Abstract

A medical device is configured to detect an atrial tachyarrhythmia episode. The device senses a cardiac signal, identifies R-waves in the cardiac signal attendant ventricular depolarizations and determines classification factors from the R-waves identified over a predetermined time period. The device classifies the predetermined time period as one of unclassified, atrial tachyarrhythmia and non-atrial tachyarrhythmia by comparing the determined classification factors to classification criteria. A classification criterion is adjusted from a first classification criterion to a second classification criterion after at least one time period being classified as atrial tachyarrhythmia. An atrial tachyarrhythmia episode is detected by the device in response to at least one subsequent time period being classified as atrial tachyarrhythmia based on the adjusted classification criterion.

Claims (88)

1. A medical device system comprising:

sensing circuitry configured to sense a cardiac signal; and

processing circuitry configured to:

classify a first time period as a tachyarrhythmia based on the cardiac signal sensed during the first time period and a first threshold without detecting a tachyarrhythmia episode based only on the first time period;

classify a second time period as the tachyarrhythmia based on the cardiac signal sensed during the second time period and a second threshold different than the first threshold; and

responsive to classifying at least the first time period and the second time period as the tachyarrhythmia, detect a tachyarrhythmia episode comprising at least the first time period and the second time period;

determine tachyarrhythmia data by at least determining a tachyarrhythmia burden based on at least a combined time of the first time period and the second time period; and

a memory configured to store the determined tachyarrhythmia data; and

a first communication circuit configured to transmit at least a portion of the tachyarrhythmia data including the determined tachyarrhythmia burden.

2. The medical device system of claim 1 wherein the processing circuitry is further configured to:

classify a third time period as non-tachyarrhythmia based on the cardiac signal sensed during the third time period and the second threshold;

detect termination of the tachyarrhythmia episode in response to classifying the third time period as non-tachyarrhythmia; and

determine the tachyarrhythmia burden based on at least the combined time of the first time period and the second time period and excluding the third time period.

3. The medical device system of claim 1 wherein the processing circuitry is further configured to:

based on the second threshold, classify one or more intervening time periods between the first time period and the second time period, wherein each of the one or more intervening time periods being classified as one of unclassified or tachyarrhythmia; and

determine the tachyarrhythmia burden based on the combined time of the first time period and the second time period and the one or more intervening time periods.

4. The medical device system of claim 3 wherein the processing circuitry is further configured to classify at least one of the one or more intervening time periods as unclassified by at least one of:

determining from the cardiac signal sensed during the intervening time period that less than a threshold number of RR interval pairs occur;

determining from the cardiac signal sensed during the intervening time period that a count of short RR intervals is greater than a short interval count threshold;

determining that more than a threshold number of ventricular pacing pulses are delivered during the intervening time period;

determining from the cardiac signal sensed during the intervening time period that a ventricular tachyarrhythmia detection is being made during the intervening time period; or

detecting ventricular oversensing from the cardiac signal sensed during the intervening time period.

5. The medical device system of claim 1 wherein the processing circuitry is further configured to adjust the first threshold to the second threshold in response to classifying the first time period as tachyarrhythmia.

6. The medical device system of claim 1 wherein the processing circuitry is further configured to:

detect a plurality of tachyarrhythmia episodes comprising a first the tachyarrhythmia episode that includes the first time period and the second time period, wherein each of the plurality of tachyarrhythmia episodes being detected based on the first threshold and the second threshold;

determine a duration of each of the plurality of tachyarrhythmia episodes; and

determine the tachyarrhythmia data by updating the tachyarrhythmia burden based on the determined duration of each of the plurality of tachyarrhythmia episodes.

7. The medical device system of claim 6 wherein the processing circuitry is further configured to determine the tachyarrhythmia data transmitted by the first communication circuit by determining a longest duration of the determined durations.

8. The medical device system of claim 1 further comprising:

an implantable medical device comprising the sensing circuitry, the processing circuitry, the memory and the first communication circuit; and

an external device comprising:

a second communication circuit configured to receive the transmitted tachyarrhythmia data; and

a display unit configured to display the tachyarrhythmia data comprising at least the tachyarrhythmia burden.

9. The medical device system of claim 1 further comprising:

a therapy delivery circuit configured to deliver a cardiac electrical stimulation therapy; and

wherein the processing circuitry is further configured to control delivery of the cardiac electrical stimulation therapy by the therapy delivery circuit based on at least the detected tachyarrhythmia episode.

10. The medical device system of claim 1 further comprising:

a therapy delivery circuit configured to deliver a therapy; and

wherein the processing circuitry is further configured to:

control the therapy delivery circuit to withhold the therapy in response to the detected tachyarrhythmia episode;

detect a ventricular tachyarrhythmia based on the sensed cardiac signal;

classify a third time period as non-tachyarrhythmia based on the cardiac signal sensed during the third time period; and

control the therapy delivery circuit to deliver the therapy in response to the detected ventricular tachyarrhythmia and the third time period being classified as non-tachyarrhythmia.

11. A method comprising:

sensing a cardiac signal;

classifying a first time period as a tachyarrhythmia based on the cardiac signal sensed during the first time period and a first threshold without detecting a tachyarrhythmia episode based only on the first time period;

classifying a second time period as the tachyarrhythmia based on the cardiac signal sensed during the second time period and a second threshold different than the first threshold;

responsive to classifying at least the first time period and the second time period as the tachyarrhythmia, detecting a tachyarrhythmia episode comprising at least the first time period and the second time period;

determining tachyarrhythmia data by at least determining a tachyarrhythmia burden based on at least a combined time of the first time period and the second time period;

storing the tachyarrhythmia data in a memory; and

transmitting at least a portion of the tachyarrhythmia data including the determined tachyarrhythmia burden.

12. The method of claim 11 further comprising:

classifying a third time period as non-tachyarrhythmia based on the cardiac signal sensed during the third time period and the second threshold;

detecting termination of the tachyarrhythmia episode in response to classifying the third time period as non-tachyarrhythmia; and

determining the tachyarrhythmia burden based on at least the combined time of the first time period and the second time period and excluding the third time period.

13. The method of claim 11 further comprising:

based on the second threshold, classifying one or more intervening time periods between the first time period and the second time period, wherein each of the one or more intervening time periods being classified as one of unclassified or tachyarrhythmia; and

determining the tachyarrhythmia burden based on the combined time of the first time period and the second time period and the one or more intervening time periods.

14. The method of claim 13 further comprising classifying at least one of the one or more intervening time periods as unclassified by at least one of:

determining from the cardiac signal sensed during the intervening time period that less than a threshold number of RR interval pairs occur;

determining from the cardiac signal sensed during the intervening time period that a count of short RR intervals is greater than a short interval count threshold;

determining that more than a threshold number of ventricular pacing pulses are delivered during the intervening time period;

determining from the cardiac signal sensed during the intervening time period that a ventricular tachyarrhythmia detection is being made during the intervening time period; or

detecting ventricular oversensing from the cardiac signal sensed during the intervening time period.

15. The method of claim 11 further comprising adjusting the first threshold to the second threshold in response to classifying the first time period as tachyarrhythmia.

16. The method of claim 11 further comprising:

detecting a plurality of tachyarrhythmia episodes comprising a first the tachyarrhythmia episode including the first time period and the second time period, wherein each of the plurality of tachyarrhythmia episodes being detected based on the first threshold and the second threshold;

determining a duration of each of the plurality of tachyarrhythmia episodes; and

determining the tachyarrhythmia data by updating the tachyarrhythmia burden based on the determined duration of each of the plurality of tachyarrhythmia episodes.

17. The method of claim 16 further comprising determining the tachyarrhythmia data by determining a longest duration of the determined durations.

18. The method of claim 11 further comprising:

transmitting the determined tachyarrhythmia burden by a first communication circuit of an implantable medical device; and

receiving the tachyarrhythmia burden by a second communication circuit of an external medical device; and

displaying the tachyarrhythmia burden by a display unit of the external medical device.

19. The method of claim 11 further comprising:

withholding a cardiac electrical stimulation therapy in response to the detected tachyarrhythmia episode;

detecting a ventricular tachyarrhythmia based on the sensed cardiac signal;

classifying a third time period as non-tachyarrhythmia based on the cardiac signal sensed during the third time period; and

delivering the cardiac electrical stimulation therapy in response to the detected ventricular tachyarrhythmia and the third time period being classified as non-tachyarrhythmia.

20. A non-transitory computer readable medium storing a set of instructions, which when executed by processing circuitry of a medical device system, cause the system to:

sense a cardiac signal;

classify a first time period as a tachyarrhythmia based on the cardiac signal sensed during the first time period and a first threshold without detecting a tachyarrhythmia episode based only on the first time period;

classify a second time period as the tachyarrhythmia based on the cardiac signal sensed during the second time period and a second threshold different than the first threshold;

responsive to classifying at least the first time period and the second time period as the tachyarrhythmia, detect a tachyarrhythmia episode comprising at least the first time period and the second time period;

determine tachyarrhythmia data by at least determining a tachyarrhythmia burden based on at least a combined time of the first time period and the second time period;

store the tachyarrhythmia data in a memory of the medical device system;

transmit at least a portion of the tachyarrhythmia data including the determined tachyarrhythmia burden; and

display the tachyarrhythmia burden in a user interface by a display unit of the medical device system.
