 11 , further comprising delivering one or more pacing pulses after delivering the anti-tachyarrhythmia shock.
