Abstract

Interspinous process implants are disclosed. Also disclosed are systems and kits including such implants, methods of inserting such implants, and methods of alleviating pain or discomfort associated with the spinal column.

Claims (44)

1. A method of implanting a device between adjacent spinous processes comprising:

creating an incision in a patient to be treated;

dilating any interspinous ligaments in a position in which the device is to be placed in the patient;

sizing a space between adjacent spinous processes; and

inserting the device between the adjacent spinous processes in a first position, the device comprising:

an elongate body comprising a first end portion, a second end portion, and a central support portion disposed between the first end portion and the second end portion;

wherein the first end portion comprises a wing member movable from the first position to a second position, the wing member including an outer surface and an inner surface that faces a central longitudinal axis of the elongate body;

wherein in the first position, the wing member serves as a ramp to engage and separate the adjacent spinous processes; and

moving the device to the second position to expand the wing member such that a portion of the inner surface of the wing member is capable of contacting a lateral surface of at least one of the spinous processes to limit movement of the device with respect thereto,

inserting a guide wire laterally through the skin and into an interspinous space;

creating a working portal concentric to the guidewire by inserting a series of sequentially larger diameter tubes to dilate the tissue surrounding the guidewire; and

once a dilating tube having a sufficiently large inner diameter to accommodate the device is positioned about the guidewire, withdrawing the smaller diameter tubes, leaving the guidewire and the outer tube.

2. The method of claim 1 further comprising securing the device to one or more of the spinous processes.

3. The method of claim 1 further comprising distracting the spinous processes apart from one another before inserting the device.

4. The method of claim 1 , wherein the device is placed between the spinous processes anterior to the supraspinous ligament, avoiding the nerves in the spinal canal.

5. The method of claim 1 , wherein a unilateral approach is used to install the device without removal of the supraspinous ligament.

6. The method of claim 1 , wherein the spinal processes are accessed from one lateral side of the patient.

7. The method of claim 1 , wherein once the device is installed, removing the guidewire leaving the device in the interspinous space.

8. The method of claim 1 further comprising, before inserting the device, inserting one or more trials to appropriately size the device.

The method of claim 1 further comprising rotating the device to advance the device between the spinous processes and, the ramp distracts the spinous processes further apart from one another, until the device is rotated and advanced laterally into an implanted position with the distraction portion positioned on a contralateral side of the spinous processes and the spinous processes are fitted into the central support portion of the device.

10. A method of implanting a device between adjacent spinous processes comprising:

creating an incision in a patient to be treated;

dilating any interspinous ligaments in a position in which the device is to be placed in the patient;

sizing a space between adjacent spinous processes; and

inserting the device between the adjacent spinous processes in a first position, wherein the device is inserted by rotating the device to advance and distract the spinous processes further apart from one another, the device comprising:

a body comprising a support portion extending laterally along a lateral axis between first and second end portions;

wherein at least a portion of the first end portion is moveable from the first position to a second position, the first end portion includes an outer surface and an inner surface that faces a central longitudinal axis of the body;

wherein when in the first position, the first end portion comprises a ramp on at least a portion of the outer surface configured and dimensioned to engage and separate the adjacent spinous processes when inserted, and a sidewall of the first end portion faces the lateral axis of the body; and

moving the device to the second position such that the portion of the first end portion that is moveable extends beyond the support portion in a direction perpendicular to the lateral axis to a greater extent than in the first position, and the sidewall of the first end portion faces the second end portion, wherein the sidewall is configured and dimensioned to engage a lateral surface of at least one of the adjacent spinous processes to limit lateral movement with respect thereto.

11. The method of claim 10 further comprising distracting the spinous processes apart from one another before inserting the device.

12. The method of claim 10 , wherein the device is placed between the spinous processes anterior to the supraspinous ligament, avoiding the nerves in the spinal canal.

13. The method of claim 10 , wherein a unilateral approach is used to install the device without removal of the supraspinous ligament.

14. The method of claim 10 , wherein the spinal processes are accessed from one lateral side of the patient.

15. The method of claim 1 further comprising, before inserting the device, inserting one or more trials to appropriately size the device.

16. A method of implanting a device between adjacent spinous processes comprising:

creating an incision in a patient to be treated;

dilating any interspinous ligaments in a position in which the device is to be placed in the patient;

sizing a space between adjacent spinous processes; and

inserting the device between the adjacent spinous processes in a first position, the device comprising:

an elongate body comprising a first end portion, a second end portion, and a central support portion disposed between the first end portion and the second end portion;

wherein the first end portion comprises a wing member movable from the first position to a second position, the wing member including an outer surface and an inner surface that faces a central longitudinal axis of the elongate body;

wherein in the first position, the wing member serves as a ramp to engage and separate the adjacent spinous processes; and

moving the device to the second position to expand the wing member such that a portion of the inner surface of the wing member is capable of contacting a lateral surface of at least one of the spinous processes to limit movement of the device with respect thereto,

further comprising rotating the device to advance the device between the spinous processes and, the ramp distracts the spinous processes further apart from one another, until the device is rotated and advanced laterally into an implanted position with the distraction portion positioned on a contralateral side of the spinous processes and the spinous processes are fitted into the central support portion of the device.
