Abstract

A method may include accessing a terminal branch of an ophthalmic artery through a face of a subject. Additionally, the method may include positioning a device within the ophthalmic artery of the subject and treating at least one of a blockage, a stenosis, a lesion, plaque or other physiology in at least one of the ophthalmic artery or a junction between an internal carotid artery and the ophthalmic artery.

Claims (32)

1. A method for treating glaucoma or age-related macular degeneration (AMD), comprising:

accessing an ophthalmic artery (OA) through an access vasculature site of a subject via a first device;

positioning the first device or a second device within the OA; and

removing material from the OA, to thereby treat glaucoma or AMD, wherein the material includes one or more of a blockage, a stenosis, a lesion, plaque, or other physiology in the OA, wherein removing the material from the OA includes inducing a vibration in a guidewire via an electric motor to loosen and/or dislodge the material.

2. The method of claim 1 , wherein removing the material from the OA further includes at least partially removing material from a junction between the OA and an internal carotid artery of the subject.

3. The method of claim 1 , wherein removing the material from the OA includes using an atherectomy device to physically remove at least a portion of the material.

4. The method of claim 3 , further including deploying a distal protection element downstream of the atherectomy device.

5. The method of claim 1 , wherein removing the material from the OA, includes opening a channel through the material to restore patency of the OA.

The method of claim 1 , wherein inducing vibration in the guidewire to loosen and/or dislodge the material includes inducing vibration in the guidewire and directly contacting the material with the guidewire, and/or application of a specific resonant frequency of vibration of the material to the guidewire.

The method of claim 1 , wherein removing the material from the OA includes expanding a balloon, inducing a vibration in the balloon via an electric motor to loosen or dislodge the material, and directly contacting the balloon with the material and/or application of a specific resonant frequency of vibration of the material to the balloon.

8. A method for treating glaucoma or age-related macular degeneration (AMD), comprising:

accessing an ophthalmic artery (OA) through an access vasculature site of a subject via a first device;

positioning the first device or a second device within the OA of the subject, wherein the positioned first device or second device includes an atherectomy device and at least one embolic protection element;

removing material from the OA via the atherectomy device, to thereby treat glaucoma or AMD, wherein the material includes at least one of a blockage, a stenosis, a lesion, plaque, or other physiology in the OA; and

during the removing, inhibiting passage of the removed material via the at least one embolic protection element.

9. The method of claim 8 , further comprising deploying the atherectomy device and the at least one embolic protection element distal of the material to be removed from the OA.

10. The method of claim 9 , wherein the at least one embolic protection element includes a distal protection element positioned distally of the atherectomy device.

11. The method of claim 8 , wherein the at least one embolic protection element includes a hydrogel configured to attract and adhere to the material removed from the OA.

12. The method of claim 8 , wherein the atherectomy device includes a basket, and wherein removing the material from the OA via the atherectomy device includes scraping the material into the basket.

13. The method of claim 8 , wherein the atherectomy device and the at least one embolic protection element are integral with the positioned first device or second device.

14. The method of claim 8 , wherein the positioned first device or second device includes a delivery sheath, and the atherectomy device and the at least one embolic protection element are mounted on the delivery sheath.

15. The method of claim 14 , wherein the positioned first device or second device includes a fluid delivery element for delivering fluid to the OA.

16. The method of claim 15 , wherein removing the material from the OA via the atherectomy device includes applying suction to the delivery sheath ( 1 ) to remove fluid delivered by the fluid delivery element and/or ( 2 ) to remove the material from the OA.

17. A method for treating glaucoma or age-related macular degeneration (AMD), comprising:

accessing an ophthalmic artery (OA) through an access vasculature site of a subject via a first device;

positioning a first embolic protection element within an internal carotid artery (ICA) of the subject via the first device;

positioning a second device within the OA of the subject, the second device including an atherectomy device and a second embolic protection element located distal of the atherectomy device;

deploying the atherectomy device and the second embolic protection element in the OA;

removing material from the OA via the atherectomy device, to thereby treat glaucoma or AMD; and

during the removing, inhibiting passage of the removed material downstream into the ICA and the OA via the first embolic protection element and the second embolic protection element, respectively.

18. The method of claim 17 , wherein the atherectomy device includes a basket, and wherein removing the material from the OA via the atherectomy device includes scraping the material into the basket.

19. The method of claim 17 , wherein deploying the atherectomy device and the second embolic protection element in the OA includes expanding the atherectomy device and the second embolic protection element in the OA by retracting a sheath of the second device.
