Abstract

Various fixation techniques for implantable medical device (IMDs) are described. In one example, an assembly comprises an IMD; and a set of active fixation tines attached to the IMD. The active fixation tines in the set are deployable from a spring-loaded position in which distal ends of the active fixation tines point away from the IMD to a hooked position in which the active fixation tines bend back towards the IMD. The active fixation tines are configured to secure the IMD to a patient tissue when deployed while the distal ends of the active fixation tines are positioned adjacent to the patient tissue.

Claims (43)

1. A method of implanting an assembly in a patient, the assembly comprising:

a leadless pacemaker comprising a first electrode and circuitry configured to deliver cardiac pacing stimulation via the first electrode; and

a set of fixation tines attached adjacent a distal end of the leadless pacemaker,

wherein the method comprises:

holding, within a lumen of a delivery catheter, the leadless pacemaker and the set of fixation tines, wherein the lumen is sized to hold each fixation tine of the set in a spring-loaded position;

deploying at least one fixation tine of the set from the spring-loaded position toward a hooked position while respective proximal ends of the fixation tines of the set remain fixed to the leadless pacemaker, wherein the at least one fixation tine of the set is configured to be deployed from the spring-loaded position toward the hooked position by being released from the spring-loaded position and allowed to bend toward the hooked position, and wherein deploying the at least one fixation tine of the set causes a distal end of the at least one fixation tine of the set to penetrate cardiac tissue; and

after deployment, testing the leadless pacemaker to assess a performance characteristic of the first electrode with respect to electrode-tissue connection.

2. The method of claim 1 , wherein, when the fixation tines of the set are in the spring-loaded position, respective proximal portions of the fixation tines of the set extend distally from the distal end of the leadless pacemaker.

3. The method of claim 1 , wherein, when the fixation tines of the set are in the spring-loaded position, the fixation tines of the set extend into a distal portion of the delivery catheter, distal of the distal end of the leadless pacemaker.

4. The method of claim 1 , further comprising releasing the leadless pacemaker from the cardiac tissue by pulling the leadless pacemaker away from the cardiac tissue and into the delivery catheter, thereby returning the set of fixation tines to the spring-loaded position.

The method of claim 4 , further comprising, after releasing the leadless pacemaker from the cardiac tissue, subsequently deploying at least one fixation tine of the set from the spring-loaded position toward the hooked position, wherein subsequently deploying the at least one fixation tine of the set causes a distal end of the subsequently deployed at least one fixation tine of the set to penetrate cardiac tissue.

6. The method of claim 5 , further comprising repositioning the leadless pacemaker after said releasing the leadless pacemaker from the cardiac tissue, and before said subsequently deploying at least one fixation tine of the set from the spring-loaded position toward the hooked position.

7. The method of claim 4 , wherein releasing the leadless pacemaker from the cardiac tissue comprises doing so without tearing the cardiac tissue.

8. The method of claim 1 , wherein a tether is configured to secure the leadless pacemaker with respect to the delivery catheter, and the method further comprises releasing the leadless pacemaker from the tether.

9. The method of claim 1 , wherein the delivery catheter comprises a distal end, and the method further comprises pressing the catheter distal end against the cardiac tissue.

10. The method of claim 1 , further comprising using a programmer to retrieve information from the leadless pacemaker.

11. A method comprising:

obtaining an assembly comprising a leadless pacemaker and a set of fixation tines attached adjacent a distal end of the leadless pacemaker, wherein the fixation tines of the set are movable from a spring-loaded position to a hooked position while respective proximal ends of the fixation tines of the set remain fixed to the leadless pacemaker;

holding, within a lumen of a delivery catheter, the leadless pacemaker and the set of fixation tines in the spring-loaded position;

deploying at least one fixation tine of the set from the spring-loaded position toward the hooked position by releasing the at least one fixation tine of the set from the spring-loaded position such that it bends toward the hooked position, whereby a distal end of the at least one fixation tine of the set penetrates cardiac tissue; and

after deployment, testing the leadless pacemaker to assess a performance characteristic.

12. The method of claim 11 , wherein, when the fixation tines of the set are in the spring-loaded position, respective proximal portions of the fixation tines of the set extend distally from the distal end of the leadless pacemaker.

13. The method of claim 11 , wherein, when the fixation tines of the set are in the spring-loaded position, the fixation tines of the set extend into a distal portion of the delivery catheter, distal of the distal end of the leadless pacemaker.

14. The method of claim 11 , further comprising releasing the leadless pacemaker from the cardiac tissue by pulling the leadless pacemaker away from the cardiac tissue and into the delivery catheter, thereby returning the set of fixation tines to the spring-loaded position.

The method of claim 14 , further comprising, after releasing the leadless pacemaker from the cardiac tissue, subsequently deploying at least one fixation tine of the set from the spring-loaded position toward the hooked position, whereby a distal end of the subsequently deployed at least one fixation tine of the set penetrates cardiac tissue.

16. The method of claim 15 , further comprising repositioning the leadless pacemaker after said releasing the leadless pacemaker from the cardiac tissue, and before said subsequently deploying at least one fixation tine of the set from the spring-loaded position toward the hooked position.

17. The method of claim 14 , wherein releasing the leadless pacemaker from the cardiac tissue comprises doing so without tearing the cardiac tissue.

18. The method of claim 11 , wherein a tether is configured to secure the leadless pacemaker with respect to the delivery catheter, and the method further comprises releasing the leadless pacemaker from the tether.

19. The method of claim 11 , wherein the delivery catheter comprises a distal end, and the method further comprises pressing the catheter distal end against the cardiac tissue.

20. The method of claim 11 , further comprising using a programmer to retrieve information from the leadless pacemaker.

21. A method for use of an assembly comprising a leadless pacemaker and a plurality of fixation tines attached to the leadless pacemaker, the method comprising:

holding, within a lumen of a delivery catheter, the leadless pacemaker and the fixation tines, with the fixation tines in a spring-loaded position and with the fixation tines extending to a location within the catheter distal of the distal end of the leadless pacemaker;

deploying the fixation tines from the spring-loaded position toward a hooked position by releasing the fixation tines from the spring-loaded position, such that the fixation tines bend toward the hooked position, wherein deploying the fixation tines causes a distal end of at least one fixation tine of the plurality to penetrate cardiac tissue; and

after deployment, testing the leadless pacemaker to assess a performance characteristic.

22. The method of claim 21 , wherein, when the fixation tines are in the spring-loaded position, respective proximal portions of the fixation tines extend distally from the distal end of the leadless pacemaker.

23. The method of claim 21 , further comprising releasing the leadless pacemaker from the cardiac tissue by pulling the leadless pacemaker away from the cardiac tissue and into the delivery catheter, thereby returning the fixation tines to the spring-loaded position.

24. The method of claim 23 , further comprising, after releasing the leadless pacemaker from the cardiac tissue, subsequently deploying the fixation tines from the spring-loaded position toward the hooked position, wherein subsequently deploying the fixation tines causes a distal end of at least one fixation tine to penetrate cardiac tissue.

25. The method of claim 23 , wherein releasing the leadless pacemaker from the cardiac tissue comprises doing so without tearing the cardiac tissue.

26. The method of claim 21 , wherein a tether is configured to secure the leadless pacemaker with respect to the delivery catheter, and the method further comprises releasing the leadless pacemaker from the tether.

27. The method of claim 26 , wherein testing the leadless pacemaker comprising testing the leadless pacemaker before releasing the leadless pacemaker from the tether.

28. The method of claim 21 , wherein the delivery catheter comprises a distal end, and the method further comprises pressing the catheter distal end against the cardiac tissue.

29. The method of claim 21 , further comprising using a programmer to retrieve information from the leadless pacemaker.

30. The method of claim 21 , wherein the fixation tines are configured to move from the spring-loaded position toward the hooked position while respective proximal ends of the fixation tines remain fixed to the leadless pacemaker.
