Abstract

A system for managing an individualized cardiac rehabilitation plan for an ambulatory cardiac rehabilitation patient includes an externally worn device configured to monitor cardiac activity of the patient and a server in communication with the externally worn device. The server includes a computer-readable medium and at least one server processor configured to receive one or more inputs from a healthcare provider, develop a patient-specific cardiac rehabilitation plan, including a plurality of exercise sessions, based on the one or more inputs, transmit the rehabilitation plan to the externally worn device, receive patient data from the externally worn device, and record one or more status identifiers for each past exercise session based on the received patient data. The at least one server processor is configured to adjust the rehabilitation plan based on the one or more recorded status identifiers and transmit the adjusted rehabilitation plan to the externally worn device.

Claims (42)

1. A system for managing an individualized cardiac rehabilitation plan for an ambulatory cardiac rehabilitation patient, comprising:

an externally worn device configured to monitor cardiac activity of the patient, and

a server in communication with the externally worn device, comprising

a non-transitory computer-readable storage medium, and

at least one server processor operably coupled to the computer-readable medium, wherein the at least one server processor is configured to

receive one or more inputs from a healthcare provider related to the patient,

develop a patient-specific cardiac rehabilitation plan based on the one or more inputs from the healthcare provider, the patient-specific cardiac rehabilitation plan comprising a plurality of exercise sessions, wherein each given exercise session comprises one or more physical activities for the patient to perform during the given exercise session,

transmit the patient-specific cardiac rehabilitation plan to the externally worn device,

receive patient data from the externally worn device after the externally worn device has started administering the patient-specific cardiac rehabilitation plan to the patient,

record one or more status identifiers for each past exercise session based on the received patient data,

wherein recording the one or more status identifiers for each past exercise session comprises recording a completed status identifier in response to the patient completing the given past exercise session or a non-completed status identifier in response to the patient not completing the given past exercise session,

wherein the one or more status identifiers further comprise an over-performance status identifier recorded in response to the patient exceeding at least one requirement of the given past exercise session by at least a predetermined over-performance amount or an under-performance status identifier recorded in response to the patient satisfying at least one requirement of the given past exercise session and not meeting at least one additional requirement of the given past exercise session by at least a predetermined under-performance amount,

adjust the patient-specific cardiac rehabilitation plan based on the one or more recorded status identifiers,

wherein adjusting the patient-specific cardiac rehabilitation plan based on the one or more recorded status identifiers comprises performing at least a first adjustment based on an over-performance status identifier recorded for a past exercise session or performing at least a second adjustment based on an under-performance status identifier recorded for a past exercise session, and

transmit the adjusted patient-specific cardiac rehabilitation plan to the externally worn device;

wherein the externally worn device is further configured to

administer the patient-specific cardiac rehabilitation plan and the adjusted patient-specific cardiac rehabilitation plan;

monitor at least the patient's heart rate during a given exercise session;

determine whether the patient's monitored heart rate is below a minimum heart rate, above a maximum heart rate, or between the minimum heart rate and the maximum heart rate; and

provide an alert based on the determination of whether the patient's heart rate is below the minimum heart rate, above the maximum heart rate, or between the minimum heart rate and the maximum heart rate.

2. The system of claim 1 , wherein the one or more inputs from the healthcare provider, via a healthcare provider user interface, comprise a selection of a cardiac rehabilitation plan difficulty.

3. The system of claim 1 , wherein the one or more inputs from the healthcare provider, via a healthcare provider user interface, comprise a selection of at least one of a frequency, a starting duration, an ending duration, or a target pace for an exercise session.

4. The system of claim 1 , wherein the one or more inputs from the healthcare provider, via a healthcare provider user interface, comprise one or more thresholds for the patient-specific cardiac rehabilitation plan.

The system of claim 4 , wherein the one or more thresholds comprises at least one of a shortness of breath prior to a given exercise session, chest pain prior to a given exercise session, fatigue prior to the given exercise session, a maximum heart rate during the given exercise session, or an average pace for the given exercise session.

The system of claim 4 , wherein the at least one server processor is configured to develop the patient-specific cardiac rehabilitation plan based on the one or more inputs from the healthcare provider by selecting a difficulty ranking for each exercise session of the patient-specific cardiac rehabilitation plan based on the one or more thresholds.

7. The system of claim 4 , wherein the at least one server processor is further configured to

determine an average heart rate for the patient for a past exercise session; and

adjust the patient-specific cardiac rehabilitation plan further based on the average heart rate for the patient for the past exercise session.

8. The system of claim 7 , wherein the at least one server processor is configured to adjust the patient-specific cardiac rehabilitation plan further based on the patient data transgressing the one or more thresholds by at least one of stopping or rescheduling the given exercise session.

9. The system of claim 1 , wherein the at least one server processor is configured to develop the patient-specific cardiac rehabilitation plan using one or more plan templates.

10. The system of claim 1 , wherein the at least one server processor is configured to develop the patient-specific cardiac rehabilitation plan by selecting a pre-set plan based on the one or more inputs from the healthcare provider.

11. The system of claim 10 , wherein the one or more inputs from the healthcare provider comprise a level of heart failure of the patient.

12. The system of claim 1 , wherein the patient data comprises ECG information for the patient generated from the monitored cardiac activity of the patient.

13. The system of claim 1 , wherein the externally worn device further comprises a motion sensor configured to monitor movement of the patient.

14. The system of claim 1 , wherein the externally worn device further comprises an RF-based tissue fluid monitor configured to monitor tissue fluid levels of the patient.

15. The system of claim 1 , wherein the externally worn device further comprises one or more non-ECG physiological sensors configured to monitor non-ECG physiological data for the patient;

wherein the patient data comprises non-ECG physiological information generated from the monitored non-ECG physiological data for the patient; and

wherein the non-ECG physiological information for the patient comprises at least one of patient motion information, respiration information, lung fluid information, pulse information, or blood oxygenation information.

16. The system of claim 1 , wherein the one or more physical activities for the patient to perform during the given exercise session comprise walking at a predetermined intensity level.

17. The system of claim 1 , wherein each given exercise session further comprises at least one of a warm-up period or a cool-down period.

18. The system of claim 1 , wherein the over-performance status identifier is recorded in response to the patient exceeding at least one of a required distance traveled or a required pace of movement for the given past exercise session by at least the predetermined over-performance amount.

19. The system of claim 1 , wherein the under-performance status identifier is recorded in response to the patient satisfying a required duration for the given past exercise session and not meeting at least a required pace of movement for the given past exercise session by at least the predetermined under-performance amount.
