Abstract

This document discusses, among other things, systems and methods to receive cardiac electrical information of a subject, detect a premature ventricular contraction (PVC) event in a first detection window using the received cardiac electrical information, determine a count of detected PVC events in the first detection window, remove cardiac electrical information associated with the detected PVC event from the first detection window based on the determined count of detected PVC events, and detect an indication of atrial fibrillation of the subject for the first detection window using remaining cardiac electrical information in the first detection window.

Claims (49)

1. A system to optimize cardiac monitoring resources, comprising: a signal receiver circuit configured to receive cardiac electrical information of a subject; and an assessment circuit configured to:

detect a premature ventricular contraction (PVC) event in a first detection window using the received cardiac electrical information from the first detection window, wherein the first detection window comprises a number of cardiac intervals or a time period comprising a plurality of cardiac intervals;

determine a count of detected PVC events in the first detection window;

remove cardiac electrical information associated with the detected PVC event from the first detection window if the count of detected PVC events in the first detection window is below an upper threshold; and

detect an indication of atrial fibrillation of the subject for the first detection window using remaining cardiac electrical information in the first detection window.

2. The system of claim 1 , wherein the assessment circuit is configured to:

control storage in a memory circuit or transmission over a network interface of the received cardiac electrical information for the first detection window based on the determined count of detected PVC events in the first detection window.

3. The system of claim 1 , wherein the assessment circuit is configured to:

remove the cardiac electrical information associated with the detected PVC event from the first detection window if the count of detected PVC events in the first detection window is above a first threshold and below the upper threshold; and

detect the indication of atrial fibrillation of the subject for the first detection window using remaining cardiac electrical information in the first detection window.

4. The system of claim 1 , wherein the assessment circuit is configured to:

remove the cardiac electrical information associated with the detected PVC event and cardiac electrical information from a cardiac interval at least one of preceding or proceeding a cardiac interval of the detected PVC event from the first detection window if the count of detected PVC events in the first detection window is above a second threshold; and

detect the indication of atrial fibrillation of the subject for the first detection window using remaining cardiac electrical information in the first detection window.

5. The system of claim 1 , wherein the assessment circuit is configured to:

suspend detection of atrial fibrillation in the first detection window if the count of detected PVC events in the first detection window is above the upper threshold.

6. The system of claim 1 , wherein the first detection window is the time period comprising the plurality of cardiac intervals.

7. The system of claim 1 , wherein the first detection window is a number of detected cardiac intervals.

8. The system of claim 1 , comprising an implantable medical device comprising the signal receiver circuit and the assessment circuit.

The system of claim 2 , wherein the assessment circuit is configured to control storage or transmission of the received cardiac electrical information for the first detection window based on the determined count of detected PVC events in the first detection window relative to the time period or the number of cardiac intervals of the first detection window or the determined count of detected PVC events in the first detection window exceeding a threshold with respect to number of cardiac intervals in the first detection window.

10. A method of optimizing cardiac monitoring resources, comprising:

receiving, using a signal receiver circuit, cardiac electrical information of a subject;

detecting, using an assessment circuit, a premature ventricular contraction (PVC) event in a first detection window using the received cardiac electrical information from the first detection window, wherein the first detection window comprises a number of cardiac intervals or a time period comprising a plurality of cardiac intervals;

determining, using the assessment circuit, a count of detected PVC events in the first detection window;

removing, using the assessment circuit, cardiac electrical information associated with the detected PVC event from the first detection window if the count of detected PVC events in the first- detection window is below an upper threshold; and

detecting, using the assessment circuit, an indication of atrial fibrillation of the subject for the first detection window using remaining cardiac electrical information in the first detection window.

11. The method of claim 10 , comprising:

controlling, using the assessment circuit, storage in a memory circuit or transmission over a network interface of the received cardiac electrical information for the first detection window based on the determined count of detected PVC events in the first detection window.

12. The method of claim 10 , comprising:

removing, using the assessment circuit, the cardiac electrical information associated with the detected PVC event from the first detection window if the count of detected PVC events in the first detection window is above a first threshold and below the upper threshold; and

detecting, using the assessment circuit, the indication of atrial fibrillation of the subject for the first detection window using remaining cardiac electrical information in the first detection window.

13. The method of claim 10 , comprising:

removing, using the assessment circuit, the cardiac electrical information associated with the detected PVC event and cardiac electrical information from a cardiac interval at least one of preceding or proceeding a cardiac interval of the detected PVC event from the first detection window if the count of detected PVC events in the first detection window is above a second threshold; and

detecting, using the assessment circuit, the indication of atrial fibrillation of the subject for the first detection window using remaining cardiac electrical information in the first detection window.

14. The method of claim 10 , comprising:

suspending, using the assessment circuit, detection of atrial fibrillation in the first detection window if the count of detected PVC events in the first detection window is above the upper threshold.

15. The method of claim 10 , wherein the first detection window is the time period comprising the plurality of cardiac intervals.

16. The method of claim 10 , wherein the first detection window is a number of detected cardiac intervals.

The method of claim 11 , wherein controlling storage or transmission of the received cardiac electrical information for the first detection window based on the determined count of detected PVC events in the first detection window comprises based on the determined count of detected PVC events in the first detection window relative to the time period or the number of cardiac intervals of the first detection window or the determined count of detected PVC events in the first detection window exceeding a threshold with respect to number of cardiac intervals in the first detection window.

18. A system to optimize cardiac monitoring resources, comprising:

a signal receiver circuit configured to receive cardiac electrical information of a subject; and

an assessment circuit configured to:

detect a premature ventricular contraction (PVC) event using the received cardiac electrical information;

detect, using the received cardiac electrical information, an indication of atrial fibrillation of a subject using one of multiple atrial fibrillation detection modes, each of the multiple atrial fibrillation detection modes configured to detect atrial fibrillation of the subject;

determine a count of detected PVC events in a first detection window;

transition between different first and second atrial fibrillation detection modes of the multiple atrial fibrillation detection modes based on the determined count of PVC events in the first detection window; and

determine one or more conditions in order to deliver a therapy to the subject based on the indication of atrial fibrillation of the subject.

19. The system of claim 18 , wherein, if the determined count of detected PVC events is below a first threshold, the assessment circuit is configured to detect atrial fibrillation of the subject using a low-PVC burden mode,

wherein, if the determined count of detected PVC events meets or exceeds the first threshold, the assessment circuit is configured to remove cardiac electrical information associated with the detected PVC event from the first detection window and to detect the indication of atrial fibrillation of the subject in the first detection window using remaining cardiac electrical information in the first detection window.

The system of claim 19 , wherein, if the determined count of detected PVC events meets or exceeds the first threshold, the assessment circuit is configured to remove cardiac electrical information associated with the detected PVC event and cardiac electrical information from a cardiac interval at least one of preceding or proceeding a cardiac interval of the detected PVC event from the first detection window and to detect the indication of atrial fibrillation of the subject in the first detection window using remaining cardiac electrical information in the first detection window.
