Abstract

Described herein are systems and methods for applying extremely low duty-cycle stimulation sufficient to treat chronic inflammation using feedback to adjust the off times between stimulations. In particular, the feedback include an assessment of the level of inflammation by the patient or the healthcare provider, or by measure the level of an inflammatory analyte or biomarker, or by detecting nerve activity correlated with inflammation.

Claims (36)

1. A system for treating chronic inflammation in a subject, the system comprising:

an implantable microstimulator configured to apply a stimulation dose to a vagus nerve, wherein the stimulation dose provides a single supra-threshold pulse or a burst of supra-threshold pulses, wherein a pulse width of the single supra-threshold pulse or each of the pulses of the burst of supra-threshold pulses is from 20 microsec to 2000 microsec, wherein each supra-threshold pulse is a biphasic pulse comprising a first pulse and a second pulse separated by a non-zero inter-pulse interval (IPI), and wherein the first pulse has an opposite pulse amplitude as the second pulse;

a nerve cuff comprising at least two electrodes configured to enclose the implantable microstimulator and to secure it around the vagus nerve; and

a controller adapted to adjust a dose regimen, wherein the dose regimen comprises a pulse amplitude, wherein the controller adjusts the pulse amplitude based on a monitored degree or level of inflammation, wherein each single supra-threshold pulse or burst of supra-threshold pulses results in long-lasting suppression of the monitored degree or level of inflammation and an off period, such that the controller adapts the dose regimen to set progressively delayed single supra-threshold pulses and progressively longer off periods, wherein the controller includes a bipolar current source to generate the biphasic pulse for the at least two electrodes,

wherein the dose regimen applied at the progressively delayed single supra-threshold pulses and the progressively longer off periods maintains an ongoing inhibition of the monitored degree or level of inflammation.

2. The system of claim 1 , wherein the controller adjusts the dose regimen based on the level of inflammation based on an electrical activity of the vagus nerve.

3. The system of claim 1 , wherein the off period is adjustable based on feedback from a sensor, wherein the controller is configured to measure an indicator of a level of inflammation based on an electrical activity of the vagus nerve.

4. The system of claim 1 , wherein the off period is between 4 hours and 48 hours.

5. The system of claim 1 , wherein the off period is between 8 hours and 48 hours.

6. The system of claim 1 , wherein the single supra-threshold pulse or the burst of supra-threshold pulses has a pulse amplitude of less than 5 mA.

7. The system of claim 1 , wherein the single supra-threshold pulse or the burst of supra-threshold pulses has a pulse amplitude of less than 3 mA.

8. The system of claim 1 wherein the single supra-threshold pulse or the burst of supra-threshold pulses has a pulse amplitude of less than 2 mA.

9. The system of claim 1 , wherein the burst of supra-threshold pulses has a burst duration of less than 5 minutes.

10. The system of claim 1 , wherein the burst of supra-threshold pulses comprises between 1 and 5 supra-threshold pulses.

11. The system of claim 1 , wherein the IPI is at least 50 microseconds.

12. A method of treating chronic inflammation in a patient, the method comprising:

applying a dose regimen comprising a series of stimulation doses from a microstimulator to a vagus nerve through at least two electrodes, wherein the microstimulator is secured to the vagus nerve by a nerve cuff;

monitoring an indicator of a degree or level of inflammation; and

adjusting a pulse amplitude of the stimulation dose based on the monitored degree or level of inflammation,

wherein each stimulation dose comprises only a single supra-threshold stimulus pulse or a burst of supra-threshold pulses, wherein a pulse width of the single supra-threshold pulse or each of the pulses of the burst of supra-threshold pulses is from 20 microsec to 2000 microsec, and wherein each supra-threshold pulse is a biphasic pulse comprising a first pulse and a second pulse separated by a non-zero inter-pulse interval (IPI), and wherein the first pulse has an opposite pulse amplitude as the second pulse, wherein the biphasic pulse is generated by a bipolar current source coupled to the at least two electrodes,

wherein each single supra-threshold pulse or burst of supra-threshold pulses results in long-lasting suppression of the degree or level of inflammation and an off-period, thereby adapting the dose regimen to set progressively delayed single supra-threshold pulses and progressively longer off periods,

wherein the dose regimen applied at the progressively delayed single supra-threshold pulses and the progressively longer off periods maintains an ongoing inhibition of the monitored degree or level of inflammation.

13. The method of claim 12 , wherein the off-time is at least 24 hours.

14. The method of claim 12 , wherein the off-time is at least 48 hours.

15. The method of claim 12 , wherein the single supra-threshold stimulus pulse or the burst of supra-threshold pulses has a pulse amplitude of less than 5 mA.

16. The method of claim 12 , wherein the single supra-threshold stimulus pulse or the burst of supra-threshold pulses has a pulse amplitude of less than 3 mA.

17. The method of claim 12 , wherein the single supra-threshold stimulus pulse or the burst of supra-threshold pulses has a pulse amplitude of less than 2 mA.

18. The method of claim 12 , wherein the burst of supra-threshold pulses has a burst duration of less than 5 minutes.

19. The method of claim 11 , wherein the method is a method of treating an intestinal disorder.

20. A method of inhibiting inflammation, the method comprising:

applying a dose regimen comprising a series of stimulation doses from a microstimulator through at least two electrodes to a vagus nerve of a patient, wherein the microstimulator is secured to the vagus nerve by a nerve cuff and further wherein each stimulation dose providing only a single supra-threshold stimulus pulse or a burst of supra-threshold pulses, wherein a pulse width of the single supra-threshold pulse or each of the pulses of the burst of supra-threshold pulses is from 20 microsec to 2000 microsec, and wherein each supra-threshold pulse is a biphasic pulse comprising a first pulse and a second pulse separated by a non-zero inter-pulse interval (IPI), wherein the first pulse has an opposite pulse amplitude as the second pulse, wherein the biphasic pulse for the at least two electrodes is generated by a bipolar current source, and wherein the bipolar current source is capacitively isolated with at least 1 microfarad (uF) ceramic capacitors coupled to each electrode;

monitoring the efficacy of the stimulation dose using a sensor of the microstimulator to determine a degree or level of inflammation; and

adjusting a pulse amplitude between each stimulation dose based on the degree or level of inflammation;

wherein each single supra-threshold pulse or burst of supra-threshold pulses results in long-lasting suppression of the degree or level of inflammation and an off period, thereby adapting the dose regimen to set progressively delayed single supra-threshold pulses and progressively longer off periods,

wherein the dose regimen applied at the progressively delayed single supra-threshold pulses and the progressively longer off periods maintains an ongoing inhibition of the monitored degree or level of inflammation.

21. The method of claim 20 , wherein the method is a method of treating an intestinal disorder.
