Abstract

An exemplary method of determining a surgical spinal correction for a subject using analysis of motion capture images of the subject, which uses the steps of obtaining pre-operative three-dimensional images of a spinal region, obtaining a pre-operative time sequenced set of images of the subject during a movement progression of said subject, calculating in a plurality of the motion capture images, alignment parameters relating to upper and lower body regions of the subject, and determining if any of the calculated alignment parameters are outside their predetermined acceptable ranges in one or more of the images, iteratively adjusting anatomical elements in three-dimensional images until all of the calculated alignment parameters are within their predetermined acceptable ranges; and adjusting spinal anatomy in the three-dimensional images according to the degree of adjustment of spinal parameters in the motion capture images to determine a surgical spinal correction.

Claims (44)

1 . A system for determining a surgical spinal correction for a subject, comprising:

at least one processor; and

memory comprising instructions that when executed by the at least one processor, cause the at least one processor to:

generate suggested spinal surgical corrections based on i) a plurality of pre-operative three-dimensional images of a spinal region of the subject, and ii) a plurality of pre-operative time-sequenced images comprising spinal anatomical elements and at least one of pelvic elements or lower limb elements of the subject during a movement progression of the subject;

measure, for each suggested spinal surgical correction, at least one pelvic parameter of the subject that results from simulating that suggested spinal surgical correction;

determine, for each suggested spinal surgical correction having the at least one pelvic parameter within an acceptable range throughout a simulated movement progression of the subject, whether at least one lower limb parameter of the subject is within an acceptable range throughout the simulated movement progression of the subject;

determine, for each suggested spinal surgical correction having the at least one pelvic parameter and the at least one lower limb parameter within their respective acceptable range, whether at least one spinal parameter of the subject is within an acceptable range throughout the simulated movement progression of the subject;

select one of the suggested spinal surgical corrections that result in the at least one spinal parameter, the at least one lower limb parameter, and the at least one pelvic parameter being within their respective acceptable range; and

output a final spinal surgical correction based on the selected one of the suggested spinal surgical corrections, the final spinal surgical correction being included in a surgical plan for performing spinal surgery on the subject.

The system of claim 1 , wherein the movement progression of the subject is either of walking or running, and the respective acceptable ranges of the at least one spinal parameter, the at least one lower limb parameter, and the at least one pelvic parameter are defined according to phases in a gait cycle while walking or running.

3 . The system of claim 1 , wherein the movement progression of the subject is spinal extension, flexion or rotation.

4 . The system of claim 1 , further comprising determining a diagnosis of the subject based on the plurality of pre-operative three-dimensional images and the plurality of pre-operative time-sequenced images.

5 . The system of claim 1 , wherein the respective acceptable ranges of the at least one spinal parameter, the at least one lower limb parameter, and the at least one pelvic parameter are defined according to a body type of the subject.

6 . The system of claim 1 , wherein the plurality of pre-operative time-sequenced images is obtained using a motion capture time sequence camera that images markers on the subject indicating the spinal anatomical elements.

7 . The system of claim 1 , wherein the pre-operative three-dimensional images are any one of CT images, X ray images, or MRI images.

8 . The system of claim 1 , wherein the at least one spinal parameter comprises at least one of cervical, thoracic, lumbar or sacral parameters.

The system of claim 1 , wherein the at least one processor executes the instructions to output the final spinal surgical correction based on one or more relationship models that relate one or more of the at least one spinal parameter, the at least one pelvic parameter, or the at least one lower limb parameter to positions of the spinal anatomical elements in the plurality of pre-operative three-dimensional images.

The system of claim 1 , wherein, when the at least one processor determines that none of the suggested spinal surgical corrections result in the at least one spinal parameter being within its acceptable range, the at least one processor executes the instructions to perform an optimization process on one or more of the pre-operative time-sequenced images to bring the at least one spinal parameter within its acceptable range throughout the simulated movement progression of the subject.

11 . The system of claim 10 , wherein the optimization process comprises an Sagittal Vertical Axis (SVA) optimization process.

12 . The system of claim 11 , wherein the SVA optimization process comprises adjusting one or more angles associated with Lumbar Lordosis and Thoracic Kyphosis.

The system of claim 10 , wherein, when the at least one spinal parameter is brought within its acceptable range by the optimization process, the at least one processor executes the instructions to determine whether the at least one lower limb parameter and the at least one pelvic parameter are still within their respective acceptable range.

The system of claim 1 , wherein the respective acceptable ranges of the at least one spinal parameter, the at least one lower limb parameter, and the at least one pelvic parameter are defined for each pre-operative time-sequenced image according to the moment in time of that image during the movement progression of the subject.

15 . A system for determining a surgical spinal correction for a subject, comprising:

at least one camera that generates a plurality of pre-operative time-sequenced images comprising spinal anatomical elements and at least one of pelvic elements or lower limb elements of the subject during a movement progression of the subject;

at least one imaging device that generates a plurality of pre-operative three-dimensional images of a spinal region of the subject;

at least one processor; and

memory comprising instructions that when executed by the at least one processor, cause the at least one processor to:

generate suggested spinal surgical corrections based on i) the plurality of pre-operative three-dimensional images and ii) the plurality of pre-operative time-sequenced images;

measure, for each suggested spinal surgical correction, at least one pelvic parameter of the subject that results from simulating that suggested spinal surgical correction;

determine, for each suggested spinal surgical correction having the at least one pelvic parameter within an acceptable range throughout a simulated movement progression of the subject, whether at least one lower limb parameter of the subject is within an acceptable range throughout the simulated movement progression of the subject;

determine, for each suggested spinal surgical correction having the at least one pelvic parameter and the at least one lower limb parameter within their respective acceptable range, whether at least one spinal parameter of the subject is within an acceptable range throughout the simulated movement progression of the subject;

select one of the suggested spinal surgical corrections that result in the at least one spinal parameter, the at least one lower limb parameter, and the at least one pelvic parameter being within their respective acceptable range; and

output a final spinal surgical correction based on the selected one of the suggested spinal surgical corrections, the final spinal surgical correction being included in a surgical plan for performing spinal surgery on the subject.

16 . The system of claim 15 , wherein the at least one imaging device comprises an X-ray scanner, a CT scanner, or an MRI scanner.

The system of claim 15 , wherein the at least one processor executes the instructions to output the final spinal surgical correction based on one or more relationship models that relate one or more of the at least one spinal parameter, the at least one pelvic parameter, or the at least one lower limb parameter to positions of the spinal anatomical elements in the plurality of pre-operative three-dimensional images.

The system of claim 15 , wherein, when the at least one processor determines that none of the suggested spinal surgical corrections result in the at least one spinal parameter being within its acceptable range, the at least one processor executes the instructions to perform an optimization process on one or more of the pre-operative time-sequenced images to bring the at least one spinal parameter within its acceptable range throughout the simulated movement progression of the subject.

19 . The system of claim 18 , wherein the optimization process comprises adjusting one or more angles associated with Lumbar Lordosis and Thoracic Kyphosis.

20 . A method for determining a surgical spinal correction for a subject, comprising:

generating suggested spinal surgical corrections based on i) a plurality of pre-operative three-dimensional images of a spinal region of the subject, and ii) a plurality of pre-operative time-sequenced images comprising spinal anatomical elements and at least one of pelvic elements or lower limb elements of the subject during a movement progression of the subject;

measuring, for each suggested spinal surgical correction, at least one pelvic parameter of the subject that results from simulating that suggested spinal surgical correction;

determining, for each suggested spinal surgical correction having the at least one pelvic parameter within an acceptable range throughout a simulated movement progression of the subject, whether at least one lower limb parameter of the subject is within an acceptable range throughout the simulated movement progression of the subject;

determining, for each suggested spinal surgical correction having the at least one pelvic parameter and the at least one lower limb parameter within their respective acceptable range, whether at least one spinal parameter of the subject is within an acceptable range throughout the simulated movement progression of the subject;

selecting one of the suggested spinal surgical corrections that result in the at least one spinal parameter, the at least one lower limb parameter, and the at least one pelvic parameter being within their respective acceptable range; and

outputting a final spinal surgical correction based on the selected one of the suggested spinal surgical corrections, the final spinal surgical correction being included in a surgical plan for performing spinal surgery on the subject.
