Electroporation for obesity or diabetes treatment
Endolumenal devices and methods can be used for the treatment of health conditions including obesity and diabetes. In some embodiments, the methods and systems provided herein can cause weight loss or control diabetes by reducing the caloric absorption of an individual. For example, this document provides several devices and methods for treating obesity and diabetes by using electroporation to modulate the duodenal mucosa. In addition, this document provides devices and methods for bypassing portions of the gastrointestinal tract to reduce nutritional uptake.
1. A method of administering electroporation energy to a patient, the method comprising:
deploying an electroporation device within a duodenum of the patient, wherein the electroporation device comprises:
a shaft defining a lumen therethrough, wherein the lumen is configured to receive an endoscope therein;
rolled-up member defining a plurality of apertures, the rolled-up member circumferentially attached about a distal portion of the shaft; and
one or more electrodes disposed on an outer surface of the rolled-up member;
energizing the one or more electrodes with electroporation energy; and
delivering the electroporation energy to the duodenum to treat diabetes while energizing the one or more electrodes.
2. The method of claim 1 , wherein an endoscope is slidably disposed within the lumen during said deploying the electroporation device.
3. The method of claim 1 , wherein deploying the electroporation device within the duodenum comprises stretching at least a portion of the duodenum.
4. The method of claim 1 , wherein the one or more electrodes comprise bipolar electrodes.
5. The method of claim 1 , further comprising measuring one or more characteristics of the delivered electroporation energy using the one or more electrodes.
6. A method of administering electroporation energy to a patient, the method comprising:
deploying an electroporation device within a duodenum of the patient, wherein the electroporation device comprises:
a shaft defining a lumen therethrough;
rolled-up member defining a plurality of apertures, the rolled-up member circumferentially attached about a distal portion of the shaft; and
one or more electrodes disposed on an outer surface of the rolled-up member;
energizing the one or more electrodes with electroporation energy; and
delivering the electroporation energy to the duodenum to treat diabetes while energizing the one or more electrodes.
7. The method of claim 6 , wherein at least a portion of the rolled-up member is spaced apart from an inner wall of the duodenum, and wherein a fluid conducts at least some of the electroporation energy from the portion of the rolled-up member that is spaced apart from the inner wall of the duodenum to the inner wall of the duodenum.
8. The method of claim 6 , further comprising delivering a drug to a mucosa of the duodenum.
9. The method of claim 6 , further comprising transitioning the electroporation device from a first configuration to a second configuration.
10. The method of claim 9 , wherein the first configuration is a contracted configuration and the second configuration is an expanded configuration.
11. The method of claim 9 , wherein transitioning the electroporation device to the second configuration stretches at least a portion of the duodenum.
12. The method of claim 6 , wherein the electroporation energy is delivered to crypts of a duodenal mucosa of the duodenum.
13. The method of claim 6 , wherein the electroporation energy comprises pulse currents.
14. The method of claim 6 , wherein the one or more electrodes comprise a plurality of spaced-apart electrodes.
15. The method of claim 6 , further comprising delivering a fluid into an interior space of the duodenum through the one or more apertures.
16. The method of claim 15 , wherein the fluid is circulated in the interior space of the duodenum while delivering the electroporation energy.
17. The method of claim 15 , wherein the fluid comprises a hydrogel.
18. The method of claim 15 , wherein the fluid comprises an electrically conductive liquid.
19. The method of claim 15 , wherein the fluid comprises a drug.
20. The method of claim 19 , wherein delivering the fluid comprises pheresing the drug to cells within a duodenal mucosa of the duodenum.
21. The method of claim 6 , wherein deploying the electroporation device within the duodenum comprises stretching at least a portion of the duodenum.
22. The method of claim 6 , wherein the one or more electrodes comprise bipolar electrodes.
23. The method of claim 6 , further comprising measuring one or more characteristics of the delivered electroporation energy using the one or more electrodes.
24. The method of claim 6 , wherein the lumen is configured to receive a guidewire therein.
