Abstract

According to some embodiments, a method of treating a subject having diabetes or symptoms associated with diabetes is provided. The method includes delivering a neuromodulation catheter within a vessel (e.g., hepatic artery) having surrounding nerves that innervate the liver (e.g., sympathetic nerves of the hepatic plexus). The method may also include modulating (e.g., disrupting, ablating, stimulating) the nerves by mechanical compression, energy delivery, or fluid delivery.

Claims (33)

1. A method of facilitating thermally-induced hepatic neuromodulation in a subject, comprising:

inserting a neuromodulation catheter into a vasculature of a subject;

advancing a distal end of the neuromodulation catheter to a location sufficient to target one or more sympathetic nerves that innervate a liver of the subject;

delivering energy to the one or more sympathetic nerves to denervate the one or more sympathetic nerves, wherein the denervation facilitates a reduction in a blood glucose level in the subject.

2. The method of claim 1 , wherein the neuromodulation catheter is configured to maintain contact with an inner wall of a common hepatic artery while the energy is being delivered.

3. The method of claim 2 , wherein the contact is maintained at a contact pressure of between 0.1 g/mm 2 and 10 g/mm 2 .

4. The method of claim 1 , wherein the neuromodulation catheter comprises at least one radiofrequency electrode.

5. The method of claim 1 , wherein the denervation is further effective to reduce at least one of a hepatic norepinephrine or a lipid level in the subject.

6. The method of claim 1 , wherein the neuromodulation catheter comprises a balloon or steerable distal tip.

The method of claim 1 , wherein delivering the energy comprises delivering radiofrequency energy to the one or more sympathetic nerves at a frequency in the range of 50 kHz to 5 MHz, wherein the one or more sympathetic nerves are located in the hepatic or celiac plexus, wherein the radiofrequency energy is delivered to heat the one or more sympathetic nerves to a temperature of up to 90 degrees Celsius and sufficient to cause ablation of the one or more sympathetic nerves.

8. The method of claim 1 , wherein delivering the energy comprises delivering ultrasound energy at a frequency in the range of 20 kHz to 20 MHz.

9. The method of claim 1 , further comprising providing cooling to a portion of a tissue that is not being targeted by the energy.

10. A method of facilitating thermally-induced hepatic neuromodulation in a subject, comprising:

inserting an energy delivery device within a hepatic artery of a subject;

advancing a distal end of the energy delivery device to a location sufficient to target one or more sympathetic nerves that innervate a liver of the subject;

delivering energy to the one or more sympathetic nerves to disrupt neural communication of the one or more sympathetic nerves, wherein the disruption facilitates a reduction in at least one of a blood glucose level or a lipid level in the subject; and

removing the energy delivery device from the subject.

11. The method of claim 10 , wherein the energy is delivered to at least one of a hepatic plexus or a celiac plexus.

12. The method of claim 10 , wherein the disruption of neural communication comprises partial or full ablation of the one or more sympathetic nerves.

13. The method of claim 10 , wherein delivering the energy comprises delivering radiofrequency or ultrasound energy.

14. The method of claim 10 , wherein delivering the energy comprises delivering pulsed energy.

15. The method of claim 10 , wherein delivering the energy comprises delivering energy at a frequency in the range of 20 kHz to 20 MHz.

16. The method of claim 10 , wherein delivering the energy comprises delivering energy to a hepatic plexus at a frequency in the range of 20 kHz to 20 MHz, wherein the energy is delivered to heat the one or more sympathetic nerves to a temperature of up to 90 degrees Celsius and sufficient to cause ablation of the one or more sympathetic nerves.

17. A method of facilitating thermally-induced hepatic neuromodulation, comprising:

providing a neuromodulation system comprising a neuromodulation catheter configured for:

accessing a branch of a hepatic artery of a subject; and

delivering energy to one or more nerves that innervate a liver of the subject,

wherein the delivered energy is sufficient to modulate ablate said one or more nerves to affect at least one of a blood glucose level or a lipid level in the subject.

18. The method of claim 17 , wherein the delivered energy comprises radiofrequency energy having a frequency in the range of 50 kHz to 5 MHz.

19. The method of claim 17 ,

wherein the delivered energy has a frequency in the range of 20 kHz to 20 MHz,

wherein the delivered energy heats the one or more nerves to a temperature of up to 90 degrees Celsius; and

wherein the one or more nerves are sympathetic nerves of the hepatic plexus.
