2. The device of claim 1 , wherein the flat and flexible main body comprises a pair of equally shaped polymer sheets attached together along at least a majority of their peripheries.

3. The device of claim 2 , wherein the first end of the first tapered section and the second end of the second are sealed.

4. The device of claim 2 , wherein the first end of the first tapered section defines a first opening and the second opening of the second tapered section defines a second opening equally sized with the first opening.

5. The device of claim 1 , further comprising cut lines formed on the main body and positioned proximate to the first end of the first tapered section and the second end of the second tapered section so as to defined predetermined opening sizes when the first and second tapered sections define said multiple implant guide conduits.

6. The device of claim 1 , wherein the rectangular section comprises a first lateral edge and a second lateral edge opposite from the first lateral edge, the first and second lateral edges extending parallel to the central longitudinal axis of the rectangular section,

wherein the first tapered section comprises a first tapered edge extending from the first lateral edge toward the first end at a first taper angle, and a first straight edge collinear with the second lateral edge and extending toward the first end, and

wherein the second tapered section comprises a second tapered edge extending from the second lateral edge toward the second end at a second taper angle equal to the first taper angle, and a second straight edge collinear with the first lateral edge and extending toward the second end.

7. The device of claim 6 , wherein the first taper angle and the second taper angle are between 110-degrees and 170-degrees.

8. The device of claim 1 , wherein the flat and flexible main body is expandable to provide an interior cavity between a first side and a second side.

9. The device of claim 8 , wherein the flat and flexible main body is entirely opaque.

10. The device of claim 8 , wherein the flat and flexible main body comprises a transparent window positioned between opaque wall regions such that the interior cavity of the main body is viewable through the transparent window.

11. The device of claim 10 , wherein the transparent window comprising a transparent strip extending entirely from the first end to the second end.

12. The device of claim 8 , further comprising an antimicrobial material positioned in the interior cavity of the main body.

13. The device of claim 1 , wherein the flat and flexible main body is non-symmetric about a central lateral axis of the rectangular section positioned midway between the first tapered section and the second tapered section and perpendicular to the central longitudinal axis.

14. The device of claim 1 , wherein the first end of the first tapered section is laterally spaced apart from the central longitudinal axis on a first side lateral side of the central longitudinal axis, and the second end of the second tapered section is laterally spaced apart from the central longitudinal axis on a second side lateral side opposite from the first lateral side.

15. The device of claim 1 , further comprising a first intermediate cut line formed on the main body and positioned at a first end of the rectangular section adjacent the first tapered section for separating the first tapered section from the rectangular section, a second intermediate cut line formed on the main body and positioned at a second end of the rectangular section adjacent the second tapered section for separating the second tapered section from the rectangular section, and a third intermediate cut line formed on the main body and positioned along a central lateral axis of the rectangular section midway between the first tapered section and the second tapered section.

16. A surgical delivery device customizable to provide multiple implant guide conduits for delivery of implants, comprising:

a flat and flexible main body having a generally trapezoidal shape defined by a rectangular section between first and second tapered sections, the first tapered section terminating in a first end and the second tapered section terminating in a second end equally sized with the first end, wherein the flat and flexible main body is non-symmetric about a central longitudinal axis of the rectangular section extending in a longitudinal direction from the first tapered section to the second tapered section, and wherein one or more of the rectangular section, the first tapered section, and the second tapered section the flat and flexible main body are separable from another to define multiple implant guide conduits; and

multiple pairs of handles attached to opposing exterior sides of the flat and flexible main body to provide grasping locations when expanding at least one of said sections of the main body into one of said implant guide conduits,

wherein the flat and flexible main body comprises a pair of planar polymer sheets attached along at least a portion of a periphery of the generally trapezoidal shape,

wherein the first end of the first tapered section defines a first opening and the second opening of the second tapered section defines a second opening equally sized with the first opening, and

wherein the first opening of the first tapered section is laterally spaced apart from the central longitudinal axis on a first side lateral side of the longitudinal axis, and the second opening of the second tapered section is laterally spaced apart from the central longitudinal axis on a second side lateral side opposite from the first lateral side.

17. The device of claim 16 , wherein the flat and flexible main body is expandable to provide an interior cavity between a first side and a second side.

18. The device of claim 17 , wherein the flat and flexible main body comprises a transparent window positioned between opaque wall regions such that the interior cavity of the main body is viewable through the transparent window.

19. A surgical delivery device customizable to provide multiple implant guide conduits for delivery of implants, comprising:
