Abstract

A multi-portal method for treating a subject's spine includes distracting adjacent vertebrae using a distraction instrument positioned at a first entrance along the subject to enlarge an intervertebral space between the adjacent vertebrae. An interbody fusion implant can be delivered into the enlarged intervertebral space. The interbody fusion implant can be positioned directly between vertebral bodies of the adjacent vertebrae while endoscopically viewing the interbody fusion implant using an endoscopic instrument. The patient's spine can be visualized using endoscopic techniques to view, for example, the spine, tissue, instruments and implants before, during, and after implantation, or the like. The visualization can help a physician throughout the surgical procedure to improve patient outcome.

Claims (46)

1 . A surgical kit comprising:

a first split cannula configured to be positioned in a first port formed in a subject, the first split cannula including:

a first U-shaped open channel,

a first port flange,

a tapered distal end configured to penetrate tissue to position the first port flange proximate to the subject's skin, and

a plurality of spaced apart motion inhibitors positioned along opposite edges of the first U-shaped open channel, wherein the motion inhibitors are configured to atraumatically contact internal tissue of the subject so as to inhibit withdrawal of the split cannula from the subject; and

a second split cannula configured to be positioned in a second port formed in the subject and spaced apart from the first port, the second split cannula having a second open channel and a second port flange; and

a container holding the first and second split cannulas.

2 . The surgical kit of claim 1 , further comprising one or more instruments configured to be positioned within the first U-shaped open channel while manipulating tissue proximate to a distal end of the first U-shaped open channel.

3 . The surgical kit of claim 1 , wherein the first port flange has a length that is longer than a depth of the first U-shaped open channel of the first split cannula.

4 . The surgical kit of claim 1 , further comprising a surgical instrument in the container, wherein the first U-shaped open channel defines an instrument-delivery passageway larger than a transverse profile of the surgical instrument.

5 . The surgical kit of claim 1 , wherein the first split cannula has a longitudinal axis, wherein the first port flange extends radially outward from the longitudinal axis.

6 . The surgical kit of claim 1 , wherein the first port flange extends along an imaginary plane that is generally orthogonal to a longitudinal axis of the first split cannula.

7 . The surgical kit of claim 1 , wherein at least one of the first split cannula or the second cannula has a plurality of motion inhibitors spaced apart from one another along an axial length of the least one of the first split cannula or the second cannula.

8 . The surgical kit of claim 7 , wherein one or more of the motion inhibitors are U-shaped or V-shaped indentations located along the edges.

9 . The surgical kit of claim 7 , wherein one or more of the motion inhibitors are grooves or protrusions extending circumferentially about a partially tubular body of the at least one of the first split cannula.

10 . The surgical kit of claim 1 , wherein the first and second split cannulas are geometrically congruent.

11 . The surgical kit of claim 1 , wherein at least one of the first cannula or the second cannula has a curved channel defining a central angle between 150 degrees and 210 degrees.

The surgical kit of claim 1 , wherein the first split cannula has a first curved body extending from a distal end to a first proximal end, wherein a width a channel of the curved body is configured to be generally aligned with a length of a linear incision when an instrument is positioned along a first passage of the first split cannula.

13 . The surgical kit of claim 1 , wherein a split thin-walled shaft of the first split cannula includes one or more motion inhibits configured to contact tissue to inhibit inadvertent removal of the first split cannula from a subject when an instrument is withdrawn from the first split cannula.

14 . The surgical kit of claim 1 , further comprising an implant configured fit in first open channel to be delivered via the first split cannula.

15 . The surgical kit of claim 1 , further comprising:

a hemostatic dispenser including in angled wand configured to extend into a proximal port end and out a distal end of the first split cannula.

16 . The surgical kit of claim 1 , further comprising at least one retractor configured to fit within the first U-shaped open channel.

17 . The surgical kit of claim 1 , further comprising:

an implant configured to be delivered via the first split cannula.

18 . A split cannula comprising:

a port flange; and

a U-shaped split shaft connected to the port flange and including:

a U-shaped channel,

a first edge and a second edge on opposite sides of the U-shaped channel,

a tapered distal end configured to penetrate tissue to position the port flange proximate to a subject's skin, wherein the U-shaped channel has a substantially uniform width from the port flange to the tapered distal end, and

a plurality of spaced apart motion inhibitors defining U-shaped or V-shaped notches located along the first and second edges, wherein the U-shaped or V-shaped notches are configured to atraumatically contact internal tissue of the subject facing the U-shaped channel and stretched between the first and second edges so as to inhibit withdrawal of the split cannula from the subject.

19 . The split cannula of claim 18 , wherein the port flange has an axial length longer than a maximum transverse dimension of the U-shaped split shaft.

20 . The split cannula of claim 18 , wherein the port flange has an axial length longer than:

a width between longitudinal edges of the U-shaped split shaft; and

a depth of a channel defined by the U-shaped split shaft.

21 . The split cannula of claim 18 , wherein the U-shaped split shaft has a longitudinal axis, wherein the port flange extends radially outward from the longitudinal axis.

22 . The split cannula of claim 18 , wherein the port flange lies along a plane generally perpendicular to a longitudinal axis of the U-shaped split shaft.

23 . The split cannula of claim 18 , wherein the motion inhibitors are evenly spaced apart from one another along an axial length of U-shaped split shaft.

24 . The split cannula of claim 18 , wherein each of the motion inhibitors has a depth greater than a wall thickness of the U-shaped split shaft.

25 . The split cannula of claim 24 , wherein the notches are U-shaped or V-shaped.

26 . The split cannula of claim 18 , wherein one or more of the motion inhibitors are grooves or protrusions extending circumferentially about a partially tubular of the split cannula.

27 . The split cannula of claim 18 , wherein the first edge is a longitudinally-extending edges each with one or more of the motion inhibitors having U-shaped or V-shapes.

28 . The split cannula of claim 18 , wherein the port flange extends along an imaginary plane that is generally orthogonal to a longitudinal axis of the split cannula.

29 . The split cannula of claim 18 , wherein the U-shaped split shaft has a concave sidewall defining a longitudinally-extending channel with a central angle between 150 degrees and 210 degrees.
