Hepatic artery nerve modulation methods
According to some embodiments, a method of treating a subject having diabetes or symptoms associated with diabetes is provided. The method includes delivering a neuromodulation catheter within a vessel (e.g., hepatic artery) having surrounding nerves that innervate the liver (e.g., sympathetic nerves of the hepatic plexus). The method may also include modulating (e.g., disrupting, ablating, stimulating) the nerves by mechanical compression, energy delivery, or fluid delivery.
1. A method of facilitating hepatic neuromodulation, comprising:
inserting an energy delivery device within vasculature of a subject;
intravascularly advancing a distal portion of the energy delivery device to a location within a portion of a hepatic artery; and
delivering energy toward one or more nerves within or surrounding a wall of the hepatic artery sufficient to ablate the one or more nerves in a manner that facilitates a reduction in at least one of a blood glucose level or a lipoprotein level in the subject; and
removing the energy delivery device from the subject.
2. The method of claim 1 ,
wherein the delivered energy is radiofrequency energy having a frequency in the range of 50 kHz to 5 MHz,
wherein the delivered energy heats the one or more nerves to a temperature of up to 90 degrees Celsius; and
wherein the one or more nerves are sympathetic nerves of the hepatic plexus.
3. The method of claim 1 ,
wherein the delivered energy is ultrasound energy having a frequency in the range of 20 kHz to 20 MHz,
wherein the delivered energy heats the one or more nerves to a temperature of up to 90 degrees Celsius; and
wherein the one or more nerves are sympathetic nerves of the hepatic plexus.
4. The method of claim 1 , further comprising confirming said ablation of the one or more nerves.
5. A method of facilitating hepatic neuromodulation in a subject, comprising:
inserting an energy delivery device into vasculature of a subject,
wherein a distal portion of the energy delivery device comprises at least one electrode;
advancing the distal portion of the energy delivery device to a location within a hepatic artery of the vasculature; and
intravascularly modulating one or more sympathetic nerves located within or surrounding a wall of the hepatic artery using the energy delivery device,
wherein the distal portion of the energy delivery device is configured to cause maintained contact of the at least one electrode with the wall of the hepatic artery during said modulating.
6. The method of claim 5 ,
wherein the delivered energy is radiofrequency energy having a frequency in the range of 50 kHz to 5 MHz,
wherein the hepatic artery is a common hepatic artery,
wherein the delivered energy heats the one or more nerves to a temperature of up to 90 degrees Celsius and sufficient to cause denervation of the one or more nerves, and
wherein the one or more nerves are sympathetic nerves of the hepatic plexus.
7. The method of claim 5 , wherein the modulation is effective to reduce at least one of: a blood glucose level, a hepatic norepinephrine or a lipid level in the subject.
8. The method of claim 5 , wherein the distal portion of the energy delivery device is steerable.
9. The method of claim 5 , further comprising delivering the energy delivery device over a guide wire.
10. The method of claim 5 , wherein advancing the distal portion of the energy delivery device to a location within the hepatic artery of the vasculature comprises advancing the distal portion of the energy delivery device through an outer sheath and beyond a distal end of the outer sheath.
11. The method of claim 5 ,
wherein the energy is delivered at a frequency in the range of 50 kHz to 5 MHz;
wherein the energy is delivered to heat the one or more sympathetic nerves to a temperature of up to 90 degrees Celsius and sufficient to cause ablation of the one or more sympathetic nerves.
12. The method of claim 5 , wherein the energy is delivered in a radial pattern to denervate the one or more sympathetic nerves at multiple locations.
13. The method of claim 10 , wherein the distal portion of the energy delivery device comprises shape memory material having a pre-formed shape configuration such that, when the distal portion is advanced out of the distal end of the outer sheath, the distal portion deflects to contact the wall of the hepatic artery, thereby facilitating the maintained contact.
14. A method of facilitating hepatic neuromodulation in a subject, comprising:
causing energy to be delivered by at least one energy delivery element of an energy delivery device positioned to deliver energy to one or more sympathetic nerves of a subject within or surrounding a wall of a hepatic artery,
wherein the energy is delivered at a frequency in the range of 3 kHz to 300 GHz, and
wherein the delivered energy is sufficient to disrupt neural communication of the one or more sympathetic nerves.
15. The method of claim 14 , wherein the hepatic artery is a common hepatic artery.
16. The method of claim 14 , wherein the hepatic artery is a proper hepatic artery.
17. The method of claim 14 , wherein the delivered energy is sufficient to permanently disable neural communication along the one or more sympathetic nerves.
18. The method of claim 14 , further comprising causing the energy delivery element to contact an inner wall of the hepatic artery while said energy is being delivered.
19. The method of claim 14 ,
wherein the energy delivery element comprises at least one radiofrequency electrode,
wherein the one or more sympathetic nerves are nerves of the hepatic plexus,
wherein the energy is delivered to the one or more sympathetic nerves at a frequency in the range of 100 kHz to 2.5 MHz, and
wherein the energy is delivered to heat the one or more sympathetic nerves in the hepatic plexus to a temperature of up to 90 degrees Celsius and sufficient to cause ablation of the one or more sympathetic nerves.
20. The method of claim 14 , wherein the energy delivered to the energy delivery element is delivered by a radiofrequency generator.
