Abstract

An intravascular ablation device, including a flexible elongate body; an expandable element positioned on the elongate body; a radiofrequency or electroporation treatment segment located distally of the expandable element; a cryogenic coolant source in fluid communication with an interior of the expandable element; and a radiofrequency or electroporation energy source in communication with the radiofrequency or electroporation treatment segment.

Claims (37)

1. A medical system, comprising:

an elongate catheter body including a proximal portion coupled to a handle and a distal portion;

an expandable cryogenic treatment region coupled to the distal portion of the catheter body;

an electroporation treatment region defining an outer surface of and a distal tip of the medical device, an entirety of the electroporation treatment region coupled to the distal portion of the catheter body at a location that is distal to the expandable cryogenic treatment region, the electroporation treatment region having a flexibility; and

a deflectable segment located between the expandable cryogenic treatment region and the electroporation treatment region, the deflectable segment having a flexibility that is greater than the flexibility of the electroporation treatment region.

2. The medical system of claim 1 , wherein the expandable cryogenic treatment region includes a balloon.

3. The medical system of claim 1 , wherein the expandable cryogenic treatment region includes a first expandable element and a second expandable element within the first expandable element.

4. The medical system of claim 3 , further comprising:

a first fluid flow path in fluid communication with the expandable cryogenic treatment region; and

a cryogenic fluid source in fluid communication with the first fluid flow path.

5. The medical system of claim 4 , further comprising:

a second fluid flow path in fluid communication with the expandable cryogenic treatment region; and

an exhaust system in fluid communication with the second fluid flow path.

6. The medical system of claim 5 , wherein the expandable cryogenic treatment region further includes an interstitial region between the first expandable element and the second expandable element.

7. The medical system of claim 6 , further comprising a third fluid flow path in fluid communication with the interstitial region.

8. The medical system of claim 7 , wherein the third fluid flow path is fluidically isolated from the first and second fluid flow paths.

9. The medical system of claim 1 , wherein the electroporation treatment region includes an electrode.

10. The medical system of claim 9 , further comprising a power source in communication with the electrode and being configured to deliver electrical pulses.

11. The medical system of claim 1 , wherein the electroporation treatment region includes a substantially linear segment.

12. The medical system of claim 1 , wherein the deflectable segment includes a bellows.

13. The medical system of claim 1 , wherein the deflectable segment is located adjacent to a segment of the catheter body, the deflectable segment having a first thickness and the segment of catheter body having a second thickness that is greater than the first thickness.

14. The medical system of claim 1 , wherein the deflectable segment is located adjacent to a segment of the catheter body, the deflectable segment being composed of a first material and the segment of catheter body being composed of a second material that is less flexible than the first material.

15. The medical system of claim 1 , further comprising a sensor coupled to at least one of the expandable cryogenic treatment region and the electroporation treatment region.

16. The medical system of claim 1 , wherein the electroporation treatment region is deflectable independently of the expandable cryogenic treatment region.

17. An intravascular ablation system, comprising:

a flexible elongate body including a proximal portion coupled to a handle and a distal portion;

an expandable cryogenic treatment element coupled to the distal portion of the elongate body, the expandable cryogenic treatment element including a first expandable element and a second expandable element located within the first expandable element, the second expandable element defining a chamber;

an interstitial region between the first expandable element and the second expandable element;

a cryogenic coolant source;

a fluid delivery lumen in fluid communication with the chamber;

a fluid exhaust lumen in fluid communication with the chamber;

an interstitial lumen in fluid communication with the interstitial region, the interstitial lumen being fluidly isolated from the fluid delivery lumen and the fluid exhaust lumen;

a substantially linear, elongated electroporation segment coupled to the distal portion of the elongate body and extending distally beyond the expandable cryogenic treatment element when the expandable cryogenic treatment element is fully expanded; and

a power source in communication with the electroporation segment, the power source being configured to transmit electrical pulses to the electroporation segment.

18. The system of claim 17 , wherein the elongate body includes a deflectable segment between the expandable element and the electroporation segment.

19. The system of claim 18 , wherein the deflectable segment includes a bellows.

20. The system of claim 18 , wherein the electroporation segment has a first flexibility and the deflectable segment has a second flexibility that is greater than the first flexibility.
