Abstract

Disclosed herein are systems, devices, and methods for the removal of objects from the body. The device may be a urethral catheter configured to aspirate kidney stones from the urinary tract through one or more aspiration ports at the distal face or along a lateral side of the catheter. The catheter may include one or more irrigation ports at the distal face or along the lateral side of the catheter for dislodging kidney stones. The device may be steerable. The spatial arrangement of the one or more irrigation ports with respect to the one or more aspiration ports may vary. The device may include an irrigation tube and/or a shield member configured to spatially confine the kidney stones adjacent the catheter. Various temporal patterns of aspiration and irrigation are disclosed for optimizing removal of kidney stones.

Claims (31)

1. A method of removing kidney stones from a kidney of a patient, the method comprising:

inserting a steerable distal portion of a catheter system through the urethra, bladder, and ureter and into the kidney, the distal portion comprising a vacuum lumen and an irrigation lumen, wherein a distal face of the distal portion comprises (i) an aspiration port and (ii) a plurality of irrigation ports in fluid communication with the irrigation lumen, wherein the aspiration port has an inner diameter larger than an inner diameter of any one of the plurality of irrigation ports;

wherein while the distal portion of the catheter system is being inserted into the kidney, the kidney is irrigated by applying irrigation fluid at a first flow rate measured in ml per minute in a first flow rate range of the ml per minute through the irrigation lumen and out from at least one of the irrigation ports; followed by,

inserting the distal portion into a first calyx of the kidney and applying irrigation fluid at a second flow rate measured in ml per minute in a second flow rate range of the ml per minute through the irrigation lumen and out from the irrigation ports into the first calyx of the kidney, wherein the first flow rate range is different than the second flow rate range and the first flow rate is different than the second flow rate;

followed by,

removing kidney stones from the first calyx through the vacuum lumen; and followed by,

removing the distal portion from the first calyx, wherein irrigation fluid is applied continuously at the first flow rate or a different flow rate in the first flow rate range, the different flow rate being different than the second flow rate, through the irrigation lumen while the distal portion of the catheter system is removed from the first calyx.

2. The method of claim 1 , further comprising:

removing kidney stones from the first calyx by applying suction through the vacuum lumen.

3. The method of claim 1 , further comprising:

after inserting the distal portion into the kidney, continuously applying irrigation fluid through the irrigation lumen until irrigation fluid returns through the vacuum lumen to outside of the patient.

4. The method of claim 1 , further comprising:

applying irrigation fluid at the first flow rate or another flow rate in the first flow rate range through the irrigation lumen and out from at least one of the irrigation ports and inserting the distal portion into a second calyx of the kidney.

5. The method of claim 4 , further comprising:

applying irrigation fluid at the second flow rate or a different flow rate in the second flow rate range through the irrigation lumen and out from at least one of the irrigation ports.

6. The method of claim 5 , further comprising:

removing kidney stones from the second calyx through the vacuum lumen.

7. The method of claim 5 , further comprising:

removing kidney stones from the second calyx by applying suction through the vacuum lumen.

8. The method of claim 4 , wherein the first calyx or the second calyx includes a calyx within a lower pole of the kidney.

9. The method of claim 1 , wherein the catheter system is configured to allow kidney stone transport through the vacuum lumen when the distal portion is bent to form a curve in the range of about 100 degrees to about 200 degrees.

10. The method of claim 1 , wherein the irrigation lumen is fixed with respect to the vacuum lumen.

11. The method of claim 1 , wherein the distal portion is configured to create an irrigation stream that extends radially outward away from a longitudinal axis of the distal portion.

12. The method of claim 1 , wherein the distal portion is configured to create an irrigation stream that diverges away from a longitudinal axis of the distal portion.

13. The method of claim 1 , further comprising a nozzle incorporated into the distal portion and the nozzle forms the distal face of the distal portion.

14. The method of claim 13 , wherein the nozzle is configured to create an irrigation stream that extends radially outward away from a longitudinal axis of the distal portion.

15. The method of claim 13 , wherein the nozzle is configured to create an irrigation stream that diverges away from a longitudinal axis of the distal portion.

16. The method of claim 1 , wherein the vacuum lumen has an inner diameter of about 2.0 mm.

17. The method of claim 1 , wherein the vacuum lumen has an inner diameter of about 2.5 mm.

The method of claim 1 , wherein the catheter system further comprises a direct visualization device configured to be movable within the vacuum lumen and inserting the distal portion into the kidney, the first calyx, or a second calyx is done while visualizing the kidney, first calyx, or second calyx using the direct visualization device.

19. The method of claim 18 , further comprising removing the direct visualization device from within the vacuum lumen prior to removing kidney stones from the first calyx and replacing the direct visualization device into the vacuum lumen prior to inserting the distal portion into the second calyx.
