Abstract

Systems and methods for managing heart failure are described. The system receives physiological information including a first HS signal corresponding to paced ventricular contractions and a second HS signal corresponding to intrinsic ventricular contractions. The system detects worsening heart failure (WHF) using the received physiological information. A signal analyzer circuit can generate a paced HS metric from the first HS signal and a sensed HS metric from the second HS signal, and determine a concordance indicator between the paced and the sensed HS metrics. In response to the detected WHF, the system can use the concordance indicator to generate a therapy adjustment indicator for adjusting electrostimulation therapy, or a worsening cardiac contractility indicator indicating the detected WHF is attributed to degrading myocardial contractility.

Claims (49)

1. A system, comprising:

a receiver circuit configured to receive a first heart sound (HS) signal during cardiac pacing, and a second HS signal during intrinsic cardiac activities without cardiac pacing;

a signal analyzer circuit configured to:

generate a paced HS metric using the first HS signal, the paced HS metric indicating cardiac contractility during the cardiac pacing; and

generate a sensed HS metric using the second HS signal, the sensed HS metric indicating cardiac contractility during the intrinsic cardiac activities; and

a therapy control circuit configured to generate a control signal to initiate or adjust a heart failure therapy based at least in part on a concordance metric between the paced HS metric and the sensed HS metric, including initiating or adjusting a first heart failure therapy when the concordance metric satisfies a first condition, and initiating or adjusting a second heart failure therapy different from the first heart failure therapy when the concordance metric satisfies a second condition different from the first condition.

2. The system of claim 1 , wherein the paced HS metric includes a paced first sound (S1) intensity determined using the first HS signal, and wherein the sensed HS metric includes a sensed S1 intensity determined using the second HS signal.

3. The system of claim 1 , wherein the paced HS metric includes a paced cardiac timing interval (CTI) determined using at least the first HS signal, and wherein the sensed HS metric includes a sensed CTI determined using at least the second HS signal.

4. The system of claim 1 , wherein the first heart failure therapy includes an electrostimulation therapy, and the second heart failure therapy includes a drug therapy.

5. The system of claim 1 , wherein the signal analyzer circuit is configured to:

determine a cardiac contractility status including a preserved cardiac contractility when the concordance metric satisfies the first condition, and a degrading cardiac contractility when the concordance metric satisfies the second condition; and

providing the determined cardiac contractility status to a user on a user interface.

The system of claim 1 , wherein the signal analyzer circuit is configured to determine the concordance metric using a first trend of the paced HS metric over time relative to a second trend of the sensed HS metric over time, wherein the therapy control circuit is configured to generate a control signal to:

initiate or adjust the first heart failure therapy when (i) the first trend indicates values of the paced HS metric decaying with time, and (ii) the second trend indicates values of the sensed HS metric substantially maintaining at a level; and

initiate or adjust the second heart failure therapy when (i) the first trend indicates values of the paced HS metric decaying with time, and (ii) the second trend indicates values of the sensed HS metric decaying with time.

The system of claim 1 , wherein the signal analyzer circuit is configured to determine the concordance metric using a correlation between a first trend of the paced HS metric over time and a second trend of the sensed HS metric over time, wherein the therapy control circuit is configured to generate a control signal to:

initiate or adjust the first heart failure therapy when the correlation falls below a correlation threshold; and

initiate or adjust the second heart failure therapy when the correlation exceeds the correlation threshold.

The system of claim 1 , wherein the signal analyzer circuit is configured to determine the concordance metric using a ratio between a change in the paced HS metric over time and a change in the sensed HS metric over time, wherein the therapy control circuit is configured to generate a control signal to:

initiate or adjust the first heart failure therapy when the ratio falls below a ratio threshold; and

initiate or adjust the second heart failure therapy when the ratio exceeds the ratio threshold.

The system of claim 1 , wherein to generate the control signal to initiate or adjust the heart failure therapy includes to generate a control signal to switch from one stimulation mode to another stimulation mode each selected from the group consisting of: a biventricular pacing mode; a left ventricle-only pacing mode; a single-site pacing mode; and a multisite pacing mode.

The system of claim 1 , wherein to generate the control signal to initiate or adjust the heart failure therapy includes to generate a control signal to adjust a stimulation strength parameter including at least one of: a pulse width; a pulse amplitude; a frequency; a duty cycle; or a stimulation duration.

11. The system of claim 1 , wherein to generate the control signal to initiate or adjust the heart failure therapy includes to generate a control signal to change a stimulation site or to select a different stimulation electrode.

12. The system of claim 1 , wherein to generate the control signal to initiate or adjust the heart failure therapy includes to generate a control signal to adjust a stimulation timing parameter.

13. A method for controlling therapy delivered via a medical system, the method comprising:

receiving a first heart sound (HS) signal during cardiac pacing, and a second HS signal during intrinsic cardiac activities without cardiac pacing;

generating a paced HS metric using the first HS signal, the paced HS metric indicating cardiac contractility during the cardiac pacing;

generating a sensed HS metric using the second HS signal, the sensed HS metric indicating cardiac contractility during the intrinsic cardiac activities; and

generating a control signal to initiate or adjust a heart failure therapy based at least in part on a concordance between the paced HS metric and the sensed HS metric using a therapy control circuit, including initiating or adjusting a first heart failure therapy when the concordance metric satisfies a first condition, and initiating or adjusting a second heart failure therapy different from the first heart failure therapy when the concordance metric satisfies a second condition different from the first condition.

14. The method of claim 13 , wherein the first heart failure therapy includes an electrostimulation therapy, and the second heart failure therapy includes a drug therapy.

15. The method of claim 13 , comprising determining the concordance metric using a first trend of the paced HS metric over time relative to a second trend of the sensed HS metric over time,

wherein generating the control signal to initiate or adjust the heart failure therapy includes:

generating the control signal to initiate or adjust the first heart failure therapy when (i) the first trend indicates values of the paced HS metric decaying with time, and (ii) the second trend indicates values of the sensed HS metric substantially maintaining at a level; and

generating the control signal to initiate or adjust the second heart failure therapy when (i) the first trend indicates values of the paced HS metric decaying with time, and (ii) the second trend indicates values of the sensed HS metric decaying with time.

16. The method of claim 13 , comprising determining the concordance metric using a correlation between a first trend of the paced HS metric over time and a second trend of the sensed HS metric over time,

wherein generating the control signal to initiate or adjust the heart failure therapy includes:

generating the control signal to initiate or adjust the first heart failure therapy when the correlation falls below a correlation threshold; and

generating the control signal to initiate or adjust the second heart failure therapy when the correlation exceeds the correlation threshold.

17. The method of claim 13 , comprising determining the concordance metric using a ratio between a change in the paced HS metric over time and a change in the sensed HS metric over time,

wherein generating the control signal to initiate or adjust the heart failure therapy includes:

generating the control signal to initiate or adjust the first heart failure therapy when the ratio falls below a ratio threshold; and

generating the control signal to initiate or adjust the second heart failure therapy when the ratio exceeds the ratio threshold.

18. The method of claim 13 , wherein generating the control signal to initiate or adjust the heart failure therapy includes at least one of generating a control signal to:

switch from one stimulation mode to another stimulation mode;

adjust a stimulation strength parameter;

change a stimulation site;

select a different stimulation electrode; or

adjust a stimulation timing parameter.
