Abstract

The present invention provides methods and devices for treating endovascular disease. Vibrational energy is delivered to change compliance and increase permeability at the treatment area. To improve clinical outcomes, one or more therapeutic drugs may be delivered to the treatment area.

Claims (37)

1. A vibrational device for treating blood clots in a body of a patient, comprising:

an elongated ultrasound transmission member having a distal end and a proximal end, wherein the proximal end comprises a connector, wherein the connector is configured to be disposed outside the patient and connectable to a transducer, whereupon receiving energy from the transducer, the connector is configured to propagate vibrational energy from outside the body through the elongated ultrasound transmission member from the proximal end to the distal end of the elongated ultrasound transmission member to form surface waves at the distal portion of the elongated ultrasound transmission member;

an anchor member extending from the proximal end to the distal end of the elongated ultrasound transmission member;

a polymer sheath extending from the distal end to the proximal end of the elongated ultrasound transmission member that encloses the ultrasound transmission member and the anchor member; and

a catheter comprising only a single lumen and a fluid port coupled to the single lumen, wherein the catheter surrounds the polymer sheath enclosing the elongated ultrasound transmission member and the anchor member, wherein fluid is configured to be delivered through the single lumen to the fluid port and wherein the distal portion of the elongated ultrasound transmission member enclosed by the polymer sheath is at least partially extended through the single lumen and wherein the ultrasound transmission member is configured to transfer the surface waves to fluid adjacent the fluid port, and wherein the frequency of the vibrational energy generated by the surface waves is configured to cause dispersion of the fluid and a therapeutic agent delivered via the catheter.

2. The device of claim 1 , wherein the elongated ultrasound transmission member has a formed distal radiopaque tip.

3. The device of claim 2 , wherein a distal end of the anchor member is attached to the distal radiopaque tip of the elongated ultrasound transmission member and a proximal end of the anchor member is coupled to the connector.

4. The device of claim 2 , wherein a distal end of the anchor member is attached to the distal radiopaque tip of the elongated ultrasound transmission member and a proximal end of the anchor member is coupled to the proximal end of the elongated transmission member.

5. The device of claim 2 , wherein a polymer shell is positioned around the distal end of the elongated ultrasound transmission member and is at least partially surrounding the distal ends of the anchor member and the ultrasound transmission member.

6. The device of claim 1 , wherein a separate tip is attached on the distal end of the elongated ultrasound transmission member.

7. The device of claim 1 , wherein the anchor member is configured to ensure that the ultrasound transmission member can be entirely removed from the body even in the case of breakage of the ultrasound transmission member.

8. The device of claim 1 , wherein the elongated transmission member comprises a first portion having a first cross section and a tapered portion having a smaller cross section than the first cross section.

9. The device of claim 1 , wherein the elongated transmission member comprises a first portion having a first cross section and a plurality of tapered portions each having a smaller cross section than the first cross section.

10. The device of claim 1 , wherein the connector has a through-hole which accommodates a pin that holds a sonic connector in place.

11. The device of claim 10 , wherein the connector further comprises an absorber spaced apart from the sonic connector at a distance corresponding to a quarter wavelength of the vibrational energy.

12. The device of claim 11 , wherein the absorber comprises one or more polymer o-rings encircling the ultrasound transmission member.

13. The device of claim 10 , wherein the through-hole is configured to allow the sonic connector to have freedom to move relative to the pin.

14. The device of claim 1 , wherein a second catheter having an aspiration lumen configured to accept the vibrational device is positioned around the vibrational device for aspiration of clots through the aspiration lumen.

15. The device of claim 1 , wherein the vibrational device is configured to deliver less than 20 Watts of ultrasound energy at a frequency of 1 KHZ-10 MHZ.

16. The device of claim 15 , wherein the vibrational device is configured to deliver more than 15 Watts of ultrasound energy and less than 20 Watts of ultrasound energy.

17. A vibrational device for treating blood clots in a body of a patient, comprising:

an elongated ultrasound transmission member having a distal end and a proximal end, wherein the proximal end comprises a connector, wherein the connector is configured to be disposed outside the patient and connectable to a transducer, whereupon receiving energy from the transducer, the connector is configured to propagate vibrational energy from outside the body through the elongated ultrasound transmission member from the proximal end to the distal end of the elongated ultrasound transmission member to the distal portion of the elongated ultrasound transmission member;

an anchor member extending from the proximal end to the distal end of the elongated ultrasound transmission member;

a polymer sheath extending from the distal end to the proximal end of the elongated ultrasound transmission member that encloses the ultrasound transmission member and the anchor member; and

a catheter comprising a lumen and a fluid port coupled to the lumen, wherein the catheter surrounds the polymer sheath enclosing the elongated ultrasound transmission member and the anchor member, wherein fluid is configured to be delivered through the lumen to the fluid port and wherein the distal portion of the elongated ultrasound transmission member is at least partially extended through the lumen and wherein the ultrasound transmission member is configured to transfer the surface waves to fluid adjacent the fluid port, and wherein the frequency of the vibrational energy generated by the surface waves is configured to cause dispersion of the fluid and a therapeutic agent delivered via the catheter.

18. A system for treating blood clots in a body of a patient, comprising:

a transducer configured to be disposed outside the body of the patient and to produce vibrational energy;

an injection pump configured to be disposed outside the body of the patient and to supply a fluid comprising a therapeutic agent;

a connector configured to be disposed outside the body of the patient;

an elongated sonic transmission member having a distal end and a proximal end configured to couple with the connector to receive the vibrational energy from the transducer and to propagate the vibrational energy from the proximal end to the distal end of the elongated ultrasound transmission member to form surface waves at the distal portion of the elongated ultrasound transmission member;

an anchor member extending from the proximal end to the distal end of the elongated ultrasound transmission member;

a polymer sheath extending from the distal end to the proximal end of the elongated ultrasound transmission member that encloses the ultrasound transmission member and the anchor member; and

a catheter comprising a lumen and a fluid port coupled to the lumen, wherein the catheter surrounds the elongated ultrasound transmission member, wherein the catheter is configured to receive the fluid from the injection pump via the connector and to deliver the fluid through the lumen to the fluid port;

wherein the distal portion of the elongated ultrasound transmission member extends through the lumen of the catheter to transfer the surface waves to the fluid delivered adjacent the fluid port, and wherein the frequency of the vibrational energy generated by the surface waves is configured to cause dispersion of the therapeutic agent delivered via the catheter.

19. The system of claim 18 wherein the vibrational energy has a frequency of 1 KHZ-10 MHZ.

20. The system of claim 18 further comprising a foot actuated on-off switch configured to provide a first output signal to a signal generator that controls the transducer.

21. The system of claim 20 wherein the foot actuated on-off switch is further configured to provide a second output signal to the injection pump.
