Abstract

Devices and methods for the treatment of chronic total occlusions are provided. One disclosed embodiment comprises a method of facilitating treatment via a vascular wall defining a vascular lumen containing an occlusion therein. The method includes inserting an intramural crossing device into the vascular lumen, positioning at least the distal tip of the crossing device in the vascular wall, advancing an orienting device over the crossing device such that an orienting element of the orienting device resides in the vascular wall, inserting a reentry device, and re-entering the true vascular lumen.

Claims (30)

1. A system for bypassing an occlusion in a blood vessel, comprising:

an intramural orienting device comprising:

an elongate shaft including a lumen extending therein to a distally facing opening at a distal end of the elongate shaft; and

an orienting element including two wings configured to shift between a collapsed delivery configuration and an expanded deployed configuration, the two wings being monolithically formed with a portion of the elongate shaft;

wherein in the expanded deployed configuration, the two wings extend outwardly from the elongate shaft in a substantially planar manner;

wherein a distal portion of the elongate shaft includes a side opening in communication with the lumen, the side opening facing orthogonally relative to the two wings.

2. The system of claim 1 , wherein in the expanded deployed configuration, the two wings extend in substantially opposite directions from the elongate shaft.

3. The system of claim 1 , wherein in the expanded deployed configuration, the two wings are oriented parallel to a central longitudinal axis of the elongate shaft.

4. The system of claim 1 , wherein in the expanded deployed configuration, a plane extending through an outermost portion of each of the two wings includes a central longitudinal axis of the elongate shaft disposed within the plane.

5. The system of claim 1 , wherein the side opening is disposed between the two wings.

6. The system of claim 1 , wherein the lumen extends from proximal of the two wings to distal of the two wings.

7. The system of claim 6 , wherein the two wings are disposed completely external to the lumen of the elongate shaft.

8. The system of claim 1 , further comprising a re-entry device having a distal tip configured to penetrate a wall of the blood vessel and enter into a true lumen of the blood vessel, the re-entry device being sized and configured to be inserted through the lumen of the elongate shaft.

9. The system of claim 8 , wherein the re-entry device includes a shaft and a distal coil disposed over a portion of the shaft adjacent the distal tip.

10. The system of claim 8 , wherein the side port is configured to direct the re-entry device away from the elongate shaft in a direction perpendicular to the two wings.

11. A system for bypassing an occlusion in a blood vessel, comprising:

an intramural orienting device comprising:

an elongate shaft including a lumen extending therein along a central longitudinal axis of the elongate shaft to a distally facing opening at a distal end of the elongate shaft; and

an orienting element including a first wing and a second wing configured to shift between a collapsed delivery configuration and an expanded deployed configuration, the first wing and the second wing being monolithically formed with a portion of the elongate shaft;

wherein in the expanded deployed configuration, the first wing and the second wing extend laterally from the elongate shaft in opposite directions from the central longitudinal axis;

wherein a distal portion of the elongate shaft includes a side opening in communication with the lumen, the side opening facing orthogonally relative to the first wing and the second wing.

12. The system of claim 11 , wherein in the expanded deployed configuration, the first wing and the second wing collectively define a substantially planar structure.

13. The system of claim 11 , wherein in the expanded deployed configuration, the first wing and the second wing are oriented parallel to a central longitudinal axis of the elongate shaft.

14. The system of claim 11 , wherein in the expanded deployed configuration, the central longitudinal axis is disposed within a plane extending through a radially outermost point of the first wing relative to the elongate shaft and a radially outermost point of the second wing relative to the elongate shaft.

15. The system of claim 11 , wherein the side opening is disposed between the first wing and the second wing.

16. The system of claim 11 , wherein the lumen extends from proximal of the first wing and the second wing to distal of the first wing and the second wing.

17. The system of claim 16 , wherein the first wing and the second wing are disposed completely external to the lumen of the elongate shaft.

18. The system of claim 11 , further comprising a re-entry device having a distal tip configured to penetrate a wall of the blood vessel and enter into a true lumen of the blood vessel, the re-entry device being sized and configured to be inserted through the lumen of the elongate shaft.

19. The system of claim 18 , wherein the re-entry device includes a shaft and a distal coil disposed over a portion of the shaft adjacent the distal tip.

20. The system of claim 18 , wherein the side port is configured to direct the re-entry device away from the elongate shaft in a direction perpendicular to the opposite directions the first wing and the second wing extend from the central longitudinal axis.
