Abstract

Methods for altering the natural history of degenerative disc disease and osteoarthritis of the spine are proposed. The methods focus on the prevention, or delayed onset or progression of, subchondral defects such as bone marrow edema or bone marrow lesion, and subchondral treatment to prevent the progression of osteoarthritis or degenerative disc disease in the spine and thereby treat pain.

Claims (20)

1. A method of treating a subchondral defect in a subchondral region of a vertebral body to alter the progression of osteoarthritis of the spine, the method comprising:

identifying a subchondral defect in a subchondral region of a vertebral body, the subchondral defect including a bone marrow lesion, the vertebral body including an articular surface adjacent the subchondral region;

creating, in the vertebral body that contains the subchondral defect in the subchondral region, a subchondral access path to an area in or adjacent to the bone marrow lesion for injecting an injectable bone filling material into said area for reinforcing bone located in the bone marrow lesion, wherein said creating preserves an existing condition of the articular surface and is conducted without further creating a void in or adjacent to the bone marrow lesion through compaction of bone tissue; and

injecting an injectable bone filling material into said area via the subchondral access path, wherein the injectable bone filling material is left in the area for hardening in the area so as to reinforce bone located in the bone marrow lesion.

2. The method of claim 1 , wherein the injectable bone filling material includes a calcium phosphate.

3. The method of claim 1 , wherein the injectable bone filling material envelopes the subchondral defect.

4. The method of claim 1 , wherein the subchondral access path is created with a needle.

5. The method of claim 1 , wherein the injectable material is hardenable in the subchondral defect in a manner that increases the mechanical strength of the bone in the bone marrow lesion.

6. The method of claim 1 , wherein the subchondral region occurs in a facet.

7. The method of claim 1 , wherein said creating is performed arthroscopically or percutaneously.

8. A method of treating a subchondral defect in a subchondral region of a vertebral body to alter the progression of osteoarthritis of the spine, the method comprising:

identifying a subchondral defect in a subchondral region of a vertebral body, the subchondral defect including a bone marrow edema, the vertebral body including an articular surface adjacent the subchondral region;

creating, in the vertebral body that contains the subchondral defect in the subchondral region, a subchondral access path to an area in or adjacent to the bone marrow edema for injecting an injectable bone filling material into said area for reinforcing bone located in the bone marrow edema, wherein said creating preserves an existing condition of the articular surface and is conducted without further creating a void in or adjacent to the bone marrow edema through compaction of bone tissue; and

injecting an injectable bone filling material into said area via the subchondral access path, wherein the injectable bone filling material is left in the area for hardening in the area so as to reinforce bone located in the bone marrow edema.

9. The method of claim 8 , wherein said creating is performed arthroscopically or percutaneously.

10. The method of claim 8 , wherein the injectable bone filling material envelopes the subchondral defect.

11. The method of claim 8 , wherein the injectable bone filling material includes a calcium phosphate.

12. The method of claim 8 , wherein the subchondral access path is created with a needle.

13. The method of claim 8 , wherein the subchondral region occurs in a facet.

14. The method of claim 8 , wherein the injectable material is hardenable in the subchondral defect in a manner that increases the mechanical strength of the bone in the bone marrow edema.
