Abstract

Transcorneal and fiberoptic laser delivery systems and methods for the treatment of eye diseases wherein energy is delivered by wavelengths transparent to the cornea to effect target tissues in the eye for the control of intraocular pressure in diseases such as glaucoma by delivery systems both external to and within ocular tissues. External delivery may be affected under gonioscopic control. Internal delivery may be controlled endoscopically or fiberoptically, both systems utilizing femtosecond laser energy to excise ocular tissue. The femtosecond light energy is delivered to the target tissues to be treated to effect precisely controlled photodisruption to enable portals for the outflow of aqueous fluid in the case of glaucoma in a manner which minimizes target tissue healing responses, inflammation and scarring.

Claims (19)

1. An ophthalmic surgical system to treat an eye of a patient, the system comprising:

a lens configured to couple to the eye, the lens comprising a concave surface, the concave surface configured to couple to a cornea of the eye;

wherein the lens is configured to receive a bidirectional beam and redirect the bidirectional beam through the cornea of the eye and toward a juxtacanalicular trabecular meshwork of the eye;

a laser source configured to deliver energy to the juxtacanalicular trabecular meshwork along a beam pathway;

an imaging apparatus configured to view a surgical procedure at the juxtacanalicular trabecular meshwork along the beam pathway; and

a bidirectional optic disposed to combine a first beam delivered by the laser source and a second beam associated with the imaging apparatus to form the bidirectional beam;

wherein the laser source is configured with pulse parameters to selectively photodisrupt a targeted tissue of the juxtacanalicular trabecular meshwork.

2. The ophthalmic surgical system of claim 1 , further comprising a mirror for redirecting the bidirectional beam to the lens.

3. The ophthalmic surgical system of claim 2 , further comprising a servo system coupled to the mirror and configured to change an orientation of the mirror to scan the bidirectional beam in two or more directions.

4. The ophthalmic surgical system of claim 3 , wherein the servo system is configured to focus the bidirectional beam onto a curved surface of the juxtacanalicular trabecular meshwork.

5. The ophthalmic surgical system of claim 1 , wherein the lens comprises a meniscus lens having a peripheral region and a central region and wherein the lens is thicker nearer the central region and thinner nearer the peripheral region.

6. The ophthalmic surgical system of claim 1 , wherein the lens comprises an indirect goniolens.

7. The ophthalmic surgical system of claim 1 , wherein the lens is configured to couple to the eye with a suction ring, the suction ring configured to contact a sclera of the eye.

8. The ophthalmic surgical system of claim 7 , further comprising a source of vacuum in communication with an annular cavity and configured to draw a vacuum and anchor the suction ring to the sclera of the patient.

9. The ophthalmic surgical system of claim 1 , wherein the imaging apparatus comprises an optical coherence tomography device.

10. The ophthalmic surgical system of claim 1 , wherein the imaging apparatus comprises a source of illumination.

11. The ophthalmic surgical system of claim 1 , wherein the targeted tissue comprises tissue of a corneo scleral angle of the eye.

12. The ophthalmic surgical system of claim 1 , wherein the targeted tissue comprises one or more of a trabecular meshwork, portions of Schlemm's canal, collector channels, aqueous veins, or a sclera of the eye.

13. The ophthalmic surgical system of claim 1 , wherein the targeted tissue comprises a trabecular meshwork, the juxtacanalicular trabecular meshwork, and portions of Schlemm's canal.
