Abstract

The invention is a delivery catheter, e.g., a guide catheter, having an imaging element in proximity to the distal end of the catheter. Catheters of the invention are able to placement of the catheter in proximity to an ostium, thereby increasing the efficiency of contrast delivery while reducing the risk of ischemia due to blocked blood supply. The invention additionally assists in maintaining catheter position during delivery of a fluid from the catheter, resulting in better performance with less fluid. For example, a catheter of the invention can be used to produce improved fluoroscopic images with less overall contrast. This improvement reduces the risk of an allergic reaction to the contrast while decreasing the length of time for a procedure, i.e., because of a need to re-contrast.

Claims (36)

1. A delivery catheter for delivering a contrast agent fluid to a blood vessel through an ostium, comprising:

an elongated member having a proximal connector and a distal end, the elongated member configured to be inserted into the blood vessel of a patient;

a delivery lumen axially disposed within the elongated member providing fluid communication between the proximal connector and the distal end of the elongated member, wherein the proximal connector is in fluid communication with a supply of the contrast agent fluid such that the contrast agent fluid is delivered into the blood vessel at a distal end of the delivery lumen located at the distal end of the elongated member;

an expandable member having a distal end, wherein the expandable member is radially expandable beyond an outer radius of the elongated member, wherein the expandable member is coupled to the elongated member and disposed proximal to the distal end of the elongated member, and wherein when expanded, the expandable member is prevented from entering the ostium, thereby assuring placement of the distal end of the elongated member inside the ostium at a predetermined length from the distal end of the expandable member for delivering the contrast agent fluid through the delivery lumen and directing the contrast agent fluid delivered into the blood vessel away from the distal end of the delivery lumen; and

an imaging element comprising a proximal end and a distal end, wherein the distal end of the imaging element is disposed proximal to the distal end of the delivery lumen at a predetermined distance of less than 1 cm, such that the imaging element provides internal imaging of the blood vessel that is flush with the distal end of the elongated member, and the proximal end of the imaging element is disposed directly adjacent and distal to, and in physical contact with, the distal end of the expandable member, wherein the imaging element is configured to obtain intraluminal images facilitating visualization of the vasculature for guided placement of the delivery catheter into the ostium, and wherein the imaging element comprises an ultrasound transducer ring array positioned around the elongate member and the delivery lumen.

2. The delivery catheter of claim 1 , wherein the delivery catheter has an over-the-wire configuration or a rapid-exchange configuration.

3. The delivery catheter of claim 2 , wherein the rapid-exchange configuration comprises a rapid exchange lumen.

4. The delivery catheter of claim 3 , wherein a proximal end of the rapid exchange lumen comprises an atraumatic catheter port.

5. The delivery catheter of claim 4 , wherein the delivery catheter further comprises a push rod configured to engage with the proximal end of the rapid exchange lumen.

6. The delivery catheter of claim 1 , wherein a signal wire of the imaging element is bonded to at least a portion of a wall of a catheter sheath.

7. A method for delivering a contrast agent fluid to a blood vessel through an ostium, the method comprising:

inserting a delivery catheter through vasculature of a patient, the delivery catheter comprising:

an elongated member having a proximal connector and a distal end, the elongated member configured to be inserted through the vasculature of the patient;

a delivery lumen axially disposed within the elongated member providing fluid communication between the proximal connector and the distal end of the elongated member, wherein the proximal connector is in fluid communication with a supply of the contrast agent fluid such that the fluid is delivered through the ostium and into the blood vessel at a distal end of the delivery lumen located at the distal end of the elongated member to enable external imaging of the blood vessel;

an expandable member having a distal end, wherein the expandable member is radially expandable beyond an outer radius of the elongated member, and wherein the expandable member is coupled to the elongated member and disposed proximal to the distal end of the elongated member; and

an imaging element comprising a proximal end and a distal end, wherein the distal end of the imaging element is disposed proximal to the distal end of the elongated member and the proximal end of the imaging element is disposed directly adjacent and distal to, and in physical contact with, the distal end of the expandable member, wherein a distance between a distal tip of the delivery catheter and the distal end of the imaging element is a predetermined distance of less than 1 cm, such that the imaging element internally images vasculature that is flush with the distal end of the elongated member, and wherein the imaging element comprises an ultrasound transducer ring array positioned around the elongate member and the delivery lumen to thereby permit passage of the contrast agent fluid through the imaging element to the distal end of the delivery lumen;

imaging, with the imaging element, within the vasculature in order to obtain intraluminal images facilitating visualization of a location of the ostium from the distal end of the elongated member for guided placement of the delivery catheter for fluid delivery through the ostium and to direct the contrast agent fluid delivered into the blood vessel away from the distal end of the delivery lumen;

based on the imaging, adjusting a position of the distal end of the elongated member such that it enters a desired distance through the ostium and into the blood vessel;

after adjusting the position of the distal end of the elongated member, expanding the expandable member; and

introducing the contrast agent fluid, via the delivery catheter, into the blood vessel, wherein when expanded, the expandable member is prevented from entering the ostium, thereby assuring placement of the distal end of the elongated member inside the ostium at the predetermined distance for delivering the contrast agent fluid through the delivery lumen and directing the contrast agent fluid delivered into the blood vessel away from the distal end of the delivery lumen.

8. The method of claim 7 , further comprising the step of: obtaining external imaging data of the blood vessel.

9. The method of claim 7 , wherein the delivery catheter defines an opening at the distal tip of the delivery catheter.

10. The delivery catheter of claim 1 , wherein the ultrasound transducer ring array comprises a transducer element selected from a group consisting of a piezoelectric micromachined ultrasound transducer, a capacitive micromachined ultrasound transducer, and a photoacoustic ultrasound transducer.

11. The delivery catheter of claim 1 , wherein the imaging element is stationary relative to the expandable member.

12. The delivery catheter of claim 1 , wherein the elongated member comprises an internal lumen extending from the imaging element to the proximal end of the elongated member, and wherein the imaging element is coupled to a plurality of signal lines that extend through the internal lumen.

13. The delivery catheter of claim 12 , wherein the internal lumen extends through the expandable member to permit communication with the imaging element that is distal to the expandable member.

14. A delivery catheter for delivering a contrast agent fluid to a blood vessel through an ostium, the delivery catheter comprising:

an elongated member having a proximal connector and a distal end, the elongated member configured to be inserted into the blood vessel of a patient;

a delivery lumen axially disposed within the elongated member providing fluid communication between the proximal connector and the distal end of the elongated member, wherein the proximal connector is in fluid communication with a supply of the contrast agent fluid such that the contrast agent fluid is delivered into the blood vessel at a distal end of the delivery lumen located at the distal end of the elongated member to enable external imaging of the blood vessel;

an expandable member coupled to the elongated member and configured to radially expand beyond an outer radius of the elongated member, wherein when expanded, the expandable member is prevented from entering the ostium, thereby assuring placement of the distal end of the elongated member inside the ostium at a predetermined length from a distal end of the expandable member for delivering the contrast agent fluid through the delivery lumen and directing the contrast agent fluid delivered into the blood vessel away from the distal end of the delivery lumen; and

an imaging element having an imaging plane that is flush with a distal tip of the delivery catheter and is configured to obtain intraluminal images of a cross-section of the blood vessel, without rotation of the delivery catheter, that facilitate visualization of vasculature for guided placement of the delivery catheter into the ostium.

15. The delivery catheter of claim 14 , wherein the expandable member is disposed proximal to the distal end of the elongated member, and

wherein the delivery catheter further comprises markers distal to the expandable member.

16. The delivery catheter of claim 1 , wherein the imaging element further facilitates visualization of the blood vessel for placement of the delivery catheter.

17. The delivery catheter of claim 1 , wherein the ultrasound transducer ring array is configured to obtain the intraluminal images of a cross-section of the blood vessel, without rotation of the delivery catheter.

18. The delivery catheter of claim 14 , wherein the ultrasound transducer ring array is configured to obtain the intraluminal images of a cross-section of the blood vessel, without rotation of the delivery catheter.
